Conditions of registration: 1. This home is registered for a maximum of 3 service users in the category of LD (Learning disability) 10th October 2006 Date of last inspection Brief Description of the Service: 136 Stewart Street is a care home for three adults with a learning disability. It is run by the Lady Verdin Trust that operates a number of homes and other services for adults with a learning disability in Crewe and Nantwich. This home is a semi-detached house in a residential area of Crewe, close to shops and other local services. The house has two floors with access between them via a staircase. All three bedrooms are single rooms and are on the first floor. Each has a hand wash basin. There are sufficient toilet and bathing facilities. There is a large communal lounge/dining area and a smaller lounge for the residents on the ground floor. There is a large well-maintained, secure garden to the rear of the home. A wheelchair is provided for a resident with mobility problems. The home has a mini bus that is used to transport residents. Staff are on duty twenty-four hours a day to provide care for residents. The cost of living in the home depends on individual needs and information provided by the manager on 10 April 2007 states the current charges are between £1226 and £1411 per week. This is an overview of what the inspector found during the inspection. This unannounced visit, part of the inspection of this service, took place on 26 July 2007 and lasted three hours. The manager was told about the findings of the inspection by telephone on 03 August 2007. This visit was just one part of the inspection. Other information received by CSCI about the home was also looked at. Before the visit, the manager was asked to complete a questionnaire to provide up to date information about the services provided. CSCI questionnaires were given to residents, their families, health and social care professionals such as district nurses and social workers to find out their views about the home. Comments from forms that were sent back to CSCI have been included in the report. During the visit, various records were looked at as well as the house itself. The managers make sure that the people who live at the home are only given as much help as they need so they can be as independent as possible. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 2 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Processes are in place to ensure that people wishing to move to the home have their needs assessed to ensure that they can be met there. EVIDENCE: Nobody new has moved into the home since the last inspection. As an organisation, The Lady Verdin Trust has established processes which are carried out before a person moves into any of the Trust’s homes. These include obtaining a full needs assessment by the person’s funding authority. Any person wishing to move to the home would also be given the opportunity to visit and have short stays before making a decision to move there. The Commission considers Standards 6, 7 and 9 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 6, 7, 9 & 10 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The people who live at the home have opportunities to try new experiences and are given support by staff to make sure that they can live as independently as possible. EVIDENCE: One person’s personal development plan was checked during the visit. It included information on how to meet the person’s needs. The records also showed that the health of all the people who live at the home is monitored and any changes are recorded in their care plan. The care plan also contained information about the person’s likes and dislikes, together with risk assessments for activities the person was involved in such as using buses and trains and horse-riding. The care plan was last reviewed on 29 January 2007 and included the person who lived at the home, key staff and other professionals involved with their care.

In addition to the written plans each person who lives at the home has a photo diary of their activities and achievements. During the visit staff interaction with the people who live at the home was observed and was seen as appropriate. Staff were aware of key phrases and jokes to prompt each person effectively. The people who live at the home were encouraged to take part in household tasks such as putting dishes in the sink and putting the bin out; sometimes this was more successful than others. Following the last visit the manager said that she is monitoring the level of support staff give to people who live at the home to make sure this is not so much that people’s independence is reduced. Personal information about the residents was seen recorded on the calendar in the kitchen; for example, blood and urine tests for individuals. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14, 15, 16 & 17 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents are supported in a variety of ways to enable them to become as independent as possible and part of the local community. EVIDENCE: The needs of the people who live in the home are such that they need support with all aspects of daily living. On the day of the visit, two people were ready and waiting patiently for transport to go the day centre. The other person who lives at the home was leaving later for planned activities, which better suited his needs. A four-week plan was seen for one person who lives at the home, which included using public transport, making use of community facilities, shops, library and the local bowling alley. The written diary and photo diary kept by each of the people who live at the home provided evidence of visits to places of interest, activities and meals out.

Two parents completed questionnaires sent by the Commission for Social Care Inspection and both were positive regarding the home. One stated, “he is looked after by a capable and caring staff, who take good care of both his physical and social needs”. Meals and mealtimes are flexible and work around the daily activities of each person who lives at the home. A record is maintained in the home of the food served each day. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 18, 19 & 20 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The people who live in the home are supported and monitored in managing their health to make sure they receive the care/support they need to keep well. EVIDENCE: Records seen during the visit showed that the personal care needs of the people who live in the home have been assessed and plans are available that show how these needs are to be met. A separate healthcare file is kept on each person who lives at the home; these showed that they receive visits from doctors, nurses and other healthcare professionals. They are registered with the local optician and dentist. Those people who live at the home who cannot manage their own medicines are given full help from staff. One medication chart was inspected and records were satisfactory. Information provided by the manager states that staff in the home have attended medication administration training within the last twelve months.

The Commission considers Standards 22, and 23 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 22 & 23 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Processes are in place to make sure that the people who live at the home are safeguarded from possible harm. Improvements need to be made to the way complaints are recorded to show that people’s concerns are listened to and acted upon. EVIDENCE: A copy of the complaints procedure is available in the home; details on how to contact the Commission for Social Care Inspection are included. A copy of the complaints procedure has been provided in a format that makes it easier for the people who live in the home to understand. CSCI has not received any complaints about the home. The manager was spoken with during the visit and said there have not been any complaints made since the last inspection. Three completed CSCI questionnaires were received from people who live at the home. They had been helped to complete the information by support staff; two stated they did not know who to complain to. This was discussed with the manager who said that staff knew if any of the people who live at the home were unhappy with any aspect of their care by their actions and/or behaviour and what they say. The manager confirmed that these matters were always looked into and resolved by either the staff or her. Included in the procedure is a copy of the government guidance ‘No Secrets’. No referrals have been made by or about the home under adult protection. This judgement has been made using available evidence including a visit to this service. The quality of the décor, facilities and furnishings in the house ensures that residents live in safe comfortable surroundings. EVIDENCE: During the visit the communal areas of the home were inspected; bedrooms were not seen during this visit. The home is domestic in style, clean and fresh throughout. JUDGEMENT – we looked at outcomes for the following standard(s): 32, 34 & 35 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Thorough recruitment processes, together with training and supervision for staff ensures that the people who live at the home are supported by a qualified staff team and are protected from harm and possible poor practice. EVIDENCE: Training and refresher training is available for staff throughout the year. The Lady Verdin Trust provides a full programme of training to ensure that staff have the opportunity to achieve all their mandatory training. Information provided by the manager before the inspection identifies that five staff have achieved a National Vocational Qualification (NVQ) at level 2 or above in care and two staff are working towards obtaining a qualification. The manager confirmed that no new staff have been employed in the home since the last inspection. Thorough recruitment processes are in place at the Trust and include a Criminal Record Bureau checks (CRB) being completed before any new staff start working at the home. This judgement has been made using available evidence including a visit to this service. The home is well managed to make sure it is run in the best interests of the people who live there. EVIDENCE: The home manager has qualifications in both care and management. Regular staff meetings and house meetings are held so that staff and the people who live there can influence the way the home is run. Information provided by the manager showed that service contracts are up to date in relation to gas supply and electrical installation. The home’s records were inspected and are maintained appropriately in respect of the fire equipment, emergency lighting, alarms and fire extinguishers.
