The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

The home is well maintained with a good standard of furnishing throughout. Residents and visitors confirmed that quality care is provided. Staff confirmed that morale is good.

What has improved since the last inspection?

Some work is being undertaken to improve the fabric of the building. A new lounge carpet was fitted and new dining tables provided. A new telephone system has been installed to improve privacy for the residents and the managers office is being refurbished. Further staff training is being provided.

What the care home could do better:

The proprietor has not undertaken his statutory visits to satisfy himself regarding the standard of care provided in the home. The manager has not submitted her application for registration. Some pharmacy guidance was not adhered to.

Date of last inspection 9 June 2005 Brief Description of the Service: St Annes is converted from three Victorian style terraced houses. It is situated on the seafront between Cullercoats and Whitley Bay and has excellent views of the seafront towards St. Marys Island. The home is on three floors. A passenger lift, two short staircase lifts and some ramps, provide access to all areas of the home. The home is registered to provide care to 40 older people, 7 of whom have a dementia. There are 34 single bedrooms, 16 of which have ensuite toilet and hand basin. There are 3 shared bedrooms, 2 of which have ensuite facilities. The communal facilities consist of; a lounge, two dining/lounge areas, a dining room with adjoining conservatory and an amenities room used for crafts and hairdressing. The home is well maintained with a large landscaped garden at the front. This was the home’s second inspection this year and took 5 hours. In that time a selection of the home’s records was reviewed, several residents were spoken with, a tour of the premises was undertaken and staff views were obtained. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2, 3. Each new prospective resident is assessed to ensure the home can meet his or her needs. Each new resident is provided with a contract. EVIDENCE: Four files were reviewed and each contained a pre admission assessment, undertaken to assess if the home could adequately meet the needs of the prospective resident. Each resident is now provided with a written contract or statement of terms and conditions. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 9. Residents’ health and personal care needs are well met. The system for dispensing medicines does not comply with pharmacy guidance. EVIDENCE: Four care plans used by staff daily in the provision of care were reviewed and were found to contain good quality information that was regularly checked. The plans contained appropriate personal information with instructions to staff in the provision of care and a record of healthcare interventions. Several residents spoken with confirmed the standard of care provided is good. The system for ordering and dispensing medicines in the home was reviewed and found to be generally satisfactory. The storage of medicines is temporarily changed as the manager’s office is being refurbished and currently medicines on the controlled register are not being stored separately. The medication for one resident was not recorded as received or given. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12. A varied programme of activities is provided in the home. EVIDENCE: Care plans now include a personal history and pen picture to assist staff in providing appropriate social stimulation for residents. An activities organiser is available to encourage residents to be involved in the home’s activities schedule. She has recently been provided with a budget to support her activities. The activities organiser was able to provide information demonstrating her involvement with many of the residents. Several residents are assessed as able to access community activities unsupported and staff assist those requiring help. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16, 18. A system is in place to support residents making a complaint and to protect them from abuse. EVIDENCE: A new notice informing residents and their visitors how to complain is posted in the lobby of the home. Those residents spoken to were aware of their rights and of how to make a complaint. Staff have access to appropriate training and information relating to caring for vulnerable adults and the manager is to attend an intensive training course in the near future. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 26. St Anne’s is a well maintained home that is safe and comfortable throughout. EVIDENCE: The home is appointed to a good standard with comfortable furnishings throughout. Bedrooms are varied in style to suit the individual with personal furnishings or furniture supplied by the home. The home was clean and well maintained throughout. The gardens at the front of the home are landscaped and professionally maintained. The new proprietor is improving the home; the outside is being painted, the manager’s office is being extended, a lounge carpet has been replaced and new dining tables provided. A new telephone system is being installed that allows private calls to be taken in residents’ bedrooms. Some bedroom door locks are not effective and one lock has been removed and not replaced. The bath water in the home was generally cooler than required. Residents spoken to stated the home was comfortable and safe and clean. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 28, 29, 30. Trained staff employed in sufficient numbers meet residents’ needs. The home operates a thorough recruitment process. EVIDENCE: The staffing levels are appropriate to meet the current residents needs. 44 of the staff team have obtained a National Vocational Qualification in care at level 2 or above and a further 36 are in progress. The new manager has not employed any staff yet but two staff files reviewed demonstrated good employment practice is in place. A comprehensive development programme for staff is in place. Induction training is provided for new staff and a training record and record of achievement is maintained. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33, 35, 38. The new proprietor has not as yet developed an effective quality assurance system. Residents and their families are encouraged to maintain their own finances. Some safety issues were not demonstrated and some records need personalising to the new owner. EVIDENCE: Although the previous provider had a system of quality monitoring, the new proprietor has not as yet developed his method of assessing the quality of care provided. The home has the Investors in People Award and this is due to be re assessed. Monies and valuables were not stored in the Home’s safekeeping facilities as service users and/or their families were encouraged to retain responsibility for their financial arrangements. The home operates a system of invoicing monthly for any goods or services obtained. The home’s records are not yet customised to reflect the new owner and the manager’s application for registration is still outstanding. The proprietor has not instigated his regulatory requirement to conduct a monthly assessment of the home. An immediate requirement is made in this respect. STATUTORY REQUIREMENTS This section sets out the actions which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards. Standard OP9 Regulation 13(2) Requirement Comply with pharmacy guidance in that all medication brought into the home should be recorded and that all medicines dispensed by care staff should be recorded. Attend to the following health and safety issues, ensure water in the home used for bathing is near 43 deg C. Produce a programme to improve those locks in the home that do not meet an approved standard. Provide the opportunity for staff to achieve NVQ level 2 or above so that 50 of the staff team obtain this award by 31 December 2005. Introduce a system of reviewing the quality of care provided in the home. The proprietor should introduce a system of visiting the home on a monthly basis. The associated report of his visit should be forwarded to the Commission. The manager should submit an application for registration as manager of the home.
