Conditions of registration: 1. The service should at all times employ a suitably qualified and experienced manager who is registered with the Commission for Social Care Inspection 2. The registered manager must achieve the Registered Manager Award by 31 December 2005. 3. The home is registered for a maximum of 15 service users in the category OP (older persons over 65) Date of last inspection 3rd March 2005 Brief Description of the Service: The Conifers is registered with The Commission For Social Care Inspection to provide personal care for fifteen residents of either sex over sixty five years old. Th home is situated in a residential area of Thornton near Cleveleys and is close to shops and local amenities. It comprises a single storey detached property with a private front garden. Accomodation is provided in two double rooms with ensuite facilities and eleven single rooms, one of which has ensuite facilities. There is a lounge ans dining room and a sun lounge. The sun lounge is a designated smoking area. Relatives and friends are made welcome. Activities are organised for residents who wish to participate. Personal needs for residents are Catered for by staff, who receive training in all aspects of care of the older person. This is an overview of what the inspector found during the inspection. This was an unannounced inspection and took place over a period of five hours. The information contained in this report was obtained from discussion with the registered manager and the owner, interviews and informal discussion with staff and residents. The inspector also chose four residents files to look at and tracked their care and interviewed them about the care they receive. He also spoke to visiting relatives and has received questionnaires from relatives and most of the residents. The inspector also had lunch at the home. A tour of the premises took place and health and safety records were seen. Staff records were examined including Criminal Records Bureau checks. Since the last inspection, which took place on 3rd March 2005 there have been no complaints received regarding the home. What the service does well:
The home has a good group of staff that provide care and support for residents. The inspector was able to confirm this from observation, discussion with residents and staff. The four residents whose care was tracked confirmed they are provided with the care they need. One resident told the inspector that the staff provide good care and more. The inspector spoke to a visiting relative, they said, “The staff have made ….. feel at home and she is pleased with the way they treat her. My observations are that the people in the home come first. A lot is done to ensure that everyone is encouraged to take part in activities in the home.” All residents spoken to confirmed these feelings about the care they receive. There is a very good rapport with residents and their relatives, this has been enhanced by activities provided which include them, for instance the garden fete recently held at the home. Meals are varied and there are plenty of choices. The inspector had lunch and observed that choices are available. Residents said that the food they receive is very good and they are able to ask for alternatives if they wish. Mealtimes are flexible according to the needs of residents. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 The admission and assessment procedure is clear and ensures the care needs of residents are met. EVIDENCE: The home has good admission procedures to ensure resident’s needs are assessed and planned for. There are individual records kept for each resident. The inspector looked at the admission and care records for four residents. These detailed quite comprehensively the care needs of the residents. The admission procedures include a visit to the prospective resident from the manager and a visit to the home, if possible. All new residents are admitted on a temporary basis to enable them to make a more permanent decision about their future. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8 and 10 Promotion of health and personal care is taken seriously and residents welfare is monitored and health needs met. Residents privacy and dignity is upheld by the home. EVIDENCE: Care records examined for four residents clearly describe their health needs including risk assessments. Staff spoken to showed an awareness of the care needs of residents, and consult the care plans which are drawn up for residents. One resident said, “They care for me very well here, I wouldn’t go home if I could, I love it here. Surveys carried out by the home indicated that staff perform personal care tasks in a respectful dignified manner protecting residents privacy. Residents spoken to told the inspector they are treated with dignity and respect and that their privacy is maintained. The inspector spoke to a visiting relative, they said, “The staff have made ….. feel at home and she is pleased with the way they treat her. My observations are that the people in the home come first. A lot is done to ensure that everyone is encouraged to take part in activities in the home.” All residents spoken to, confirmed these feelings about the care they receive.

Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 and 15 Social activities are provided which are varied according to the needs of individual service users. Dietary needs of residents are well catered for and there are a choice of meals available to meet individual needs. EVIDENCE: Information regarding residents interests and hobbies are recorded in their care records. Residents and visiting relatives said that staff make every effort to involve residents in activities. Every afternoon activities are arranged. These include, pet therapy, coffee mornings, summer fetes, reminiscence sessions, entertainers, sing alongs, mobile library and outings. Each resident has an individual activities record. The inspector saw a varied menu, available for residents, special diets are catered for. Residents expressed satisfaction with the quality and variety of food they receive. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 and 18 Arrangements for complaints are good and an effective whistle blowing policy ensures protection of residents. EVIDENCE: The home has a detailed complaints procedure. Staff spoken to were able to explain the process, residents spoken to were aware of the procedure which is provided in their service users guide. They said that they are confident, any concerns they have, are dealt with promptly by the manager. A record of internal complaints is kept and outcomes recorded. There has been no complaint made to the CSCI since the last inspection. There is a procedure available to protect residents from abuse including a whistle blowing policy. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,25 and 26 The standard of furnishings and decoration are good and provide a homely environment, which is clean and homely. EVIDENCE: The inspector toured the building and consulted maintenance records. Records showed that each room receives a full check and any repairs undertaken. Most areas of the home have recently been redecorated. Residents said they are happy with the standard of accommodation offered by the home. They are able to bring their own belongings to assist in furnishing their rooms. There are good hygiene procedures in place to prevent odour and infection, staff receive training to ensure they know these policies and procedures. All areas of the home were observed to be clean and hygienic. Staff records showed that staff receive TOPPS health and safety training. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,28 and 29 The policy and procedures for the recruitment and training of staff are robust and provide safeguards which protect residents. The number of staff on duty was sufficient to meet the needs of residents living at the home. There are under 50 of staff with the National Vocational qualification in care at level 2. EVIDENCE: The inspector saw four staff files all contained the necessary recruitment forms and checks to ensure the protection of residents. These include an application form, two references a POVA and CRB check. Induction and training records were seen and showed all staff receive an induction and training in providing care. Records also showed that 2 out of 10 care staff are trained to NVQ 2 however 2 are currently doing NVQ 2, 3 NVQ 3 and one NVQ 4. Staff were interviewed and confirmed they receive induction and training. Rotas examined showed that there are sufficient staff on duty to meet the needs of residents. Sufficient staff were observed to be on duty at the time of the inspection.

Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33,35,37 and 38 Management of the has improved during the past year there is leadership, guidance and continuity for residents and staff. The registered manager is currently doing the Registered Managers award and hopes to complete this by December this year. There are Health and Safety policies and procedures in place which protect residents and staff. EVIDENCE: The registered manager has worked in a senior capacity for several years, she is qualified to NVQ 4 IN care and is currently doing the Registered Managers Award which she hopes to complete by December this year. Staff and residents spoken to said management was open an approachable. The manager or the owner is available to residents in the home on a daily

The inspector saw the homes policies and procedures in relation to health and safety. These ensure safety for those living and working in the home. To ensure staff are fully aware of health and safety issues they complete the TOPPS health and safety training. This was confirmed from talking to staff and consulting training records.
