Conditions of registration: Date of last inspection 22nd August 2005 Brief Description of the Service: 136 Stewart Street is a care home for three adults with a learning disability. It is part of the Lady Verdin Trust organisation that operates a number of facilities for adults with a learning disability within Crewe and Nantwich. Located in a residential area of Crewe the semi-detached house is close to shops and other local amenities. The home has a mini bus that is used to transport residents. Access between the ground and first floors is by a stairway. The three bedrooms are single and are situated on the first floor. Each of the bedrooms has hand-washing facilities. Sufficient toilet and bathing facilities are provided. A large communal lounge/dining area and a smaller lounge are provided for the residents. There is a large well-maintained secure garden to the rear of the home. A wheelchair is provided for a resident with mobility problems. This is an overview of what the inspector found during the inspection. This inspection took place over three hours on the 6th February 2006. Feedback following the inspection was given to the new manager for the service on the 14h February 2006. One hour was spent preparing for the inspection which included reading the previous inspection report and reviewing the service history for the home. During the inspection two residents and two staff were spoken with. A partial tour of the building was carried out. The residents have limited communication capabilities. What the service does well: A safe, homely and well-maintained environment is provided for the residents. The home is domestic in appearance and has easy access to shops, pubs and other local facilities. Staff are employed by the home that know the residents and are able to understand and communicate with them. Information on the service offered by the home is available as is a service user guide to the home. Pre-admission visits to the home by prospective residents and their representative are encouraged. Care plans are in place which show how residents are to be cared for. These include risk assessments that help keep residents safe and encourage their independence. A separate file is kept in which residents’ health care needs are set out. This includes the outcome of visits by doctors, nurses and other healthcare professionals. The complaints and adult protection procedure helps ensure residents concerns and worries are listened to and acted upon. The Lady Verdin Trust provides support and guidance to the manager and staff to ensure the home is run in the best interests of residents. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1/2/3/4/ Admission procedures are in place that ensures prospective residents are fully assessed and have the information they require to make a decision about moving into the home. EVIDENCE: The three residents have lived in the home for some time. During the inspection staff were seen caring for residents and were aware of their assessed needs. One of the residents is temporarily living in another Trust home whilst he recovers from a major operation. The Trust has developed policies and procedures and provides information to ensure prospective residents and/or their representatives have sufficient information to make a decision about moving into the home. Staff from the home will also meet with prospective residents to carry out a needs assessment. A block contract is in place with the funding authority. This includes terms and conditions of residency. Copies of the contract are kept in the main office for the Trusts main office in Crewe. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6/7/8/9/10 Risk assessments are in place which identifies possible dangers to residents and safeguards are in place to protect them. Residents are consulted about the day-to-day running of the home. EVIDENCE: Comprehensive Personal Development Plans are available for each resident. Two plans were seen during the inspection. These included background information, personal histories and how the assessed care needs of residents are to be met. Plans also showed that residents’ care needs are monitored and updated. Client assessments are also regularly reviewed. However, the staff carrying out the assessment does not sign these. See Recommendation Number 1. Residents are encouraged and supported by staff to make decisions about their daily lives. For example, during the inspection one resident had chosen to sit outside in the garden and was asked regularly by staff if he wished to move into the communal lounge. Also residents were asked what they wanted for tea. A policy has been provided by the Trust on the confidentiality of information, a copy of which is readily available to staff. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 11/12/13/14/15/16/17 Residents are enabled to take part in activities that fit within their abilities. Staff support and encourage residents’ to be part of the local community. EVIDENCE: The assessed needs of residents are such that they require support and help with day-to-day living. Opportunities for personal development, although limited, are offered to residents. Residents’ records showed that they are enabled to take part in various activities. This can include visiting pubs/restaurants and other places of interest. Residents also attend community day care services. Support to access these services can be provided by staff from the Trust’s daily option scheme. Relatives and friends are able to visit the home and be involved in the care of residents. Staff spoken with are made aware of their roles and responsibilities as part of their induction training, individual staff supervision, staff meetings and ‘on the job’ monitoring.

During the inspection residents were offered drinks by staff. Residents can help with food preparation with support from staff. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18/19/20/21 Residents are well looked after with regard to their health and personal care needs. They receive full support from staff with their personal and emotional needs. EVIDENCE: Residents’ healthcare needs are recorded in a separate file. Included in the file is their contact with doctors, nurses and other healthcare professions and the outcome of their visits. A record of residents’ prescribed medication is also included in the file. The record of medication administered to the residents was seen and was signed by staff. Residents require assistance from staff with their prescribed medication. The records seen showed that residents’ health needs, both physical and emotional, are monitored and action taken to address any concerns. Copies of letters showed that residents receive input from specialist services at the local hospital. From discussion with staff it was evident that they were aware of the healthcare needs of residents and what action to take. The Trust has provided policies and procedures to assist staff care for residents who are sick. Also provided are policies on what action to take in the event of the death of a resident.

JUDGEMENT – we looked at outcomes for the following standard(s): 22/23 There is a complaints and adult protection procedure in place. This ensures residents’ concerns are listened to and that they are protected from abuse. EVIDENCE: The Trust has provided a complaints procedure that includes details on how to contact the Commission for Social Care Inspection are included in the procedure. The staff spoken with said no complaints have being received about the home. CSCI have not received any complaints about the home. The organisation has provided an adult protection procedure, a copy of which is kept in the home. A copy of the government guidelines ‘ No Secrets’ was also available in the home. JUDGEMENT – we looked at outcomes for the following standard(s): 24/25/26/27/28/30 A safe, homely and comfortable standard of accommodation is provided for residents. Single bedrooms are provided that meet the needs of residents EVIDENCE: A number of improvements have been carried out in the home. Bedrooms and the front lounge have been re-decorated. A raised garden has been created to the rear of the home. These further improvements to the environment have made the home more homely and comfortable for residents. Residents’ single bedrooms are located on the first floor and contain handwashing facilities. They are also individually decorated and furnished. One toilet and bathroom is provided on the first floor and one toilet on the ground floor. Communal space consists of a large lounge/dining area and a smaller lounge. A wheelchair is provided for one of the residents. A secure well-maintained garden is located to the rear of the home and is easily accessible to residents. The area to the front of the home provides car parking space. JUDGEMENT – we looked at outcomes for the following standard(s): 31/32/33/35/36 Staff are aware of their roles and responsibilities with regard to meeting the needs of residents. Staff receive individual supervision from the senior staff in the Trust. EVIDENCE: Staff spoken with said their roles and responsibilities are discussed during individual supervision with senior staff. Staff meetings are also held where home and organisational issues are discussed. They also said their care practices are monitored by senior staff. The staffing rota showed that there are normally two staff members on duty when the residents are in the home. Two of the residents attend a community day care centre five days a week whilst the remaining resident is supported by staff from the Trust’s daily option scheme or staff from the day centre during the day. A member of staff sleeps in the home overnight. Staff spoken with said they have achieved their NVQ and that the Trust provide a range of training opportunities. One member of staff said he had received comprehensive induction training when he first came to work for the Trust. JUDGEMENT – we looked at outcomes for the following standard(s): EVIDENCE: Since the last inspection the registered manager has left the organisation. A new manager has been appointed who is in the process of completing her application to be the Registered Manager. Other managers from the Trust are providing support and guidance whilst the new manager is completing her induction training. Staff spoken with were positive in their comments about the new manager and feel her experience will benefit the service. They said the change of manager has not affected the care given to residents. The Trust has provided a range of policies and procedures to assist staff in their caring for residents.
