Conditions of registration: Date of last inspection 31 January 2005 Brief Description of the Service: 136 Stewart Street is a care home for three adults with a learning disability. It is part of the Lady Verdin Trust organisation which operates a number of facilities for adults with a learning disability within Crewe and Nantwich. Located in a residential area of Crewe the semi-detached house is close to shops and other local amenities. The home has a mini bus which is used to transport residents. Access between the ground and first floors is by a stairway. The three bedrooms are single and are situated on the first floor. Each of the bedrooms has handwashing facilities. Sufficient toilet and bathing facilities are provided. A large communal lounge/dining area and a smaller lounge are provided for the residents. There is a large well-maintained secure garden to the rear of the home. A wheelchair is provided for a resident with mobility problems. This is an overview of what the inspector found during the inspection. This inspection was carried out over 2.5 hours on the 22nd August 2005 as part of the yearly inspection process. One hour was spent preparing for the inspection which included reading the previous inspection report and reviewing the service history for the home. Three residents and three staff were spoken with during the inspection. A number of the home records were seen during the visit. The residents have limited communication capabilities. What the service does well:
A safe, homely and well-maintained environment is provided for the residents. The home is domestic in appearance and has easy access to shops, pubs and other local facilities. Staff are employed by the home who know the residents and are able to understand and communicate with them. Information on the service offered by the home is available as is a service user guide to the home. Pre-admission visits to the home by prospective residents and their representative is encouraged. Care plans are in place which show how residents are to be cared for. These include risk assessments that help keep residents safe and encourage their independence. A separate file is kept in which residents health care needs are set out. This includes the outcome of visits by doctors, nurses and other healthcare professionals. The complaints and adult protection procedure helps ensure residents concerns and worries are listened to and acted upon. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1/2/3/4/ Admission procedures are in place that ensure prospective residents are fully assessed and have the information they require to make a decision about moving into the home. The home is able to meet residents’ needs. EVIDENCE: The three residents have lived in the home for sometime. During the inspection staff were seen caring for residents and were aware of their assessed needs. A copy of the statement of purpose and service user guide was available in the home. The service user guide has pictures to help residents with communication problems. JUDGEMENT – we looked at outcomes for standard(s) 6/7/8/9/ Risk assessments are in place which identify possible dangers to residents and safeguards are in place to protect them. Residents are consulted about the day-to-day running of the home. EVIDENCE: Personal Development Plans are available for each resident. These include background information and how the assessed care needs of residents are to be met. Plans also showed that care needs are monitored and updated. The care needs of residents is such that they need full support with daily living tasks, for example, using local shops and other community facilities. Staff were seen talking to residents about what to have for tea and where they wished to spent their leisure time. Residents were comfortable in the company of staff and were seen approaching them for help. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 11/12/13/14/15/17 Residents are enabled to take part in activities that fit within their abilities. Staff support and encourage residents’ to be part of the local community. EVIDENCE: Residents are supported by staff to manage daily living tasks. This can include making drinks, using the bathroom and in-house activities. Residents have a weekly programme of activities which can include attendance at a day centre. Residents are also supported by staff from the daily options scheme with their daily activities. The location and appearance of the home helps ensure it is seen as part of the local community. Staff confirmed that relationships with the neighbours is, on the whole, is satisfactory. Plans of care showed that relatives and other visitors are able to visit the home and be involved in caring for the residents. For example, two of the residents are enabled to spend time with their relatives in the family home.

Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18/19/20 Residents are well looked after with regard to their health and personal care needs. They receive full support from staff with their personal and emotional needs. EVIDENCE: Residents’ healthcare needs are recorded in a separate file. Included in the file is their contact with doctors, nurses and other healthcare professions and the outcome of their visits. A record of residents prescribed medication is also included in the file. During the inspection staff were seen caring for, and supporting, residents in a manner that respected their privacy and dignity. Residents require full assistance with the administration of their medication. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22/23 There is a complaints and adult protection procedure in place. This ensures residents concerns are listened to and that they are protected from abuse. EVIDENCE: A copy of the complaints procedure was seen during the inspection. Details on how to contact the Commission for Social Care Inspection are included in the procedure. The registered manager said no complaints have being received about the home. CSCI have not received any complaints about the home. The organisation have provided an adult protection procedure, a copy of which is kept in the home. A copy of the government guidelines ‘ No Secrets’ was also available in the home. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24/25/26/27/28/29/30 A safe, homely and comfortable standard of accommodation is provided for residents. Single bedroom are provided that meet the needs of residents. EVIDENCE: A number of improvements have been carried out in the home. These include new windows, new front door, new carpets downstairs and a new television. The staff have redecorated the entrance hall. A raised garden has been created to the rear of the home. A Community Safety Officer has visited the home to advise on safety and security. The improvements to the environment has made the home more homely and comfortable for residents. Residents’ single bedrooms are located on the first floor and contain handwashing facilities. They are also individually decorated and furnished. One toilet and bathroom is provided on the first floor and one toilet on the ground floor. Communal space consists of a large lounge/dining area and a smaller lounge. Service users benefit from well supported and supervised staff. The Commission considers Standards 35 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31/32/33/35/36 Staff are aware of their roles and responsibilities with regard to meeting the needs of residents. Staff receive individual supervision from the manager. EVIDENCE: The staffing rota showed that there are normally two staff members on duty when the residents are in the home. Two of the residents attend a community day care centre five days a week whilst the remaining resident is supported by staff from the Trust’s daily option scheme or staff from the day centre during the day. A member of staff sleeps in the home overnight. Staff spoken with confirmed that their roles and responsibilities are re-enforced at staff meetings and one-to-one sessions with the manager. Staff confirmed that they receive training from the organisation. This includes the in-house Supported Assistant General training and NVQ training. They also said the organisation’s training manager arranges courses on first aid, handling/lifting and communicating with residents. Staff were seen caring for residents and meeting their assessed needs. For example, helping in the kitchen with drinks and using the bathroom. Staff details as required by the regulations are maintained in the home.

JUDGEMENT – we looked at outcomes for standard(s) 37/38/39/42/43 The arrangements for managing the home are satisfactory. The health, safety and welfare of service users underpins the overall management of the home. EVIDENCE: The registered manager has worked for the Trust in a senior capacity for a number of years. She is also the registered manager for another home that is run by Lady Verdin Trust and is located in Crewe. Training on the day-to-day skills required to manage a care home is provided by the organisation’s training manager. The manager for the home has the experience and knowledge to ensure that is run in the best interest of residents. Staff spoken with said they, and residents and their representatives, are encouraged to comment on the service offered by the home. This included the

Although the residents have limited communication it was evident during the inspection that staff understand and communicate very well with residents. A partial tour of the building showed that maintenance and health/safety issues are addressed. During the inspection then home received the monthly visit from a member of the management committee.
