This inspection was carried out on 30th December 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home continues to have a good group of staff that provide care and support for residents. Observation and discussion with residents and staff confirmed this. Residents whose care was tracked confirmed they are provided with the care they need. One resident said that the staff provide good care and more. Observations from visiting relatives are that the people in the home come first. A lot is done to ensure that everyone is encouraged to take part in activities in the home. All residents spoken to confirmed these feelings about the care they receive. There is a very good rapport with residents and their relatives, this has been enhanced by activities provided which include them, for instance by garden fetes held at the home.

What has improved since the last inspection?

There has been an on going programme of redecoration and environmental standards have improved. Residents are consulted about decorations and said that they are happy with the comfort provided by the home. The inspector observed a very good atmosphere in the home, which continues to improve. There was good rapport between staff and residents.

The service should at all times employ a suitably qualified and experienced manager who is registered with the Commission for Social Care Inspection The registered manager must achieve the Registered Manager Award by 31 December 2005 The home is registered for a maximum of 15 service users in the category OP (older persons over 65) 25th August 2005 Date of last inspection Brief Description of the Service: The Conifers is registered with The Commission For Social Care Inspection to provide personal care for fifteen residents of either sex over sixty-five years old. The home is situated in a residential area of Thornton near Cleveleys and is close to shops and local amenities. It comprises a single storey detached property with a private front garden. Accommodation is provided in two double rooms with ensuite facilities and eleven single rooms, one of which has ensuite facilities. There is a lounge and dining room and a sun lounge. The sun lounge is a designated smoking area. Relatives and friends are made welcome. Activities are organised for residents who wish to participate. Trained staff meet all the personal needs of residents, medical needs are managed by resident’s general practitioners and district nurses. This was an unannounced inspection; the proprietor completed a pre inspection questionnaire. The inspection place over a five-hour period beginning at 10.00 am. Care records were consulted and most of the residents were spoken with. Visiting relatives were also consulted. Comments received from residents and relatives were positive and were complementary about the care they receive. One resident said, “The care we get is very good and there are plenty of things to do, the staff are really helpful”. Another said, I have everything I need and am very happy here”, The building was toured and staff were interviewed. The registered manager was consulted and records and policies and procedures. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 There are good policies and procedures in place, which promote safe administration of medication, however not all records were completed properly. EVIDENCE: Records showed that only staff who are trained in administration of medication are involved in ordering and giving medication out. Those staff signatures were seen on the drugs administration sheets. On admission residents are asked whether they want to deal with their own medication and a risk assessment is carried out regarding this, residents care files were seen and confirmed this to be the case. An audit of one person’s medication showed that there were incorrect entries for one drug, which showed that some tablets were not accounted for. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 13 and 14 Residents are able to maintain contact with family and friends and are enabled to maintain their links in the community. Most resident’s families deal with their finances; there is access to external advocacy services if required. Residents are able to bring their own personal possession into the home. EVIDENCE: The inspector spoke to residents and visitors to the home and was told that there are good contacts maintained with relatives and friends. The home organises social events for families and friends to take part in. Residents said they look forward to these and enjoy them. Visitors are able to see residents in private and are always made welcome. Residents finances are dealt with by families, however the home does administer some personal allowances and keeps records of this. These were seen during the inspection. Residents said they have access to their allowance whenever they need to pay for anything. Residents said they are happy with their accommodation and the routines are flexible to allow them to have some control over what they wish to do. Information regarding advocacy services was seen in the home. The local advocacy service was used recently regarding one resident. Residents have access to their files and records showed they or their representatives sign reviews of care that take place. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28 and 30 Most carers are provided with training within the home to enable them to carry out their duties. However there needs to be more staff trained to NVQ2 in care to fully meet this Standard. EVIDENCE: The inspector saw the homes training records; these showed that all staff are provided with a Skills to Care Induction (formally TOPPS) and foundation training. This covers the aims and objectives of the home and the knowledge needed to provide care including the principles, dignity, independence, privacy and choice. Records showed the following training also being provided by the home are, Protection of Vulnerable Adults, Data Protection, confidentiality, Effective Communication, Abuse awareness, Whistle blowing, Health and Safety, Medication Administration, Fire Safety, Food Hygiene, Principles of Care, Dementia Care and moving and Handling. It was also noted that there are members of staff doing NVQ2 and 3 and that while the level presently is below 50 for staff with this qualification this will shortly be above this level required by Standard 28 The Conifers DS0000009857.V270646.R01.S.doc Version 5.1 Page 15 Management and Administration The intended outcomes for Standards 31 – 38 are: 31. 32. 33. 34. 35. 36. 37. 38. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31 The registered Manager is experienced and is currently completing the Registered Managers Award. This qualification needs to be fully completed to fully meet this Standard. There are clear lines of accountability in the home. EVIDENCE: The registered manager has more than two years experience in management of a care home, however at the time of the inspection had not completed the Registered Managers Award. Staff and residents spoken to were clear about who is in charge of the home and are confident that they can approach management regarding any concerns.
