This inspection was carried out on 31st January 2006.

What the care home does well

The home continues to offer a homely and comfortable environment in which to live. The systems in place in the home ensure the safety and welfare of the residents.

What has improved since the last inspection?

Internal decoration of the home is ongoing.

What the care home could do better:

The toilet on the first floor is in need of repainting and some new linoleum. There were no requirements and no other recommendations made following this inspection.

One Service User can be accommodated with Category of Mental Disorder (MD) The Home is registered as a Care Home with Nursing for a maximum of 27 Service Users in the categories of PD(E) 27, OP 18 14.06.05 Date of last inspection Brief Description of the Service: The Yelverton Nursing and Residential home is a Victorian house that has been successfully modified over the years to provide nursing and residential care to a maximum of 27 Service Users. The home is very close to Yelverton village and there is level access to local amenities. The home is on three floors with passenger lift or stair lift access to all parts of the building. It offers a variety of accommodation, to include two double rooms and many rooms with en suite facilities. There is a pleasant patio approach to the home and a large patio area at the rear.The home employs registered nurses to deliver nursing care and to support care staff in the delivery of care. The home is well equipped to deliver the care for which it is registered. A varied inhouse activities programme is available for those able to take advantage and appropriate transport can be hired as necessary for any trips that are arranged. The unannounced inspection took place over 2 hours and 25 minutes and was conducted with the matron. The inspector spoke generally to a number of residents and staff. The main purpose of this inspection was to assess the core standards that were not assessed during the last inspection (June 2005). A brief tour of the premises took place and a variety of records were inspected. On the day of the inspection the home had 24 residents (10 of whom require nursing care and 14 who require personal care only). Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 The medication at the home is well managed, promoting good health EVIDENCE: The manager demonstrated the homes procedures when handling medications. The receipt, recording, storage, handling, administration and disposal are all in accordance with laid down legislation. The drug fridge temperatures are recorded daily and all of the stock examined in the treatment room was in date. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14 The staff encourage residents to remain as independent as possible allowing them to make personal choices and maintaining some control over their life. EVIDENCE: A tour of the home confirmed that residents are encouraged to personalise their rooms with their own possessions. Discussion with the manager during this and past inspections confirmed that residents are supported to manage their own financial affairs if possible and that residents come and go as they please either independently or with relatives/ friends who might take them shopping, out for trips or to the local church for example. The home has policies and procedures about access to personal records held concerning individual residents. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 (partially) The standard of the environment within the home and the grounds is good providing clients with attractive and homely place to live. EVIDENCE: During a tour of the home it was noted that painting of corridors was ongoing and the manager said that some rooms are being done also. The residents toilet on the first floor was in need of repainting and the linoleum was looking in a poor condition and needs replacing. The front ‘garden’ of the house was looking tidy and well kept. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 29 The homes recruitment policy and practice protect the residents. EVIDENCE: Staff files include 2 written references, an up to date CRB check and relevant personal information required by Schedule 4. All potential staff are interviewed using a standard format/ scoring system. Once employed staff are issued with terms and conditions of employment and are issued with a GSCC code of conduct. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for the following standard(s): 33 The systems in place in the home promote and protect the health and safety and welfare of the clients and staff. EVIDENCE: The home uses the RNHA ‘Blue Cross System’ of quality assurance. The manager completes the sections when they are due for review as an ongoing process. The completed audit summary form indicates if an area is not met and gives opportunity to record the actions to be taken to meet the element. Quality assurance questionnaires were distributed to staff and residents (or their representatives) in November 2005. The results are to be collated and any actions required will be addressed by the manager and owner as necessary. When visiting the home he meets with staff, residents (and sometimes relatives) and tours the home to review the general condition of the house and the atmosphere within the home.
