The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

A qualified manager who is experienced in providing care for older people runs the home. There is a pleasant atmosphere in the home created by a lively caring staff team and a homely decor. Residents` bedrooms are individual in style with many personal items of furniture and memorabilia. Residents and visitors praised the home and the standard of care provided.

What has improved since the last inspection?

Work is ongoing to improve the fabric of the building. Any shortfalls in training have been identified and a programme put in place. A quality assessment document has been purchased.

What the care home could do better:

A new system for assessing residents` needs has been introduced. This has resulted in a lack of information for staff to ensure good care is provided. Although a quality assessment system has been purchased, no work has been done as yet to assess the quality of care provided.

It is situated on the seafront between Cullercoats and Whitley Bay and has excellent views of the seafront towards St. Marys Island. The home is on three floors. A passenger lift, two short staircase lifts and some ramps, provide access to all areas of the home. The home is registered to provide care to 40 older people, 7 of whom have a dementia. There are 34 single bedrooms, 16 of which have ensuite toilet and hand basin. There are 3 shared bedrooms, 2 of which have ensuite facilities. The communal facilities consist of; a lounge, two dining/lounge areas, a dining room with adjoining conservatory and an amenities room used for crafts and hairdressing. The home is well maintained with a large landscaped garden at the front. Weekly fees are in the range £356 - £385. This was the home’s first annual unannounced key inspection visit. The inspection was conducted over two separate days and took 10 hours. Time was spent talking to the manager, some care staff, the cook and several residents and their visitors. Some of the home’s care records were reviewed and the systems that maintain residents safety. Questionnaires were provided for residents and visitors to the home and information provided by professional visitors is used in the production of the report. Nineteen responses were received. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 3, 6. A good range of information is available to prospective service users and needs assessments are conducted before a placement is accepted. Intermediate care is not provided. EVIDENCE: The home’s Statement of Purpose and Service Users Guide are currently being upgraded to further improve the information provided to anyone interested. A home’s brochure has been produced with colour pictures and brief information. A quarterly newssheet is produced for residents and visitors to keep them informed about events in the home. Residents and visitors spoken with during the inspection were all happy with the level of information provided. One recently admitted resident suggested that information informing new residents of daily routines and hairdressing days would be useful. The manager is considering this useful suggestion. A senior member of staff prior to a place being offered assesses each resident. This is to limit the possibility of unsuitable placements being accepted. Four files reviewed each contained this information. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9, 10. Quality in this outcome area is adequate. This judgement has been made from evidence gathered both during and before the visit to this service. Individual care plans do not support the home’s provision of health or social care. Substantial health assessments are not available for all residents. A reasonable system for administration of medicines is in place. Residents’ privacy and dignity are respected. EVIDENCE: Residents seen during the inspection were in good health with their personal care needs addressed. Residents and their visitors spoken with were content with the care provided. Appropriate equipment such as pressure mattresses are available in the home to support residents physical needs. Four care plans reviewed did not provide the level of information expected to instruct staff in the provision of good health and personal care. The information was of varying quality, with some health assessments not done and others with no personal care details provided. The home are introducing a new format of care planning and this has left many case files short of the information necessary to ensure staff provide appropriate care. It is expected that plans will be introduced urgently for those residents whose needs are greatest. St Annes Residential Care Home DS0000063195.V295186.R01.S.doc Version 5.2 Page 11 Throughout the day the staff team demonstrated good practice in relation to respecting residents privacy and dignity. This is supported by comments made by residents. Residents have limited privacy when using the public phone but this has been addressed by the proprietor who has installed a system that allows residents connect to the public phone system from their bedrooms. The system for recording and administering medicines in the home was checked. An office in the home is being converted to store medicines and additional lockable units are being purchased. Currently because of a lack of storage residents are sharing a liquid medication. This is not good practice and should cease immediately. Staff are trained and appropriate policies and procedures are in place to assist them. One resident who self medicates has been assessed and details are recorded in the care plan. A lockable fridge is available should the home need this facility and a system is in place for recording medicines on the controlled register. An error was found in this system caused by staff not following the home’s procedures. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14, 15. Quality in this outcome area is good. This judgement has been made from evidence gathered both during and before the visit to this service. Residents enjoy a range of appropriate activities. There are many regular visitors in the home. Good meals are provided. EVIDENCE: Although again some records require updating, residents confirmed they choose how to spend their day. Residents were seen coming and going throughout the day, both unsupported and with relatives, friends and staff. Activities to meet a range of tastes are provided by the home activities organiser and other members of the staff team. Visitors to the home confirmed they are well received and can have privacy if they wish. Church visitors conduct regular services with communion for those interested residents. By arrangement residents can bring to the home, items of furniture and other personal possessions. Many instances of this were noted during a tour of the building. Should anyone require advocacy, details of this service is advertised on the home’s notice board. Hot and cold drinks and in-between meal snacks are available throughout the day and night where required. In the kitchen there was a four-week rotating menu with a choice of food available at all main meal times. Appropriate quantities of foodstuffs including fresh fruit and vegetables were available to kitchen staff. The inspector had lunch with residents in one of the home’s three dining areas. The meal was well presented, hot and tasty. This judgement has been made from evidence gathered both during and before the visit to this service. A system is in place to support residents making a complaint and to protect them from abuse. EVIDENCE: Each resident is provided with a copy of the complaints procedure when they enter the home and a copy is posted in the lobby for the benefit of visitors. Staff are aware of how to protect vulnerable people, they have received recent training and supportive procedures and department of health guidance is available to them in the home. The manager and some senior staff have attended additional training. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 26. Quality in this outcome area is good. This judgement has been made from evidence gathered both during and before the visit to this service. The home is well maintained, safe and comfortable. EVIDENCE: St Anne’s is maintained to a good standard throughout. The frontage is recently redecorated and the gardens that are well maintained, have recently won a North Tyneside in Bloom competition. Lounges contain comfortable furniture and bedrooms are individual in style with personal furnishings and those supplied by the home. The home is clean with no odours noted. A good standard of safety is maintained. The temperature of the water in two bathrooms was below a comfortable level. This was noted at the last inspection also. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29, 30. Quality in this outcome area is good. This judgement has been made from evidence gathered both during and before the visit to this service. Trained staff employed in sufficient numbers meet residents needs. The home operates a thorough recruitment process. EVIDENCE: The rota demonstrated that sufficient staff are employed to meet residents needs. One of nineteen questionnaire returned from a visitor felt that staffing was not always sufficient. Residents and visitors spoken to during the inspection were happy with the staffing and several comments were made regarding the good qualities in the staff team. All staff are over 18 years old and no one under 21 years old is left in charge. The majority of care staff have a National Vocational Qualification and the manager has recently completed an audit of outstanding training and this is now booked. Staff files for two recently employed staff were reviewed. These contained appropriate references and Criminal Records Bureau checks. These staff are being taken through a formal induction process to ensure they are competent to care for older people. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35, 38. Quality in this outcome area is good. This judgement has been made from evidence gathered both during and before the visit to this service. An experienced manager is in charge of the home. Quality monitoring has not been developed. Financial monitoring systems are in place. Safe working practices are promoted. EVIDENCE: The manager who is experienced in providing care for older people has achieved the Registered Managers Award. She is in day-to-day control of all aspects of care. A quality assurance system has been purchased for use in the home. As yet no system of self-monitoring has been developed and residents/visitors views have not been formally solicited. The home’s systems have been assessed previously and an Investors in People Award attained. A receipt is retained for any purchases or services provided in the home in addition to the weekly charges. The home is safe for residents. Risks are assessed and minimised. Maintenance certificates are in place for gas and electrical systems and staff are trained in this area of care.
