Registered for 19 older people (OP) of whom 9 may have dementia (DE(E)). 9th January 2006 Date of last inspection Brief Description of the Service: The Old Vicarage is owned by Mrs Barbara Siddiqi who is also the registered manager. Mr Kamal Siddiqi acts as the business/general manager. The home is situated in the quiet village of Allithwite, which is within two miles of Grange over Sands. There is a bus route near by and the nearest shop is half a mile away. The Old Vicarage is registered to provide accommodation for up to nineteen older people, nine of whom may have varying forms of dementia. The building is an older property that has been adapted for its present use as a care home. There are thirteen single rooms and three doubles, although all the rooms are currently used for single occupation. Accommodation is provided over two floors with the upper being served by a passenger lift. The toilets and bathrooms are equipped to assist people with a disability. There is a lounge, dining room and conservatory providing communal space for the residents. The Old Vicarage (Allithwaite) DS0000022646.V295563.R01.S.doc Version 5.2 Page 5 SUMMARY
This is an overview of what the inspector found during the inspection. This unannounced visit, which forms part of the key inspection, took place on one day in July. During the inspection, time was spent talking with the general manager, the deputy manager, senior care staff, and care staff on duty. The inspector was also able to speak with residents, many of whom were sitting in the garden. The inspector also spent time looking at records to do with the day-to-day running of the home and the care of residents. The report may refer to “case tracking”, a process by which the inspector is able to focus on a small number of residents and includes a review of their care documentation. It should be noted that this process is not detrimental to the other residents living in the home Time was spent with some of the residents individually and in groups, and most parts of the home were looked at. The fees in this service range from £390.00 - £440.00 per week as at April 2006. There is a charge for hairdressing, aromatherapy, newspapers, chiropody and personal toiletries. What the service does well:
All prospective residents have a full assessment before moving into the home to ensure all individual needs can be met. A complete care planning system is in place with most of the residents involved in the process. All care plans are signed by the resident or a family member. The details on the care plans ensure the carers are given all the information they need to deliver a high level of care and meet the assessed needs. All healthcare needs are met and there is an up to date record of professional healthcare visits and appointments documented in the care plan file. Arrangements are made for residents to have access to chiropody, optical services and dental treatment. There is a programme of organised activities for residents to join in if they wish, and Holy Communion is offered to those who want to partake. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2, 3, 4 & 5 Quality in this area is good. Admissions to the home only take place if the manager is confident staff have the skills, ability and qualifications to meet the assessed needs. The assessment process is good and ensures the needs of the residents can be met. Each resident is provided with a contract and terms and conditions that sets out in detail the facilities the home provides. Prospective residents are given the opportunity to spend tine at the home prior to admission. This judgement was made using available evidence including a visit to the service. EVIDENCE: All prospective residents are fully assessed prior to admission to ensure the home is able to meet the assessed needs. Information obtained from this assessment is then used as a basis for the plan of care. All residents have a contract and also a copy of the terms and conditions of residency. Copies of these documents are held on each file.

This also gives opportunity to view the accommodation provided. The needs of those people already living in the home are taken into account when assessing prospective residents. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 & 10 Quality in this outcome area is good. Residents health, personal and social cares needs are addressed in the care plans and care needs are met effectively. The home works to an efficient medication policy supported by procedures and practice guidance. Residents are treated with dignity and respect at all times. This judgement was made using available evidence including a visit to the service. EVIDENCE: The deputy manager has responsibility for the care planning system, which is comprehensive and extremely effective. Each resident has a care plan that is used as a working tool and is understood by all staff. It is written in clear language with resident involvement wherever possible. Each care plan includes a comprehensive risk assessment. Management of risk takes into account the needs of residents and demonstrates a balanced view in

The rights of others living in the home are also considered when drawing up an assessment of risk. The care plans are updated each month and are completely reviewed with the resident every six months. All professional healthcare visits are recorded in detail and the deputy manager and senior carer confirmed that they have a very good working relationship with the doctors and district nurses who visit the home. The medication is received in a monitored dosage system and all the care staff responsible for giving out the medication have completed training in “safe handling of medication”. Records were checked and found to be in order. The deputy manager and senior carers have recently completed a level 2 course in safe Handling of Medication. As a result, the deputy manager has put a more efficient audit trail as a further safeguard for residents. Residents who spoke with the inspector said that the staff always treated them with respect and kindness and that any personal care required is given in the privacy of their own rooms. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 & 15 The quality in this outcome area is good. The routines of the home are planned around the residents’ needs and wishes and are flexible enough to meet the changing needs of the individual. Family and friends feel welcome and know they can visit the home at anytime. Maintaining independence and enabling the residents to make their own decisions about how they wish to live is a key objective for the home. An experienced cook is responsible for providing quality, nutritional meals that meet the cultural and dietary needs of the residents. This judgement was made using available evidence including a visit to the service. EVIDENCE: Discussions with the residents evidenced that they all have control over their lives. They can rise when they want to and retire when they wish. Some of the residents were in their rooms during the inspection, whilst others were enjoying conversations in the lounges. Those who spoke with the inspector said, “we can do what we like “ and “ the staff let us please ourselves”.
The Old Vicarage (Allithwaite) DS0000022646.V295563.R01.S.doc Version 5.2 Page 13 There is a programme of activities based on what the residents like to do and some of the residents were in the garden enjoying handicrafts on the day of the visit. Other residents were sitting in the shade enjoying the sunshine. Plans were being made for a strawberry tea arranged for the week following the inspection. A volunteer brings a “pets as therapy” dog to visit the residents. Contact with friends and family is encouraged and visitors are always made welcome, although no one was visiting on the day of the visit. The inspector was in the home over the lunch period and discussions with the residents evidenced that all were happy with the choice of food. Special diets are catered for. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16, 17 & 18 Quality in this outcome area is good. The home has a complaints procedure in place that is clearly written and easy to understand. Training in the area of protection has been completed and staff are aware of the procedure to follow. Residents are made aware of their rights and their ability to access places of worship and community facilities of their choice. This judgement was made using available evidence including a visit to the service. EVIDENCE: The home has a complaints procedure that forms part of the information pack given to all prospective residents and/or their families. There is a complaints register in place but there have been no complaints to record. Currently all the residents have family members or solicitors to help with their affairs but the home has previously used the advocacy service provided by Age Concern South Lakeland. All service users who wished to vote were provided with postal votes. The care staff have completed adult protection training and this subject is also covered in the staff induction programme and NVQ training. The staff who spoke to the inspector showed a good knowledge of abuse issues and the process to follow should this ever be required. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 22, 23, 24, 25 & 26 Quality in this outcome area is good. The home offers very comfortable, homely accommodation and all areas are quite well maintained. This contributes to a good quality of life for the residents. The home provides aids and equipment to meet the care needs of the residents. The residents’ rooms are suitable to meet the assessed needs. This judgement was made using available evidence including a visit to the service. EVIDENCE: The home provides comfortable surrounding with reasonably good quality furniture and fittings. Since the last inspection two of the residents’ rooms have been re-decorated and two vanity units have been replaced. Arrangements are being made for a further 4 to be replaced in the near future.

There is also a conservatory that opens out onto the garden. There are well kept gardens with garden furniture for the residents to enjoy during the warm weather All the bedrooms are for single occupancy, with 6 having en-suite facilities. Two have recently been re-decorated. The general manager is aware that some other bedrooms need to be redecorated and plans are in hand for a further 4 to be completed. Plans are also in hand for the ground floor to be refurbished to include new carpets. The home has specialist equipment for those residents requiring it. This includes, handrails on corridors, assisted bathing, raised toilet seats and hoists. All of these facilities ensure the residents live in safe, comfortable and suitable surroundings. Domestic arrangements in the home ensure that it is kept clean and the risk of cross infection is kept to a minimum by the staff adopting good practices. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29 & 30 Quality in this outcome area is good. Staffing arrangements are good, ensuring the needs of the residents are met. Staff are well trained (having completed the required training) and provide a skilled and experienced workforce. This judgement was made using the available evidence including a visit to the service. EVIDENCE: The home currently has 1 full time staff vacancy but generally has a very low staff turnover. There are general assistants employed in addition to care staff and this arrangement has been of benefit to the residents and staff. It has meant that the care staff have more time to deliver the care to the residents. The staff team is both experienced and qualified with many having already completed NVQ levels 2 and 3 and others working towards the awards. The deputy manager has completed the Registered Manager’s Award and is also an NVQ assessor. The inspector was able to observe the staff and found their attitude to be caring, supportive and enabling. This attitude ensures that the residents can maintain as much independence for as long as possible.

Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 32, 33, 35 & 38 Quality in this outcome area is good. The deputy manager has the required experience and qualifications and is competent to run the home and is highly regarded by the staff and residents. The home has comprehensive policies and procedures that are reviewed on a regular basis. Sound health and safety procedures ensure the residents’ safety. This judgement was made using the available evidence including a visit to the site. EVIDENCE: The deputy manager, who holds the registered manager’s award and has considerable experience in the care of older people, is currently responsible for running the home. She is resident focused and works hard to ensure the home provides a high standard of care. During the visit she demonstrated an awareness of the residents’ needs and the importance of meeting those needs.

The home does not hold any personal monies on behalf of the residents and any personal expenditure is paid by the home and recovered via monthly accounts. The general risk assessments are completed by the general manager and are reviewed annually and there is also a fire risk assessment in place. The gas safety check was up to date and all the equipment is serviced regularly under annual contracts.
