What the care home does well

101 Prospect Mount Road is comprised of three separate units, each unit having a different function. The service provided is pertinent to the individuals in each unit and the staff were seen to be respectful and patient at all times. In the rehabilitation unit service users are encouraged to be as independent as they can and staff work with health professionals in order that they can go home as quickly as they can. A service user on this unit said that staff were `brilliant`. The service users on the respite unit said that the staff were `wonderful and always cheerful and pleasant`. The staff in the dementia care unit were seen to be patient and respectful of the service users and there was clearly a good relationship within the unit.

What has improved since the last inspection?

Since the last inspection the service users admitted to the respite unit have usually had a care plan and an assessment provided by a care manager before they go into the home. This means that staff are aware of the help people need when they arrive. The management team also carry out their own assessments of the service users with dementia and this enables them to provide the right service for those people admitted to that unit.

What the care home could do better:

The management team need to ensure that the administration supporting the work done by the resource workers is in place from the assessments to the day-to-day recording.

The home is divided into three units; an intermediate care unit, a dementia care unit and a unit that takes respite care. There are currently five long stay service users within the respite care unit. No further long stay admissions are planned. North Yorkshire County Council owns the home.The home is located on the outskirts of Scarborough. There is a bus service from outside the home into the town centre. All the bedrooms are used as single accommodation. The home has accessible grounds and the EMI unit has a secure garden area directly from the unit. This inspection including preparation time took place over 15 hours. A tour of the building was conducted. A number of the service users records, and records about the management of the home were inspected. Service users in the rehabilitation unit and on the respite unit were able to share their experiences in the different units, whilst time was spent observing the service users in the dementia care unit. Several staff, a district nurse and a GP were spoken with during the inspection. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 The service users can be assured they will be admitted to the right unit and their care needs will be met. EVIDENCE: Service users admitted to the dementia care unit have an assessment provided through the care management process and a member of the management team carries out their own assessment. The service users admitted to the rehabilitation unit have a multi-disciplinary assessment provided through the local health team. Due to the nature of the respite unit a care manager usually provides an assessment to the home prior to the placement. However on the day of the inspection a service user whose need for respite care was described as ‘urgent’ was admitted and the care manager was doing the assessment and care plan whilst he was being admitted. The home should ensure that an assessment of need is available for all service users not admitted on an emergency basis prior to their placement. Discussion with the team manager confirmed that where a respite placement is made that is not appropriate then an alternative placement will be made within 48 hours of a request made. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, and 9 The health, social care and personal needs of the service users are met. EVIDENCE: Service user files were seen in the dementia care unit, the rehabilitation unit and the respite unit and they all contained an appropriate care plan. The care plans on the rehabilitation unit are based on a programme of improvement and were pertinent to the individual. These plans are reviewed weekly in a multi disciplinary meeting. The care plans on the dementia care unit are called Promotion of Independence Plans and contained information about the service users needs but also their support networks outside of the home. There was evidence to show these plans are reviewed on a monthly basis. The service users admitted to the respite unit had a basic care plan and this was reviewed on subsequent stays. Contact sheets are maintained for all the service users but the recording on these documents did not appear to be done on a regular basis. The service users spoken with said that they could see their GP on request and staff assisted them in maintaining appointments to see other health professionals. A GP visiting the home said that the staff provided them with assistance as necessary during a visit and all visits were made in private. The handling, storage and administration of the medication was seen to be appropriate for the dementia care unit and respite unit. The service users in the rehabilitation unit manage their own medication and have lockable facilities in which to store their tablets. Service users are assessed as their capabilities in handling medication with a view to going home and on several occasions a monitored dosage system has been provided to the service user prior to their discharge. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 and 13, The service users are able to keep their own routine within the home and join in with social activities. EVIDENCE: The service users spoken with during the inspection confirmed that the routines of the home were flexible. The service user population within the respite unit is ever changing, yet the staff were knowledgeable about the service users likes and dislikes. Service users using the respite unit confirmed that the staff took time with them during their admission to the home. The service users in the rehabilitation unit said that their routines were dictated by their recovery plan. The service users spoken with did not mind this, as they were keen to get home. The routine in the dementia care unit was seen to be flexible and supportive of the service user. There is a visitor’s policy in place and service users can receive visitors at any time. They would be supported if they did not want to see anyone. Information about the visitor’s policy is available in the Service Users Guide. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 and 18 Service users were confident that any complaints or concerns they had could be raised and acted upon. Service users are protected from possible abuse through staff training. EVIDENCE: North Yorkshire Social Services has a detailed complaints policy. This policy is provided to all the service users and is displayed around the home. There have been no complaints made to the CSCI but several have been made to the home in the last 12 months. Records showed that these were dealt with to the satisfaction of the service users and their relatives. Service users and their relatives spoken with said that if they had any problems they would go to the manager. There is an Adult Protection Policy in place. Staff spoken with had received training in Adult Protection issues and they had a good understanding of their responsibilities if they believed any inappropriate behaviour was taking place. Policies are also in place regarding the management of service users money. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 and 26 The service users live in a home that is clean and well maintained. EVIDENCE: During the inspection several areas of the home were seen and found to be clean and odour free. The communal areas were comfortable and warm. The furniture was said to be comfortable by the service users. The home is located close to some shops and is suitable for its stated purpose. The grounds are well maintained and accessible to the residents. There is a separate garden area for the dementia care unit. The garden area is secure and allows for unsupervised access by the service users. There is currently a major improvement programme going on in the home to allow for the development of day services and any area of décor affected by this work will be made good on its completion. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27. The service users needs can be met by the current staffing levels. EVIDENCE: The staffing levels allow for three staff to be on the respite unit, two staff on the dementia care unit and two staff on the rehabilitation unit over the day. During the nights there are three waking night staff. Service users spoken to on the rehabilitation unit said the staff were very helpful and positive in their approach. They respected their privacy at all times. Service users in the respite unit were equally positive about the staff although felt that they did not always have the time to spend and talk with them. However, there is a programme of planned activities including when possible trips out. The dementia care unit although not full appeared to be quite busy as the dependency levels of the service users in residence was high, one service user was provided with their own carer during the day in addition to the established staff. Whilst it is unlikely extra permanent staff will be provided the management team must monitor the dependency levels to ensure that any further admissions made do not compromise the service currently provided. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 38 The health and safety of the service users and staff is promoted. EVIDENCE: The staff spoken with confirmed that they had received training in health and safety, first aid, back care, food hygiene, COSHH, and fire training. Evidence of this was seen in the staff files. The equipment used in the home is serviced at the prescribed intervals. Health and safety notices were displayed around the building. Accidents are properly recorded and where necessary reported to the Commission for Social Care Inspection. Information from the accident records is used in the care planning process.
