This inspection was carried out on 7th September 2005.

What the care home does well

What has improved since the last inspection?

Radiator covers have now been fitted on every radiator in the home, which has ensured that the residents are at no risk from scalding or burns from hot surfaces. All of the staff have now seen the " No Secrets " video and are aware of the procedures for dealing with the alerting of abuse. The homeowner has undertaken training in nutritional needs of residents and is planning to update and improve this part of the care plans.

What the care home could do better:

It was identified during the inspection that fire training was overdue. The home wishes to continue to improve standards and ensure that residents remain happy and safe.

The property is a detached bungalow set in a third of an acre of land on the outskirts of Seaton. The accommodation consists of three bedrooms one of which is en suite. There is also a separate dining room, lounge and two further bathrooms. There is parking on site and pleasant level gardens both to the front and rear of the property.The home is owned and managed by Mrs Roberts. This inspection was unannounced and took part as part of the normal programme of inspection. It took place over three hours, between 10.15 am and 12.15pm. The inspector met all three residents and spoke with the homeowner. She looked around the entire inside of the home and looked at paperwork in relation to care planning and other documentation relating to the home. The owner of the home had completed a pre-inspection questionnaire and the Commission prior to the inspection received this. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3,4,5 Pre admission arrangements ensure that residents’ needs and wishes are fully understood and that they are helped make the right choice of home. EVIDENCE: Written and verbal information is given to prospective residents before admission; this includes a copy of the statement of purpose. The owner said that residents and their families are invited to visit the home and meet the staff before admission. There was evidence of this in pre assessment documentation and one resident confirmed that he had been to visit prior to admission. Prospective service users have a thorough assessment before their admission to the home and are assured whether or not their needs can be met, written conformation of this was seen in the care plans. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,10, Service users’ needs are met. The staff and owner at St Nicholas respect Service users’ privacy and dignity. The system of recording of medicines was not inspected. EVIDENCE: Every resident has his or her own care plan. They clearly identify health and social needs and actions required to meet them. They included assessments of all health, social and nutritional need. Risk assessments for the prevention of falls and pressure sores were completed. There is also a section for medical appointments, oral health, chiropody, hearing, sight and dental needs. The plans are reviewed monthly. Privacy and dignity are given a high priority and residents are treated with respect. The homeowner was observed interacting in a friendly and respectful manner with the residents. One resident commented that all the staff were “extremely kind and helpful”. The medicines for each person are stored in individual cupboards in their bedrooms. Documentation relating to medication was seen and was all in good order. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,15 The residents have a choice as to their preferred lifestyle within the home. Residents are encouraged to maintain contact with their families or friends as they wish. Resident’s nutritional needs are met. Residents receive a wholesome and varied diet. EVIDENCE: A variable, nutritious diet is provided at the home. Residents confirmed this. Choices are available and special diets catered for. Some comments made by the residents on the day included “ the food is very good”, and “ the food could not get any better”. Residents confirmed that they were able to have visitors at any time and that they were always made welcome. Residents confirmed that they could get up and go to bed when they chose to and that they can decide how they may spend their day. JUDGEMENT – we looked at outcomes for standard(s) 16,18 Residents are confidant that they are listened to and their requests actioned. Arrangements for protecting service from abuse users are satisfactory. EVIDENCE: Residents confirmed that their views are taken seriously, that their concerns are fully taken on board and that action is taken where needed. All staff have had recent training in the protection of vulnerable adults. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,26 Residents live in a safe, comfortable and clean environment that is well maintained. EVIDENCE: The home is well maintained and decorated. Each bedroom is individually decorated to suit the individuals taste. There was no evidence of offensive odours. There is level access into and around the home. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,29,30 The numbers and skill mix of competent staff are sufficient to meet residents’ needs. EVIDENCE: The staff rota showed that the allocation of staff on each shift was adequate to meet the need of the current service users. Residents confirmed that they were given all the help they needed and that they did not feel rushed or hurried in anyway. The staff team have regular meetings and a yearly appraisal. Each member of staff has a development plan, which identifies their training needs. Personal staff files were looked at and all contained all the necessary information to ensure safe recruitment practice. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,38 On the whole service users safety and welfare is well protected. Improvements are needed in one area. The home is managed well. EVIDENCE: This standard was not fully inspected. All the radiators are guarded to prevent harm from burns. Water temperatures are regulated to prevent harm from scalding. Mandatory checks and tests are carried out with regard to fire safety and all were satisfactory. The staff undertake regular training in fire safety. However, it was identified during the inspection that the staff had not attended this training for some St Nicholas D54-D06 22038 St Nicholas 241186 070905 Stage 4.doc Version 1.40 Page 15 time. As a result of this a requirement was made to ensure that staff were adequately updated. This was undertaken the very next day. The homeowner/manager has recently completed an NVQ Level 4 in care management and is hoping to complete the Registered Managers Award by September 2005.
