The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home is divided into four separate units. Service users and relatives of service users in all three residential units considered that they were provided with good quality care from the staff team. The day unit was not inspected. The home is pleasantly decorated and one service user who had been there for ten years stated "I wouldn`t be here is it was not a very good home with good staff and good food. I have no complaints at all"

What has improved since the last inspection?

The number of long stay service users has been reduced from five to three. No further long term service users will be admitted to the main part of the home which is to become a short term respite care unit. Decorations have been carried out and part of the residential care unit is now a separate EMI day unit for twelve people and opens from Tuesday to Saturday each week.

What the care home could do better:

A review of the staffing hours for the dementia unit should be undertaken to ascertain if two members of staff on duty are sufficient to care for up to twelve service users with dementia whose dependency levels appear to be increasing.

The home is divided into four units; an intermediate care unit, a dementia care unit, a unit that takes respite care and an EMI day unit. There are currently three long stay service users within the respite care unit. No further long stay admissions are planned. North Yorkshire County Council owns the home. The home is located on the outskirts of Scarborough. There is a bus service from outside the home into the town centre. All the bedrooms are used as single accommodation. The home has accessible grounds, the EMI unit has a secure garden area directly from the unit and there is a separate enclosed outside area from the EMI day care unit. The evidence used to inform this report was obtained from the information documented in previous inspections reports, a pre-inspection questionnaire completed by the registered manager of the service, a site visit, completed survey forms and discussions with six service users, five members of staff and two relatives. Prior to the inspection, ten survey forms were sent to service users and two were returned, five survey forms were sent to GPs and two were returned and four were sent to care managers and one was returned. This unannounced inspection took place on the 5th February 2007 and commenced at 9.15am. A number of records were inspected including service users’ assessments, care plans, medication and health and safety records. A tour of the home was made with a team manager and staff were observed interacting with a number of service users. Weekly charges on the date of the inspection ranged from £94.45 to £368.90. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 3 and 6. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The assessments prior to admission are comprehensive and provide informed decisions about moving into the home. EVIDENCE: A comprehensive care management assessment is made prior to a potential service user being considered for admission to the home. Service users admitted to the intermediate care unit are assessed by a multi-disciplinary health team and are placed in the unit for rehabilitation work to allow service users to be able to return to their own homes, if that is possible, at the end of their stay in the intermediate care unit. The unit operates with regular input from the health teams. Service users in the unit were very pleased with the input of the staff in attempting to assist them to return to their own homes. Over 90 of people admitted are discharged home after their rehabilitation. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 and 10. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. There are good systems in place to ensure that the health needs of service users are met. EVIDENCE: Care plans were examined during the inspection. The plans continue to provide information that enables staff to care for service users in an appropriate way. The files are reviewed as necessary and at least every month in the main home and in the dementia unit. Care plans in the intermediate care unit are reviewed frequently and there is a meeting about service users each week with health service staff. Staff were observed treating service users with respect and dignity. All service users spoken to stated that they were well treated by the staff team. Some service users self medicate. There is a monitored dosage system for other service users who require medication. People admitted for short-term care will bring either a monitored dosage system in with them or tablets in the original boxes dispensed by the pharmacist. Observation of a member of staff administering medication showed that the appropriate action was taken and records maintained. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 and 15. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Visiting arrangements are flexible allowing service users to maintain good and regular contact with family and friends. Dietary needs of service users are met. EVIDENCE: Service users spoken to stated that their routine was flexible. Activities were arranged within the home and outings to places of interest in the community. Service users who wished to participate in the activities stated that they enjoyed taking part and particularly liked the trips out of the home. Specific activities are arranged in the rehabilitation unit to assess the skills that service users would require if they returned home. Some service users stated that they enjoyed going out for car rides with their relatives on a regular basis. Service users stated that there was also choice available at meal times and that they enjoyed their meals. The menu is varied according to the season of the year and fresh food is used whenever possible. Visitors are encouraged to see their relatives and friends in the home at anytime and some service users stated how they looked forward to their visitors coming to see them. The relatives spoken to considered that the service users were very well cared for in a pleasant environment. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. There are satisfactory complaints and abuse procedures in place and staff are aware of what action to take if any complaints or alleged abuse situation occur. EVIDENCE: There is a complaints policy. One complaint has been made since the last inspection. This was recorded and investigated within the time scale of the policy. A relative of a service user stated “I would always ask who to approach if I was not satisfied with the care my relative was receiving. I have, however, always been very satisfied with the care that is provided within the home.” The home has a copy of the York and North Yorkshire Vulnerable Adults Policy and staff spoken to knew what action to take in the event of a suspected abuse situation occurring. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The standard of the environment of the home is good providing service users with a homely place in which to live. EVIDENCE: The home is clean and comfortable. The communal areas are suitable for the needs of the different service users who use separate parts of the home. Service users spoken to stated that they were satisfied with their own accommodation. The grounds are accessible and well maintained. There is a separate secure garden area for the use of service users in the dementia unit. This allows service users to walk in the gardens anytime that they wish. The newly developed day unit also has a separate outside garden area. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29 and 30. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The service users needs are met by the staffing levels in the main part of the building and in the rehabilitation unit. The staffing hours in the dementia unit need reviewing. Staff receive appropriate training and new staff are only appointed after appropriate employment checks have been made. EVIDENCE: The staffing levels for the main unit and the rehabilitation unit are satisfactory. A relative of a service user stated, “Staff are extremely helpful”. A GP who visits the home stated, “The staff are excellent and caring and communication is good. I have no concerns”. A review of the staffing hours in the dementia unit needs to be undertaken to ascertain if two staff on duty are sufficient for up to twelve service users whose dependency needs appear to be increasing. Service users spoken to stated that the staff respected their privacy and were polite at all times. The home has a staff development programme and 78 of care staff have now obtained an NVQ Level 2 in care. The employment files checked showed that the necessary checks are made before a new member of staff commences work at the home. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35 and 38. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The service users live in a well managed home with appropriate health and safety standards maintained. EVIDENCE: The registered manager is experienced and qualified in the care of older people. Service users stated that the senior staff were available to discuss any issues that they wanted to discuss. There is a quality assurance programme where the home assesses the service they provide against the National Minimum Standards for older people on a three monthly basis. Residents meetings, staff meetings and team briefings are held and quality is incorporated into the meetings. Personal allowance money is kept in the home for some service users.
