Conditions of registration: None Date of last inspection 8 September 2004 Brief Description of the Service: West View is a privately owned care home that provides care and accommodation for older people. It is situated in the village of Bere Alston and involves itself in village life as much as possible. Accommodation is available on three floors, access between floors being provided by stairs and stairlifts. Rooms are well presented and the registered provider has an on going programme to upgrade the accommodation and fit en suite facilities where ever possible. There is a choice of communal space, which includes a conservatory and outdoor patio for use in fine weather. The exterior is well maintained with colourful hanging baskets etc. There is a garden and greenhouse at the rear, available for anyone active enough who wishes to take an interest in this. This is an overview of what the inspector found during the inspection. This was a unannounced inspection that took place between 9.45am and 4.30p.m. The inspector spent time with the registered provider, service users and care staff. Records were inspected and any issues discussed. The inspector had lunch with service users who described how they spent their time in the home. The home was full although one person was in hospital at the time of this inspection. The home was felt to provide a good caring service for its residents. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,3,4,5, Service users are offered accommodation following an assessment to ensure that their needs and the home’s ability to meet these are compatible. EVIDENCE: The statement of purpose and service users guide continue to reflect the services offered. They are provided for all service users. Potential service users are visited and assessed by the Care Manager with any required information being sought from the relevant agencies. Assistance and advice is sought from other professional bodies or agencies, as needed, for service users. This supplements the care provided by the home’s care staff. The home is clear about the level of care that it can provide and where a person’s needs are felt to exceed this, the staff will help in finding more suitable accommodation. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9,10,11 Attention to the service users’ health and personal care is afforded a high priority. EVIDENCE: A problem orientated care plan with goals and an action plan is available for each service user. They are updated regularly. The carers make regular entries in the contemporary notes; this may not always be daily if nothing of any consequence has occurred. There was a discussion about reviewing entries and whose responsibility this should be. Staff felt that they were a close team and all issues of importance and concern were always passed on at handover. It is important to remember that previous thorough recording becomes vital should any incident/injury develop after an event. The service users are well supported by the local Health Centre in the village and the community nursing service. A number of the service users, having been resident in the village prior to moving to West View, appreciate being able to retain the services of the same doctors. West View D52-D04 S43014 West View V246156 220805 Stage 4.doc Version 1.40 Page 10 Medication is carefully documented and recorded on medication administration record sheets so that it adheres to the home’s written policies and procedures. A member of staff has the responsibility for ordering and checking medication as it arrives and returning unused items. Certain service users look after their own medication. It is documented and discreetly monitored. Service users said they had no problems with regard to their privacy and dignity, which they felt was respected at all times. The home always wishes to care for a person until his or her life’s end and is usually successful in doing so unless admission to hospital is indicated. Letters commending the home’s care were seen. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,15 Service user can choose whether or not to participate in the social activities that are arranged within the home and are encouraged to involve their families whenever occasions arise to do so. EVIDENCE: The home is commended for the way in which it promotes social activities and mental stimulation for the service users. It seeks active input from the service users via its residents’ committee and produces minutes of the matters discussed as well as a monthly newsletter. The latter contains the events planned for the coming month, which allows people to choose what they wish to join in with. Activities include a regular Fellowship group, swimming, film/video afternoons, outings and various musical events. Service users commented that they had enjoyed barbecues to which families and friends had been invited. There is a mix of people in the home, some of whom are mentally frail and may wander around the various communal areas. It was noted that other service users were tolerant and caring towards this group. The registered person said he tried to keep a balance of service users in the home as he wished to offer a service that reflected all levels of the community. The

Visitors were seen to come and go throughout the day and people went out either with their families or into the village. JUDGEMENT – we looked at outcomes for standard(s) 16,17,18 Service users or their relatives can be assured that any complaints or concerns are taken seriously and that staff are alert to any incidents of abuse. EVIDENCE: The home has policies and procedures to protect service users from abuse and staff have attended courses on this. The complaints procedure is displayed. JUDGEMENT – we looked at outcomes for standard(s) Apart from looking at the alterations that have been completed the premises were not inspected. There has been no change to the registered numbers. The intended outcomes for Standards 27 – 30 are: 27. 28. 29. 30. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,28,29,30 The staff were positive about their work and enjoyed the opportunities provided to improve their knowledge and skills. The home is commended for its commitment to training. EVIDENCE: Staff commented to the inspector that they received plenty of training opportunities, which they enjoyed. They indicated that this gave them a continuing interest in their work, which was one of the reasons they chose to stay at West View. National Vocational Qualifications are encouraged and ‘Away Days’ have been arranged for staff. Anyone who attends a seminar arranged by an external body is expected to cascade the information gathered to other staff. Recently this has included No Secrets, holistic care and dementia awareness. Written references are taken up for any new staff with Criminal Records Bureau & PoVA checks made. If, due to staffing shortages, a person starts their induction prior to all checks being returned the registered person said that they never worked unsupervised. One carer aged 17 years is employed in the home. She works with on non-caring activities or may occasionally assist with for example toileting, under supervision. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,32,33,34,35,36,37 The home is run in a positive way that has the best interests of the service users at heart. EVIDENCE: A trained nurse is employed as the Care Manager for the home. She understands her role in a non-nursing care setting and liaises with the registered person to provide the best care for service users. An administrative assistant assists the registered provider. The home is commended for the way in which it seeks to involve service users and their relatives in assessing the services offered by the home. This is sought via the residents’ committee comments and periodic questionnaires. The home prefers not to administer service users’ finances, asking families to undertake this. Any extra expenses are billed on a monthly account. This is understood by the service user and their representatives on admission. The home likes to ascertain whether or not there is Power of Attorney appointed in behalf of any service user. Records required by statute were inspected and found to be satisfactory. The home’s health & safety policies were not inspected on this occasion.
