Conditions of registration: None Date of last inspection 22/11/2004 Brief Description of the Service: St Georges House is a care home providing personal care and accommodation for up to 19 older people.The home is privately owned and managed by Mr and Mrs Lindley, who purchased the property in 1982. The home is located a short distance from the centre of Tiverton, a market town offering a full range of amenities and facilities. The home is situated next to a large public park, which is enjoyed by service users.The home, originally a small hotel, comprises a large detached Georgian two-storey building, surrounded by attractive and well-maintained gardens. There is ample parking to the front of the property.There are 16 bedrooms in the home, 13 single and 3 double, although all bedrooms are used as singles. There are 10 bedrooms with ensuite facilities. The shared communal rooms are spacious, comfortable and cosy. The home is best suited to service users who are ambulant and able to manage stairs. This is an overview of what the inspector found during the inspection. The inspection took place over 5 hours during a day in April. The majority of inspection time was taken speaking with the residents, both in the lounge and in their private rooms. 9 of the 16 residents talked with the inspector about their lives and the home. The proprietor (Mrs Lindley) and the deputy manager were present throughout the inspection. All of the staff took part in the inspection, and also 2 visitors from the local church. A tour of the building was made, but not all service user rooms were seen on this occasion. A variety of records were inspected including, care plans, service user files, the menu diary, staff records and rota, medication records/procedures, accident and fire records. Care plans continue to improve with clearer concise records.

Full information of agency staff is now properly recorded. A new induction programme for staff has been introduced. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 The homes assessment process is thorough and ensures that it is able to meet residents’ needs. EVIDENCE: The provider and deputy manager undertake a considered assessment of individual need, which assure prospective residents of the homes ability to meet their needs. The provider is aware of the need to balance the needs of the current residents with those of a prospective resident to ensure that everyone’s needs are met. Where there is a doubt a refusal is made. Written details of assessments are contained within resident’s files. A care plan is written once a person has moved into the home. Residents are involved in the process, although many said they are disinterested in what is recorded. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9,10 The staff have a good understanding of the health and personal care needs of residents, which is documented clearly in care plans. EVIDENCE: There is a consistent care planning system in place to provide staff with the information they need to meet the needs of service users. Care plans are well informed and are consistent. Care plans records are up to date and are reviewed monthly with the involvement of residents’. Residents’ confirmed that their health care needs were promptly met. Residents thought that the staff were respectful and provided individuals with their privacy. This was evident in the interaction between staff and residents. Staff have a good understanding of the health and personal care needs of each resident. The systems for the administration of medication are well managed ensuring that service users medication needs are met. Some residents self medicate and store their medication in their rooms. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,15 Social activities and meals are both well managed, creative and provide daily variation and interest for people living in the home. Residents are encouraged to maintain their independence, exercising choice and taking control of their lives. EVIDENCE: The majority of residents spoken with said how good the food was and how much they looked forward to meals. Residents said there is a choice at every meal and that times of meals are flexible. Food is freshly prepared and home cooked. The menu diary showed the variety of foods offered in providing a balanced wholesome menu. Daily menus are displayed on dining tables. Activities are arranged during the afternoons. Some of the residents are able to attend community events maintaining previous interests and contact. There is an exceptionally good track record of involvement and contact with the local community. Several of the residents said they liked to walk in the nearby park. Residents share their crosswords from the daily newspaper (Daily Telegraph), in the lounge. They said it helped to “keep the little grey cells working!” Residents are encouraged to maintain their independence in whatever way or manner is meaningful to them. The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 The home has a robust complaints procedure and residents feel that they can raise any issue of concern which will be listened to and acted upon. EVIDENCE: There is a detailed complaints procedure with timescales, which is displayed in the entrance hall of the home. Residents said they have no complaints but if they have any issue or concern would have no hesitation raising issues with the proprietor or one of the staff. Anyone who has raised an issue in the past said it had been dealt with promptly, usually on the same day. Although all residents are aware of the complaints procedure one resident said they would not wish to make a fuss by complaining. Care staff said that they would report any concern or complaint to the deputy manager or provider. They also confirmed that any issue raised by residents or themselves is always acted addressed promptly by the provider. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,26 The standard of the environment is high, providing residents with attractive, safe and clean surroundings. EVIDENCE: The standard of accommodation is high. Rooms have high ceilings and large windows. Considerable thought is given to attaining the right décor and furnishings to suit the premises. Attention is paid to the small details too (e.g. complimentary colour schemes, vases of fresh flowers) to make the home comfortable and homely. Since the last inspection several residents’ rooms have been redecorated. Residents and staff commented on the tasteful decoration of rooms. Rooms are routinely refurbished as they become vacant. The dining room has recently been completely refurbished and a new carpet fitted. The gardens are very attractive and are well maintained. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,29 Arrangements for the induction and training of staff are good with staff demonstrating a clear understanding of their role and responsibilities. EVIDENCE: Many of the staff have been employed at the home for several years and work well together. Staff are able to explain their role and responsibilities. All of the staff spoken to enjoy working at the home and are committed to providing a high standard of care for residents. A number of staff have been employed at the home for several years. Staff said they were “a good stable team with good bosses”. Staff support each other in times of pressure or absence. Staff like working for a “family business” and consider that they receive good training and support in doing their jobs. Sufficient staff are on duty to meet the needs of service users. A rota is displayed indicating who is on duty and in which capacity. Care, domestic and catering staff said they work together well as a team and are supported at all times. Regular supervision is provided. A robust recruitment procedure in place with systems in place to ensure all essential information is contained in each staff members file. Agency staff records show that the home receives their full details before a shift is worked. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33,38 The management of the home ensures clear leadership and direction, giving staff guidance and support to meet the needs of residents. There are reliable systems in place to ensure the good health safety and welfare of residents and staff. EVIDENCE: The provider and deputy manager give a clear lead and direction to staff. Residents feel the provider and deputy manager are approachable and seek to ensure all their needs are met. The systems for service user consultation are good with evidence that indicates residents’ views are both sought and acted on. Safe working practices are employed and risk assessments undertaken. Records should that regular fire drills and staff undertake training. Fire alarms and safety lighting are regularly checked.
