Conditions of registration: No conditions are applied. Date of last inspection 28th February 2005 Brief Description of the Service: St Annes is converted from three Victorian style terraced houses. It is situated on the seafront between Cullercoats and Whitley Bay and has excellent views of the seafront towards St. Marys Island. The home is on three floors. A passenger lift, two short staircase lifts and some ramps, provide access to all areas of the home. The home is registered to provide care to 40 older people, 7 of whom have a dementia. There are 34 single bedrooms, 16 of which have ensuite toilet and hand basin. There are 3 shared bedrooms, 2 of which have ensuite facilities. The communal facilities consist of; a lounge, two dining/lounge areas, a dining room with adjoining conservatory and an amenities room used for crafts and hairdressing. The home is well maintained with a large landscaped garden at the front. St Annes Residential Care Home Version 1.10 Page 5 SUMMARY
This is an overview of what the inspector found during the inspection. This unannounced inspection was conducted after a change of owner in January and a change in manager earlier in the month. The purpose of the inspection was to ensure a smooth transition had taken place and that residents’ care and safety was maintained. The inspection lasted 5 hours and in that time several residents and their visitors were spoken with, a tour of the premises was undertaken and staff views were obtained. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 2, 3, 6. Each care plan contains an assessment of need to support the care of residents. No progress has been made to amend the home’s documentation to that of the new proprietor. New service user contracts have not been developed. EVIDENCE: New contracts were not available for inspection and documentation seen in the home was that of the previous proprietor. Care plans contained assessments conducted prior to admission. The information was appropriate to reduce the risk of inappropriate admissions being accepted. The new manager is currently reviewing the residents’ files as part of her induction into the home. The home does not accept referrals requiring ‘rehabilitation’ style care. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9, 10. Residents spoken with confirmed their health and personal care needs are well met. This was supported by the working care plans reviewed. EVIDENCE: Four care plans used by staff daily in the provision of care were reviewed and were found to contain good quality information that was regularly checked. The plans contained appropriate personal information with instructions to staff in the provision of care and a record of healthcare interventions. More comprehensive case records were not reviewed. Several residents spoken with confirmed the standard of care provided is good and that their privacy and dignity is respected. Visitors stated they were provided with the information they required by senior staff. The system for ordering and dispensing medicines in the home was reviewed and found to be generally satisfactory. The manager’s attention was brought to the following issues; a variable dose medicine was not adequately recorded, a medication required by one resident was not available and hand written entries in the medical administration records were unsigned. Staff did not have access to a contemporary medical reference book and all staff dispensing medicines have not yet received appropriate training.

Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 15. Arrangements are in place to provide residents’ with choice and control over their lives. A range of food is provided with the opportunity for individual choice. A range of appropriate activities are in place both within and outside of the home. Visitors are made welcome EVIDENCE: An activities organiser is available to encourage residents to be involved in the home’s activities schedule. She does not have a defined budget but has access to a range of aids. The activities organiser was able to provide information demonstrating her involvement with many of the residents but she is not involved in any areas of care planning. Several residents are assessed as able to access community activities unsupported and those requiring help are assisted by staff. Residents and their visitors spoken to confirmed that visiting is flexible, that a quiet area can always be made available to them and that staff are very welcoming. Residents confirmed they are offered three full meals each day with at least one of these being a hot meal. Hot and cold drinks and in-between meal snacks are available throughout the day and night where required. In the kitchen there was a four-week rotating menu with a choice of food available at all main meal times.

Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 23, 24, 25, 26. St Anne’s is a well maintained home that is safe and comfortable throughout. EVIDENCE: The home is appointed to a good standard with comfortable furnishings throughout. Bedrooms are varied in style to suit the individual with personal furnishings or furniture supplied by the home. The home was clean and well maintained throughout. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 28. Trained staff employed in appropriate numbers care for residents. EVIDENCE: Staff spoken to were aware of the needs of residents and on the day of inspection were employed in numbers appropriate to meet their needs. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 37, 38. The home is managed by a competent person. Some safety issues could not be demonstrated. The home’s records required personalising to the new proprietor. EVIDENCE: The manager is known to the Commission, having been registered previously for other homes. An application for registration as manager is outstanding. Having been in post less than 2 weeks, the manager was not required at this inspection to demonstrate her awareness of the home’s systems. These will be reviewed at the next inspection. Staff and residents spoken to stated that the transition between previous and new managers had been conducted with a minimum of disruption to the service. The home’s records were not customised to reflect the new proprietor.

STATUTORY REQUIREMENTS This section sets out the actions which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards. A contract/statement of terms should be in place for each resident. The following pharmacy guidance should be adhered to; record the variable medication doses given to residents, medicines prescribed should be available in the home at all times, hand written entries in medical administration records should be signed by the transcriber, all staff dispensing medicines should have received accredited medication training, a contemporary medical reference book should be available to staff. The registered provider must visit the home unannounced each month to examine the quality of care provided. THIS REQUIREMENT IS OUTSTANDING FROM THE PREVIOUS INSPECTION.
