This inspection was carried out on 16th July 2009.

It is an annual review prepared by CQC after examining previous reports and information from the provider. At the time of this report, CQC judged the service to be Excellent.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

If yes, what have they been: Date of last key inspection: Date of last annual service review (if applicable): Brief description of the service The Old Vicarage is an established care home, situated in the village of Leigh, about six miles from Sherborne. The home is set in spacious and attractively landscaped gardens, car parking is available. The home is registered to accommodate a maximum of 35 older people (age 65 and over), both male and female. The main house is a Victorian building, originally a vicarage which has been extended. The accommodation is spread over two floors and contains 31 single rooms and 2 Annual Service Review Page 2 of 7 Change of managers name. double rooms, most rooms are on the ground floor, 6 rooms are located on the first floor. A stair lift on the main staircase provides access to the first floor. Many of the ground floor rooms have direct access to the courtyard and gardens. Communal facilities include two dining rooms and a small lounge area. The home is registered under Tovic Limited, Annie Sinnott is the proprietor, she takes an active role in the operation of the home and is a well-respected ambassador for the social care industry. Annie Sinnott is made a Member of the Order of the British Empire (MBE) for services to social care. Natalie Adams was approved as Registered Manager in June 2007. She is responsible for the day-to-day running of the Old Vicarage and is supported by a care manager, deputy manager and office manager. Weekly fees (June 2007) range from £707.00 to £742.00. Fees include all care and accommodation costs, including meals, housekeeping, laundry and activities. Additional charges are made for therapeutic services (a qualified aromatherapy masseur and reflexologist), hairdressing and chiropody. People are expected to pay for their own personal items. Annual Service Review Page 3 of 7 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all the information that we have received, or asked for, since the last key inspection which took place on 20 June 2007 and the annual service review completed on 8 October 2008. This included: The annual quality assurance assessment (AQAA) that was sent to us by the service. The AQAA is a self-assessment that focuses on how well outcomes are being met for people using the service. It also gave us some numerical information about the service. Surveys returned to us by people using the service and from other people with an interest in the service. Information we have about how the service has managed any complaints. What the service has told us about things that have happened in the service, these are called notifications and are a legal requirement. The previous key inspection and the results of any other visits that we have made to the service in the last 12 months. Relevant information from other organisations. What other people have told us about the service. What has this told us about the service? The home sent us their annual quality assurance assessment (AQAA) when we asked for it. It was clear and detailed and gave us all the information we asked for. We also received completed surveys from 8 people living at the home. Several people had received assistance from staff in completing the forms. The surveys tell us that people receive enough information about the home prior to making a decision to move in. The homes terms and conditions are provided. Two of the respondents were not aware of whether they had received a contract or not. Everyone says they receive the medical care and support they need and that the staff listen and act on what is said. Staff are always available when needed. Several positive comments were received from people living at the home about what the home does well, including: I feel part of the whole place and always welcomed by the staff, This one (home) is really lovely and remarkable, the home cannot be bettered in ethos, because it is well led from the top by a dedicated team. Comments were also received for improvements to the service, including hurry forward planned developments as discussed and agreed at residents meetings and the reception area is too often closed. Annual Service Review Page 4 of 7 The AQAA tells us that as result of listening to what residents say a dining room has been turned into a social room for the residents with space to hold activities and events. An activities co-ordinator organises a range of activities and has set up a gardening club, which is very popular with residents. Our surveys confirmed that there are frequent activities arranged which residents want to participate with. Other comments included, activities are extremely well organised and include exercises. The garden room is available for crafts, flower arranging or socialising with friends and family. The home aims to operate at full occupancy. There have been 16 new admissions in the last year. Every resident has a detailed care plan which is regularly reviewed. Particularly noted is the care, attention and support given to residents who are terminally ill. A member of staff has designated responsibility for medication and over the year improvements have been made to the system. We received 3 completed surveys from health care professionals. These confirmed that the services assessment arrangements ensure that accurate information is gathered to provide the right service for each individual. Professionals say that the home provides, a caring, stimulating environment, and that the home creates an environment which respects people, ensures privacy is maintained and where possible leaves people self determinant. There is clear complaints procedure in place and complaints are dealt with promptly. All concerns are dealt with in a confidential manner through the confidentiality policy. Everyone responding to our survey said they knew who to talk to if they had a concern and how to make a complaint. No complaints were received during the last year. We received 9 completed surveys from staff employed at the home. These tell us that staff are given up to date information about the needs of the people they care for. The staff surveys note that the staff feel they are well supported and encouraged to undertake further training. There is a strong feeling of teamwork among the staff. Staff feel valued and are positive about their working environment. Several surveys commented on the the high standards and professionalism of the home. Surveys from people living at the home say that the staff are helpful, respectful and cheerful and give good attention. Some staff would like to be able to spend more time just talking with residents. There is strong commitment to staff training. The AQAA reports that the home has 147 places on different courses. 57 of staff are trained to NVQ level 3. Each year staff are entered into awards, to recognise their commitment to the care industry and the organisation. In the last year there have been changes to the management structure and a new deputy manager appointed. The home aims to provide a home within a home promoting independence, choice, privacy and dignity for people living at the home supported by a well trained staff Annual Service Review Page 5 of 7 team. The home continues to let us know about things that have happened since our last key inspection and they have shown that they have managed issues well. They work well with us and have shown us that their service continues to provide good outcomes for the people who use it. There is no evidence to indicate that outcomes for people who use the service have changed since the last inspection of the service. What are we going to do as a result of this annual service review? We are not going to change our inspection plan, and will do a key inspection by 20 June, 2010. However, we can inspect the service at any time if we have concerns about the quality of the service or the safety of the people using the service.
