Conditions of registration: Date of last inspection 24th November 2005 Brief Description of the Service: Rosehill House is a privately owned care home providing care and accommodation for older people, up to 10 of whom may have dementia. It is an older property with an extension. The home changed ownership in July 2004 but continuity for the service users has been maintained as the manager has become the registered manager and the staff have remained constant. Accommodation is offered on two floors, which are connected by a shaft lift and stairs. The majority of rooms are for single occupation and some are en suite. There is a choice of communal sitting areas and a dining room on the ground floor. Toilets are sited throughout the property. Externally the gardens are well-kept and accessible to service users. A summerhouse in the garden offers a place to sit in good weather. Adequate car parking is available to the side of the home. In addition to providing residential care Rosehill House provides a limited number of day places to local people. This is an overview of what the inspector found during the inspection. This unannounced inspection took place on 9th March 2006, over a period of approximately 5 hours. The inspector met with the registered manager, staff, service users, and a visitor to the home. The inspector toured the building, and inspected care documentation. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3, 4, 5 Prospective service users are assessed fully prior to admission to the home. The registered manager and care documentation demonstrate a good understanding of the care needs of the service users. There is opportunity for prospective service users and their representatives to visit the home prior to admission. EVIDENCE: The care documents for the most recent admission show that a full care needs assessment was carried out prior to admission. The care documentation is under review, but shows a good understanding of the care needs of the service users. The registered manager advised the inspector that there is always opportunity for prospective service users and their representatives to visit the home, and that this is included in the home’s Service User Guide. Some admissions result from previous respite stays and others from word of mouth in the local community. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 8, 9, 10 The home and service users benefit from close liaison with health care professionals. The home operates an appropriate medicine procedure. Staff were observed to respect service user’s right to privacy. EVIDENCE: The registered manager is currently reviewing the care documentation and is making progress in this area. The staff monitor and assist with the hygiene and health care needs of service users; other help being sought from the community nurse or GP as needed. The care documentation, supported by information provided by the registered manager, shows that service users have ready access to health care professionals as necessary, and specialist equipment can be arranged via these links. A number of service users currently have Community Psychiatric Nurse’s visiting regularly. A chiropodist visits regularly. Opticians visit annually, but do not see all the service users. The registered manager should arrange for all the service users to receive an annual eye test. Medicine Administration Records were generally in order and an appropriate medicine procedure is in operation at the home.

Two initials are entered when manually transcribing prescriptions onto Medicine Administration Records 2. Medicine Administration Records should be rewritten as necessary to avoid confusing entries (one medicine was seen to have twice a day and four times a day instructions) 3. The medicine trolley is full and consideration should be given to purchasing a larger trolley in conjunction with the provision of additional storage cupboards in this area. Staff were seen to interact with service users in a manner that respected the service user’s right to privacy, and this was confirmed by the service users. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 14 The home provides various activities, some organized by the staff, others by outside entertainers. Everyone is encouraged to join in although the wish not to do so is respected. EVIDENCE: Rosehill House welcomes visitors and people are able to receive visitors in private if they wish. Service users are encouraged to go out with families and friends if they wish. The home provides various activities, some organized by the staff, others by outside entertainers. All service users are encouraged to make their own arrangements for managing their financial affairs, with the assistance of families or friends if necessary. The home will hold money if asked. Weekends are kept free for service user visitors, but activities are arranged every other day of the week and are changed regularly. The forthcoming weeks activities are displayed in the entrance to the home. Some of the service users spoken with said they enjoyed the activities offered, but all confirmed that they are free to choose whether or not to join in. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16, 17 The home has a comprehensive complaints procedure available to service users and relatives. The procedure for enabling service users to vote and seek advocacy help is outlined in the statement of purpose & service users guide. The registered manager said that any service users who did choose to vote usually did so by post. EVIDENCE: Neither the Commission for Social Care Inspection nor the home has received any complaints since the last inspection. The complaints policy is made available to all service users and is also displayed in the entrance to the home. Service users are registered on the electoral roll and have the opportunity to participate in the civic process. Mail is delivered unopened, though assistance is available if so desired. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 20, 22, 23, 24, 25 The home provides appropriate sitting, recreational and dining space. Specialist equipment is arranged via liaison with health care agencies such as District Nurses. Service user rooms are clean and pleasant and personalised to varying degrees. The home is safe and comfortable throughout. EVIDENCE: Communal space is provided in one large lounge, a smaller ‘quiet’ one and a conservatory. People are free to move between the areas as they wish. The dining room is a good size with people being sat at small tables in compatible groups. Access throughout the home is assisted by the provision of handrails and grab handles at strategic points. A stair lift is currently being fitted to assist access via six steps between the 1st and 2nd floor. The home offers 1 double room and 29 single rooms (1 en-suite). Service user rooms are clean, comfortable, and personalised to varying degrees. Since the last inspection the registered manager has audited staff hand washing facilities and provided paper towels and liquid soap in all lavatories and bathrooms. The registered manager is also drawing up a cleaning schedule for the home.

Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 30 Staff numbers and skill mix, and training available reflect a commitment to service user welfare and staff development. EVIDENCE: At the time of the inspection the registered manager and 4 care staff were on duty, though one member of staff had called sick so there would normally have been 5 on duty plus the registered manager. The registered manager has obtained National Training Organisation induction programmes, which will be introduced for all new staff. There are 20 care staff (including the registered manager). Eleven staff have been enrolled on a forthcoming food safety training programme. Seven staff are undertaking NVQ Level 2 or above, and 10 staff have already achieved this qualification. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 35, 36 Service user’s financial interests are safeguarded though improvements could be made in the recording system in operation. Staff are not appropriately supervised. EVIDENCE: Service user financial records are accurate, current, and stored securely. The registered manager should ensure that there is a receipt to support the record for all transactions. The registered manager must arrange for all staff to receive formal documented supervision at least six-times a year.
