Conditions of registration: 1. Service users in the category PD must be at least 25 years of age. 2. A maximum of 4 service users in the categories PD/PD(E) may be accommodated at any one time. 3. Service users in the categories PD/PD(E) are only to be accommodated on the ground floor. Date of last inspection 14.02.05 Brief Description of the Service: Whitegates care home provides accommodation and support for twenty one older people, four of whom with physical disabilities may also be accommodated on the ground floor of the home. The home is situated in a quiet residential area, just north of Ringwood town centre. The already much improved home continues to benefit from regular decoration and maintanence, the garden is well presented. This is an overview of what the inspector found during the inspection. This unannounced inspection took place over six hours. The inspector was assisted during the inspection by Mrs Kay Chapman, the registered manager, and Mr JJ Dubois, the responsible individual. Despite work being carried out to replace internal doors, and decorating at the time of the inspection, the home was clean and appeared homely. The people living at Whitegates prefer to be referred to as residents. Nineteen of the twenty residents were at the home, the inspector spoke with the majority of the residents and two visitors. The local Doctor made a house call during the inspection. What the service does well: What has improved since the last inspection?

The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3,6 A comprehensive procedure for assessing the needs of potential new residents is in place to ensure the resident and the homes needs are met prior to admission. Whitegates does not provide intermediate care. EVIDENCE: The inspector witnessed a residents file which included an enquiry form, and a completed assessment form which was completed during the residents day visit to the home. Family/representatives are invited to participate in the completion of the assessment to obtain as much information as possible. The manager confirmed this procedure and informed the inspector of the importance of the prospective resident spending a day at the home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9 Residents health, personal and social needs are fully met, as set out in individuals care plans and risk assessments. The system for the administration of medication, which included residents slf medicating, are good with clear and comprehensive arrangements being in place to ensure residents medication needs are met. EVIDENCE: One resident explored their care plan with the inspector, which demonstrated the amount of involvement the service user has in completing all aspects of their care plan and risk assessments. The care plans and risk assessments are reviewed on a monthly basis, unless needs change sooner. The inspector witnessed the administration of the lunch time medication, ( the home uses the monitoring dosage system and monitoring administration record sheets) and the records kept in conjunction with medication received and returned. One resident self medicates, as per their care plan, the inspector recommended a risk assessment be completed for this activity. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 13,14 Links with the local community are good, and contact with family, friends and representatives are encouraged. Practices in the home demonstrate that the home promotes individual choices and encourages residents to have control over their lives. EVIDENCE: A record of visitors to the home was seen by the inspector, which showed family, friends, social workers and health professionals ( a Doctor visited his patient at the home at the time of the inspection which was carried out in the privacy of the residents bedroom). The inspector asked a small group of five residents what sort of activities they enjoy participating in, all explained each year the home organises a “family and friends” day which is held in the garden. The manager and one of the visitors confirmed this. A local taxi firm takes small groups of people to places of local interest, music/sing along afternoons are enjoyed within the home, so too are quizzes. . One service user said “I enjoy going for a walk by myself, I inform staff when I`m going and when I return, to be courteous, I am free to come and go as I please.”

The majority of residents preferred a cold lunch, so the menu was adapted accordingly. The inspector witnessed one resident (who was enjoying a hot meal, not a salad) ask staff for some beetroot, as they saw it was provided with the salad, the request was provided promptly. The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 Residents and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon, however, the procedure for logging complaints requires revising. EVIDENCE: The inspector witnessed the complaints log and found one complaint, the outcome of the complaint was not logged. The inspector recommended that the pages of the log be numbered consecutively to show a true record of complaints received. The homes complaints procedure was viewed by the inspector, a copy of which had been sent to “the plain English campaign” to be approved. Two visitors said they have not had the need to complain, if they did, they were sure adequate action would be taken. Residents stated they were sure their complaint would be taken seriously and be dealt with appropriately. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,26 Residents live in a safe, well-maintained environment. The home is clean, pleasant and hygienic. EVIDENCE: At the time of the inspection, parts of the home were being re decorated, and new doors were being replaced which presents a well maintained, comfortable home. The gardens are well maintained and residents enjoy using it, to walk around, sit in or look at from inside the home. The inspector witnessed the homes procedures to ensure hygiene is maintained, visitors are requested to use the hand wash provided at the door before entering the home, staff use protective clothing whilst completing their duties of personal care, cleaning and supporting at mealtimes. Staff said they receive training on infection control, provided by “Pinnacle” training. The inspector witnessed the advice sought, and put into practice following a resident contracting MRSA. The policy on infection control was not available, as it had been sent with the complaints procedure to be approved.
