Conditions of registration: Date of last inspection 17th May 2005 Brief Description of the Service: Rosemount is a well established registered care home providing personal care and accommodation for up to 17 older persons in the category of (OP). It is not registered to accommodate persons who require nursing care. The home also provides an integrated day care service for up to two day care service users per day. The home was opened in 1984 and is owned by the Rosemount Trust. The home is a two storey building situated in its own grounds, located in the centre of Consett, close to local amenities. Personal accommodation is provided on both floors. Communal space and service areas are located on the ground floor. Throughout the home is well maintained and equipped offering a domestic style environment. Bedrooms are individual in size and layout, personalised, well equipped, furnished and decorated to a good standard. This is an overview of what the inspector found during the inspection. This unannounced inspection took place on the 28/10/05 over a period of 6 hours. During that time the inspector talked to service users staff and a number of records were examined. There were no outstanding requirements from the last inspection. The area regarding risk assessments in individual’s care plan still need to be developed further inline with the identified need in the plan of care to ensure where required the element of risk to the service user is prevented. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standard 1 The homes Statement of Purpose and Service User Guide are good providing service users and prospective service users with details of the services the home provides enabling an informed decision about admission to be made. EVIDENCE: The home had in place a comprehensive Statement of Purpose and Service User Guide. These documents as well as containing up to date information about the service, also included recent comments on the quality of the care and service delivery from relatives of service users living in the home. Comments made included: “ I visit Rosemount 3 to 4 times per week and I am always made welcome regardless of the time of day”, “ Everything is excellent her I have no fault with it at all. I would commend this home to anyone”. This information and real life comments enable prospective service users and their relatives to decide if this is where they wish to live. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standard 7 and 9 Care planning has improved but needs to be developed further in the area of risk assessment to ensure individual service users are protected from harm. The medication at this home is well managed promoting good health. EVIDENCE: At the last inspection a recommendation was made to develop further the new care plan format to include risk management strategies in line with identified needs where the service user may be at risk. On examination of care plans some progress had been made but further development in this area is recommended, to ensure the safety of service users is maintained. The policy and procedure for the safe handling of medication in this home promotes the health of service users. Good recording procedures where in place for the receipt, handling administration, recording, storage and disposal of medicines. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. The intended outcomes for Standards 27 – 30 are: 27. 28. 29. 30. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standard 27, 28, 29 and 30 The home is adequately staffed with the skills and abilities to provide a good service to the people who live there and other visitors to the home. The policy and procedure for recruitment of staff is good ensuring service users are protected from harm or abuse. EVIDENCE: Staff rotas examined showed that staff are employed in adequate numbers and skill mix to meet the needs of the people living in the home. A service user spoken to during the inspection commented that there was always staff available to assist saying, “ Can I help you with anything, but I like to do as much as I can for myself”. The home is fully staffed at present and has low staff turn over which results in consistency of care for the people who live in the home. Discussion with the manager and evidence available showed that many care staff have achieved or commencing NVQ2 in Care. The manager is positive towards equipping staff with the necessary skills and abilities to provide a good standard of service to the people who live in and visit the home. The people living in the home are protected from abuse or harm by robust recruitment procedures in place. Evidence was available to show that all staff had a current Criminal Records Bureau check and two satisfactory references had been obtained prior to commencement of employment.

Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 31, 32, 33, 34, 35, 36 and 37 The manager has a clear vision for the home, which she communicates effectively with service users, staff and relatives. The home regularly reviews aspects of its performance through a programme of self-review and consultation with service users, staff and relatives. Staff are supported appropriately by the manager to ensure they are equipped with the necessary skills to carry out their role of responsibility to the people who live in the home. EVIDENCE: The manager is a qualified nurse and has achieved the Registered Managers Award as well as having many years experience in working with older people. The manager described her management style as being a good listener, approachable and having a good rapport with relatives and other health professionals who visit the home.
Rosemount DS0000007499.V261168.R01.S.doc Version 5.0 Page 16 The home has a quality self-monitoring system in place to ensure a good quality service is maintained. Evidence was available to show that service user and staff have the opportunity through regular meetings to make their views known on the service offered in the home. The home has an annual Business Plan in place and Reg 26 visits are carried out by the provider on a monthly basis and forwarded to CSCI. The home only holds small amounts of money as requested by service users on their behalf. Records viewed for money held on behalf of service users in the home were maintained appropriately with receipts evident for all financial transaction and signed for by the service user and the manager. A system of formal supervision of staff is in place ensuring all aspects of the procedure is followed including all aspects of care, philosophy of care and career development. This process ensures that staff have the opportunity to discuss all aspects of their role of responsibility with their line manager to ensure they are competent and provide a good standard of service to the people who live in and visit the home.
