The building offers52009 accommodation to a maximum of 80 people, in the main building there are three floors; the ground floor offers a service to people needing specialist dementia care, the first floor is for older people and the second floor is a 25 bedded nursing unit. To the side of the building is another unit specialising in intermediate care helping people to return to independent living.The accommodation includes single rooms only and each has en-suite facilities including walk-in shower. What the care home does well The home has a good audit process for medicines which is carried out diligently by senior staff. People we spoke to on the intermediate care unit were appreciative of the way staff had looked after them and organised their medicines for discharge.All the staff we spoke to had a clear understanding of the importance of accuracy and maintaining clear records when administering medicines. What the care home could do better: Where people`s medicines have been omitted due to their social commitments a record of the discussion with the person`s GP should be kept. Homely remedies should only be used for up to 3 days before seeking the guidance of the GP. Care Homes for Older People Page 3 of 8 What we found: This inspection was a random inspection visit by the Pharmacist Inspector to look at all areas of medicine handling in the home. We looked at the arrangements for storage and record keeping on the nursing, residential, dementia and intermediate care wings and observed a medication round in the intermediate care unit. We talked to staff on all four units and some people who were being supported in the intermediate care unit prior to being discharged to their own homes. The home has a comprehensive audit procedure for medicines and we saw evidence of senior staff following up errors in documentation that had been made by nurses, senior carers and agency staff. Medicines on all four units are stored securely, including controlled drugs. Appropriate records are kept of medicines received, administered and returned to the pharmacy for disposal or given back to people when they return home. The medication round we observed was conducted accurately with regard to good practice and peoples individual needs. Separate records are made by the carers of any creams or lotions that are used in peoples care. Some people are supported to manage their own medicines, this can be particularly important on the intermediate care unit where they may be returning to their own homes and need to maintain the necessary skills. Risk assessments are carried out and reviewed regularly for these people and suitable lockable storage is available. The records we examined showed the instructions of other healthcare professionals had been recorded and put into practice. Care plans were drawn up for medicines, including those prescribed for use as required. People in the home take part in activities away from the home and for some people it was found by the nurses to be necessary to omit a dose of medicine to allow them to take part, there was no record of a discussion with the persons GP about this situation. The home has a comprehensive policy and procedure, information about medicines and training on the safe handling of medicines available to all staff. The list of homely remedies is small and appropriate. One person had been receiving homely remedies for longer than the recommended three days and the record of the medicine on their administration chart did not include the dose. This person should have been referred to their GP. The home has a safe and well managed system for handling medicines and people are allowed to exercise their own choices as to how those medicines are given to them.
