This inspection was carried out on 4th July 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home has an excellent care planning system that reflects the level of care required to meet all the assessed needs. Residents and/ or family members are involved in the preparation and reviews of the care plans, which take place monthly. There is also an in-depth review completed twice a year.

What has improved since the last inspection?

Since the last inspection the new care planning system has been completed and all the reviews are up to date. A system for storing and recording controlled drugs has been introduced. The records are all up to date and correctly completed.

Date of last inspection 05 January 2005 Brief Description of the Service: The Old Vicarage Vicarage is owned by Mrs Barbara Siddiqi who is also the registered manager. Mr Kamal Siddiqi acts as the business manager. The home is situated in the quiet village of Allithwite, which is within two miles of Grange over Sands. There is a bus route near by and the nearest shop is half a mile away. The Old Vicarage is registered to care for up to nineteen older people, nine of whom may have varying forms of dementia. The building is an older prperty that has been adapted for its present use as a care home. There are thirteen single rooms and three doubles, although all the rooms are currently used for single occupation. Accomodation is provided over two floors with the upper being served by a passenger lift. The toilets and bathrooms are equipped to assist people with a disability. There is a lounge, dining room and conservatory providing communal space for the residents. This was an unannounced inspection of the home, and took place over one day. During the visit, time was spent talking with the proprietor, business manager, deputy manager and care staff on duty. Records to do with the dayto-day running of the home and the care of residents were examined and time was spent with some of the residents individually and in a group. During the inspection, some parts of the home were looked at. What the service does well: What has improved since the last inspection? This would be of benefit to the residents and ensure the staff received more help and support to meet the assessed needs. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 3 & 5 The home’s information pack provides prospective residents and their families with details of the services the home offer so that an informed decision can be made about admission to the home. Residents benefit from an admission procedure that ensures all the assessed needs can be met and all interested parties are invited to visit the home prior to admission. EVIDENCE: The owner has made slight alterations to the Statement of Purpose and residents’ charter in order to ensure all the information is up to date. A copy of the information pack is made available to any one who makes an enquiry about admission to the home. There is a clear admission procedure, which includes a full assessment of needs, to ensure these can be met. Details of the assessment are held on the care plan. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8 , 9 & 10 The home has a clear and consistent care planning system, which ensures residents health, and social care needs are met in a way that promotes their privacy and dignity. The medication at the home is well managed promoting good health. EVIDENCE: The care plans are excellent and contain information about residents assessed needs, including moving and handling. They are regularly reviewed and updated with the involvement of the resident. There is also a six-monthly review with the key worker, resident and a family member. The plans provide the care staff with the information they need to meet resident’s needs. Records are kept about GP appointments and when district nurses visit, and people said that an appointment was made as soon as they asked to see the doctor or nurse. The arrangements for storing and administering medication in the home are safe and well organised, and residents received their medication as prescribed. All controlled drugs are recorded with the records signed by two members of staff. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13 & 5 Social activities provide residents with a range of choices and opportunities to stimulate them. Dietary needs of residents are catered for with a balanced and varied selection of food available and offer a choice at each meal EVIDENCE: There are activities available for the residents with details of the daily/ weekly programme on display in the hall. All residents are given the opportunity to join in if they wish. There are some highly dependent residents and they are encouraged to take part in activities that will stimulate them. The home organises outing during the summer months. There is a six-weekly menu with a choice at each mealtime providing residents with a variety of food. The menu is displayed so that people can see what is available. Special diets are catered for, and information is recorded about residents’ dietary needs and preferences. Meals can be taken in the dining room or in the person’s own room. Those residents who wish are able to attend Church on a Sunday and monthly Communion is provided for those wanting to partake. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 & 18 The home has a satisfactory complaints system with evidence that residents feel that their views are listened to and acted upon. Staff understand adult protection issues, which protected residents from abuse. EVIDENCE: Residents are given information about how to complain when moving into the home, and said that if they raised any issue it would be dealt with promptly. Information about making a complaint is displayed in the entrance area of the home. The deputy manager and staff on duty during the inspection showed an awareness of adult protection issues and this subject is covered during NVQ training with external abuse and neglect training completed in March of this year. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 22, 23 &26 Residents benefit from a warm, comfortable and safe environment with rooms that are suitable to meet their needs. EVIDENCE: The maintenance and upkeep of the home is in the hands of the business manager who uses external contractors for any work required. There is a large garden for residents to enjoy and ample communal space for activities or just chatting with friends. There is specialist equipment to assist any resident with disability. The bedrooms are, on the whole, well decorated and provide the residents with personal space if they want to spend time by themselves. Domestic arrangements in the home ensure it is clean, pleasant and hygienic. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 29 & 30 Residents benefit from trained and experienced staff. All are appointed following a thorough recruitment procedure. This ensures maximum protection for people. EVIDENCE: There is a full complement of staff, some of whom are trained to NVQ levels two and three. There were two members of care staff on duty on the day of the inspection, plus the cook, domestic and the business manager. These staff were able to attend to peoples needs, keep the home clean and provide meals and snacks during the day. The home has a full recruitment and selection process and no new staff are appointed until the Criminal Records Bureau check has been completed. These measures contribute to the protection of the residents. There is a full training programme available. JUDGEMENT – we looked at outcomes for standard(s) 32, 33, 34, 36 & 38 There is clear leadership, guidance and direction to staff to ensure residents receive consistent, high quality care. Good financial procedures and professional accountants safeguard residents. Staff benefit from regular management supervision and appraisal. Health and safety policies provide a safe environment for staff and residents EVIDENCE: There is a warm and relaxed atmosphere in the home providing the residents with a safe and friendly environment. The deputy manager works hard with the staff to promote good care practices and ensures the home is run in the best interests of the residents. She has recently completed the NVQ level four, the NVQ assessor’s award and the registered manager’s qualification.
