This inspection was carried out on 17th May 2005.

What the care home does well

Rosemount provides the people who live there with a comfortable and well maintained home, a relaxed atmosphere and friendly well trained staff. Residents spoken to during the inspection commented on their satisfaction with the care received and the accommodation, comments included " I like the telly, dinners and friends ", " Food marvellous, lovely staff and company ", " You even get your laundry done for you", " Satisfied with bedroom, able to bring own things ". Staff spoken to during the inspection who had worked at the home for several years presented a caring attitude. One staff member commented " Lovely residents and staff ". From speaking to the manager, staff and residents it was clear that there is a good social programme of activities available if a resident wishes to join in.

What has improved since the last inspection?

Since the last inspection the new care planning documentation has been implemented for all residents. The carpet in three bedrooms has been renewed.

What the care home could do better:

The manager must continue with ongoing programme to improve the care plan content, including documenting identified risks and the intervention required to maintain residents safety, where a risk has been identified.

It is not registered to accommodate persons who require nursing care. The home also provides an integrated day care service for up to two day care service users per day. The home was opened in 1984 and is owned by the Rosemount Trust. The home is a two storey building situated in its own grounds, located in the centre of Consett, close to local amenities. Personal accommodation is provided on both floors. Communal space and service areas are located on the ground floor. Throughout the home is well maintained and equipped offering a domestic style environment. Bedrooms are individual in size and layout, personalised, well equipped, furnished and decorated to a good standard. There is a range of lounges, a sun lounge and a separate dining room located on the ground floor. This inspection took place on 17th May 2005 over a period of five hours. Time was spent during the inspection talking to residents and staff. A number of records were examined and a tour of the building was carried out. A recommendation made at the last inspection regarding the documentation of risk management processes remains outstanding. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2, 3, 4 and 5 The home has a clear and comprehensive admission procedure in place to ensure the care needs of the prospective residents are met. EVIDENCE: Each resident on admission to the home is issued with a copy of the Terms and Conditions of Residency. This document makes clear the home’s responsibility to the resident and the responsibility of the resident while living in the home. This document was seen to be signed by the resident or their representative as to their agreement. The manager said all prospective service users have a pre- admission assessment. Care plans examined contained evidence of pre – admission assessment of need, carried out by people trained to do so. No service users are admitted to the home without a pre – admission assessment , to ensure the home has the capacity to meet their individual needs. A resident spoken to said she had visited prior to admission with her daughter and been invited to have a meal with other residents who lived in the home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 10 and 11 The care plan format is good, but the risk management strategies need to be developed to ensure the protection of residents living in the home. Staff have a good understanding of residents support needs, this is evident from the positive relationships formed between staff and residents. EVIDENCE: Since the last inspection the revised care plan format has been implemented fully. Examination of three care plans showed that further development is required for documenting risk management strategies in line with identified individual needs of residents. (outstanding since the last inspection). The registered manager was able to demonstrate that the home promotes and maintains residents health and ensures access to other health care services to meet assessed needs. Staff during discussion were able to demonstrate that they respected residents’ privacy and dignity when assisting with personal care and in the event of death occuring. Residents spoke positively about the caring attitude of staff and that they were encouraged to do as much for themselves as possible. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13 and 14 Social activities are well managed, creative and provide daily variation and interest to people living in the home. Links with the community are good enriching the residents’ opportunity for social interaction. Residents are supported by staff to make lifestyle decisions with an appropriate range of information available. EVIDENCE: Residents and staff spoke positively about the flexible routines of daily livng in the home. Social activities are made available to suit residents preferences, and capacity. A resident spoken to said she particuarly enjoys ” musical entertainment ”. The home is well established in the area. The manager said neighbours occasionally drop in for a chat and a cup of coffee with residents. A visitors policy is displayed in the main foyer providing information on visiting the home. Residents spoken to said their visitors are always made welcome by the manager and staff. Information on advocacy services is displayed in the home to give residents an alternative choice if they wish to have someone speak on their behalf to make their views known. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16,17 and 18 The home has a satisfactory complaints system with some evidence that residents views are listened to and their rights protected. Staff are given the skills and knowledge through training to ensure residents are protected from abuse. EVIDENCE: A clear complaints policy and procedure is in place to enable residents to raise any concerns they have about the care service delivery. A resident spoken to said she had not had any reason to complain but would speak to the manager if she felt dissatisfied in any way. Since the last inspection 1 x complaint had been received by the home from a relative with reference to a housekeeping issue. This complaint was partially upheld. At the recent general election a postal system of voting was made available to residents to enable them if they so wished to participate in the electoral process. Since the last inspection the majority of staff working in the home had attended training on “No Secrets”. Staff spoken to were able to demonstrate an awareness of POVA (Protection of Vulnerable Adults) and said they would report any incident of abuse observed in the home. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20 21,22, 23, 24, 25 and 26 The standard of the environment in this home is good providing residents with an attractive and homely place to live, as well as meeting the collective needs and changing needs of the residents who live there. EVIDENCE: The home was observed to be comfortable, traditional in style, located appropriately, well maintained, accessible and safe. Since the last inspection carpet had been renewed in several bedrooms. A resident who had recently moved into the home said “I am very satisfied with my bedroom, able to have my own things around me ”. Bathing facilities in the home were adequate in meeting the collective needs of the residents who lived there. Three bedrooms have ensuite facilities extra to the communal facilities for sole use of the resident who lives in that particular room. The home was viewed to be commendably clean, well ventilated and no unpleasent odours present.
