The home is divided into three units; an intermediate care unit, a dementia care unit and a unit that takes respite care. There are currently five long stay service users within the respite care unit. No further long stay admissions are planned. North Yorkshire County Council owns the home. The home is located on the outskirts of Scarborough. There is a bus service from outside the home into the town centre. All the bedrooms are used as single accommodation. The home has accessible grounds and the EMI unit has a secure garden area directly from the unit. This is an overview of what the inspector found during the inspection. This inspection including preparation time took place over 10 hours. A tour of the building was conducted. A number of the service users records, and records about the management of the home were inspected. Service users in the rehabilitation unit and on the respite unit were able to share their experiences in the different units, whilst time was spent observing the service users in the dementia care unit. The registered manager must look at the support arrangements at lunchtime and all other meal times so that staff can properly assist the service users. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6 Service users receive appropriate support to help them maximise their independence so that they can return home. EVIDENCE: Dedicated lounge / dining, bathroom and bedroom facilities are available on the second floor of the building for up to 12 residents who are admitted for intermediate care. However, on the day of the inspection the rehabilitation unit was temporarily located on the first floor of the building using separate facilities to the respite service users. A separate staff team including support from physiotherapy and occupational therapy staff cares for this client group. Inspection of the unit’s facilities and records was made and were found to comply with required standards. The staff of this unit spoke enthusiastically about the unit and they are clearly committed to successfully assisting people to return to their own homes. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7 and 10 The service users personal care needs are identified in a plan and staff treat the service users with respect. EVIDENCE: The care plans for service users on the respite dementia care and respite unit continue to provide pertinent information about the person they are about. The staff now record on the contact sheets on a regular basis and this allows for a more comprehensive review of the care plans. The staff on the dementia care unit were seen to be treating the service users with respect at all times. They gave them time and encouragement to undertake any tasks the staff asked them about. The service users on the respite and rehabilitation unit were positive in their praise for the staff. They said that staff always treat them with respect and try to preserve their dignity at all times. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14 and 15 The service users are able to keep their own routine within the home and join in with social activities. The service users have a varied and balanced diet EVIDENCE: The service users spoken with said that they are able to follow their own routines within the routines for the home. They are able to choose where they want to be in the home and can go back to their rooms at any time. The service users spoken with expressed satisfaction at the quality and quantity of the food provided. They confirmed that choices are available at each meal. The menu was examined and appeared to provide a balanced and nutritious diet. The cook explained that local fresh produce is used and they are able to vary the menu to include seasonal foods. The meal observed in the dementia care unit was chaotic. There were two staff with 11 service users. One member of staff served up dinner whilst the other member of staff assisted the service users. The service users were unsettled and one member of staff could not provide the support and assistance required, consequently some of the service users were leaving the dinning room and unsettling the others. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28, 29 and 30. Staff that receive regular training support the service users and are experienced carers. Staff are employed following a thorough recruitment process. EVIDENCE: There has been a low turnover of staff in the last twelve months. The staff files of three staff members employed in that time were inspected and were found to contain all the necessary documentation as required by the Care Homes Regulations 2001 There is 71 of the staff team with a National Vocational Qualification level 2 and the remaining staff are currently in the process of completing this course. Staff also have access to induction, foundation, back care, challenging behaviour, first aid, fire awareness, and food hygiene. The manager also utilises the expertise of the visiting professionals to the rehabilitation unit and organises awareness sessions other health related issues. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33 and 35. The service users live in a well managed home where the service is regularly checked to ensure the standards don’t drop. EVIDENCE: The registered manager has been managing the home for several years and has completed the Registered Manager’s Award. The service users spoken with said that the manager was always available and were confident that she would deal with any concerns they may have positively. Relatives also spoke highly of the manager and expressed confidence in her North Yorkshire Social Services has introduced a quality assurance programme. This involves the home assessing the service they provide against the National Minimum Standards for Older people every three months. This information is then used to develop an action plan to improve services. Service user meetings, staff meetings, team briefings, and the quarterly

Staff have received training in the quality assurance system. The personal allowance for several service users is held in the office and the records were checked and found to tally with the cash held. Receipts are obtained for any purchases made.
