Registered for 19 older people (OP) of whom 9 may have dementia (DE(E)). 4th July 2005 Date of last inspection Brief Description of the Service: The Old Vicarage is owned by Mrs Barbara Siddiqi who is also the registered manager. Mr Kamal Siddiqi acts as the business manager. The home is situated in the quiet village of Allithwite, which is within two miles from Grange over Sands. There is a bus route near by and the nearest shop is half a mile away. The Old Vicarage is registered to provide accommodation for up to nineteen older people, nine of whom may have varying forms of dementia. The building is an older property that has been adapted for its present use as a care home. There are thirteen single rooms and three doubles, although all the rooms are currently used for single occupation. Accommodation is provided over two floors with the upper being served by a passenger lift. The toilets and bathrooms are equipped to assist people with a disability. There is a lounge, dining room and conservatory providing communal space for the residents. The Old Vicarage (Allithwaite) DS0000022646.V271306.R01.S.doc Version 5.1 Page 5 SUMMARY
This is an overview of what the inspector found during the inspection. This was an unannounced inspection of the home and the second one this year, with the visit taking place over late morning and early afternoon. During the inspection, time was spent talking with the deputy manager, business manager, care staff on duty and the cook. The inspector looked at the care plans and other documentation and records to do with the day-to-day running of the home and the care of residents. Time was spent talking with some of the residents in the lounge and dining room. The standards not assessed during this inspection were inspected and met during the previous visit that took place in July 2005. What the service does well: What has improved since the last inspection?
Two new service contracts have been started, one for the gas appliances and one for the hot and cold water tanks and points.

Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2&4 Residents and their families benefit from the opportunity of visiting the home prior to admission. Residents are safeguarded by the signing of a formal contract/terms and conditions. EVIDENCE: All residents have a contract and terms and conditions, which is issued when they are admitted to the home. Prospective residents and/or their families are encouraged to visit the home and meet the staff and other residents before deciding whether or not to move in. This gives the opportunity to discuss whether or not the assessed needs of the prospective resident can be met. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8 & 11 The healthcare needs of the residents are met, with evidence of a good working relationship between the home and the healthcare professionals. Residents benefit from a staff team that treats them with respect at all times. EVIDENCE: The home has an excellent care planning system in place and this standard was assessed and met during the previous inspection. However, the inspector was shown a number of care plans that have been fully operational for the last 12 months. These gave an in-depth picture of the residents’ health and social care needs over during the year and how those needs have been met. Discussions with the deputy manager and care staff confirmed that the home has a good working relationship with the visiting healthcare professionals. The district nursing service is currently visiting the home on a daily basis and the local general practitioners visit whenever a request is made. All details of healthcare visits and appointments are documented in the residents’ files.

Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14 The staff have a good understanding of the residents’ support needs. This was evident by the interaction between them and the residents. Residents benefit from being given freedom of choice about how they spend their days. EVIDENCE: It was evident during the inspection that all the staff interacted well with the residents and were seen to ask them where they wished to sit and what they wanted to do. The inspector observed the caring way in which residents were spoken to and the manner in which the staff approached them. Residents’ meetings do not take place often but those residents who spoke with the inspector said, “We can do what we like. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 17 Service users benefit from the availability of an advocacy service should this ever be required. EVIDENCE: Currently all the residents have either their families or solicitors to assist them to manage their affairs. The deputy manager was, however, aware of the advocacy service and the residents are given advice how to access this should it be necessary. Residents had taken part in past elections through the postal voting system, and had been helped with this by staff when necessary. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 21, 24, 25 There has been no change in the décor or furnishings in the last 12 months and although this does not pose a risk to the residents, the environment could be enhanced by some internal decoration. EVIDENCE: Discussions with business manager evidenced his awareness that the internal decoration of the home now needs some attention, particularly in the communal areas. He was able to produce a copy of the business plan for the next financial year that showed proposed renovations and improvements to the interior of the home. Six of the bedrooms have en-suite toilet facilities and there are sufficient communal bathing and toilet facilities to meet the residents’ needs. A sample of the bedrooms was inspected and they were found to be safe and made personal to the residents with ornaments, pictures and photographs. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28 & 30 Residents benefit from a staffing policy that ensures there are sufficient numbers of staff are available with the necessary skills to meet the assessed needs. EVIDENCE: The inspector was able to speak with the care staff that were on duty at the time of the inspection. They were appreciative of the support they received from the deputy manager and the training that is organised by the registered provider. Over 70 of the care staff have completed the NVQ level 2 or 3 and all mandatory training is up to date. This includes, first aid, manual handling and food hygiene. Staff are also trained in medication, infection control, fire awareness, dementia care and protection of vulnerable adults. the business manager provided the inspector with a schedule of training he hopes to organise for this year. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 32 & 37 Residents benefit from a deputy manager who gives guidance and direction to staff to ensure residents receive consistent, high quality care. EVIDENCE: Currently the deputy manager is responsible for the running of the home and ensures that the residents benefit from her relaxed style of management. She works closely with the registered provider and business manager to make certain all the assessed needs are met. She has completed her NVQ level 4 in management and the Registered Manager Award and is also an assessor for the NVQ qualification. Added to this she has considerable experience in the care of older people. The home has a full set of policies and procedures that are available for the staff to access when they wish.
