What the care home does well Good assessment procedures ensure that the needs of the individual are understood and can be effectively met. The residents can be confident care will be provided in line with their individual wishes and preferences. People who live in the home are encouraged and supported to be as independent as possible. They have the opportunities to take part in a range of activities inside and outside the home, collage, evening clubs and at day care centres. Provisions are in place to ensure that the residents can be confident that their personal and healthcare needs will be met in a way that respects their individual wishes, preferences and upholds their privacy and dignity. The service has a robust policy for the safe handling of medication.The Commission for Social Care Inspection has not received any complaints about the service since the last inspection. The people who live at the service are protected from abuse by rigorous policies and procedures that are fully implemented. People who live in the home benefit from the support of a committed, well trained and competent staff team. The staff and manager understand their needs and actively promote their best interests. The home is managed in the best interests of the people who live there. There is strong evidence to indicate that individuals are listened to and are encouraged to participate in the operations of the home. What has improved since the last inspection? In line with a requirement from the previous inspection report the service has become more proactive in the reporting of any incident that falls into the remit of safeguarding adults. Evidence sampled during this site visit indicated that incidents are now reported without any delay to the Local Authority Social Care Team and to the Commission for Social Care Inspection. The registered person has implemented a local safeguarding adult protocol. All staff have undertaken training in infection control. Paper disposable hand towels are now available in the home. What the care home could do better: No requirements were made during this site visit. However four recommendations have been made for consideration. This means the people who use this service experience good quality outcomes. The Commission has since the 1st April 2006, developed the way it undertakes its inspection of care services. This inspection of the Service was an unannounced “Key Inspection”. It was a thorough look at how well the Service is doing. It took into account detailed information provided by the Service’s in the form of a Annual Quality Assurance Assessment and any other additional information that the Commission for Social Care has received about the Service since the last inspection. The inspector spoke to four of the people who live at Fengates to gain their views on the care provided. The Commission for Social Care received four comment cards about the service and these comments are reflected in this report. A partial tour of the premises took place the inspector was unable to gain access to the bedrooms on the request of the people who occupied the rooms. Staff training records, staff files and policies and procedures were sampled. The inspector spoke to two members of staff. The inspector would like to thank the residents, the manager and staff members for their time, assistance and hospitality during the inspection. A robust assessment procedure is in place this ensures that the people who use the service can be assured that their needs will be highlighted and can be met by the service. EVIDENCE: The home has a robust system in place to ensure that in the event of a vacancy at the home any person being considered for the place will have a full needs assessment and an evaluation of their needs prior to being offered the place. The manager stated that there has been one new admission to the home since the previous inspection by the Commission for Social Care 06/08/07. A random review of the residents files including the most recent admission provided evidence that assessment are conducted, they were in depth and provided full details of the person and their needs. EVIDENCE: The people who use the service all have individual care plans. The care plans are developed with the full involvement of individuals, relatives, key workers, care managers and all relevant healthcare professionals. Care planning is designed to fully consider the person’s individual needs and choices and to develop a plan of action to meet these needs. Care plans are reviewed regularly with more formal reviews taking place annually or as needs change, to ensure that the residents can be confident that their needs will continue to be met. The care plan sampled included details of personal care needs and individual preferences; risk-assessments, activities, diet, health care needs and religious needs. The right of the people who use the service to make decisions about how they wish to live their lives are respected. The residents are supported to make decisions in a number of ways. Each resident has an allocated key worker who is trained to offer one to one support. They know the resident well and understand their needs both individually and within the group. There are good risk management processes in place to protect individuals from harm. The manager and the key workers have developed a set of individual risk assessments for all activities and routines of daily life. This judgement has been made using available evidence including a visit to this service. People who live in the home are encouraged to be as independent as possible. They have the opportunities to take part in a range of educational, social and leisure activities. EVIDENCE: The people who use the service are supported to be as independent as possible. They are actively encouraged to make their own choices and to live their lives as they wish, so far as they are able. Records sampled indicate that religious and cultural needs are known and respected. During the site visit the residents were engaged in a variety of activities such as, listening to music, enjoying drinks, relaxing and interacting with each other and with staff and going out to collage. The manager stated that each person has their own activity plan detailing activities they enjoy, these include swimming, shopping, attending a daycentre, collage and attending clubs. There is comprehensive information of the service providing the people who live at Fengates with sufficient support to enable them to maintain contact with family and friends. The home has a visitor’s policy and staff stated that, visitors are welcome in the home at all reasonable times. The completed Annual Quality Assurance Assessment provided information that the resident’s privacy is respected at all times. During discussion with and observation of staff throughout the site visit it was evident that the staff are committed to promoting the interests of people who live in the home. Interaction observed during the visit was warm, supportive and respectful. A recommendation has been made to ensure that all staff undertakes training or a refresher course in understanding diversity and cultural awareness. The manager informed the inspectors that specialist advice had been sought to assist the home with specific dietary problems of two residents. The guidelines were clearly available and informed the reader of measures to be taken to safeguard the residents from refusing food, eating very little and over eating. Systems are in place for safe storage, handling and administration of medication. EVIDENCE: The people who live at Fengates are provided with care in a way that respects their individual wishes and preferences and guarantees their privacy and dignity. There is good evidence of routines being flexible and relevant. The care plans sampled and observation during the visit demonstrated that individuals are able to choose how to live their lives and their choices are respected. The residents are appropriately supported with their healthcare needs. Each person has an individual health action plan; this document helps staff monitor and promote peoples’ health. Records show regular visits are arranged to the local General Practitioners and everyone has a regular health check. The people who use the service are supported to access other healthcare Fengates Road (5) DS0000013510.V357593.R01.S.doc Version 5.2 Page 14 professionals where necessary, including the dentist, optician, chiropodist and physiotherapist. Where people have specialist healthcare needs, the home liaises with relevant healthcare professionals to ensure they receive the appropriate care. Individuals living at the home are protected by a set of policies and procedures for the handling of medication. Medication is stored securely and records are well maintained and up to date. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 22 & 23 were assessed during this visit. People who use the service experience a good quality outcome. This judgement has been made using available evidence including a visit to this service. The people who use this service benefit from a, clear and accessible complaints procedure, which includes timescales for the process. All required policies and procedures are in place to ensure that individuals are safeguarded from harm or abuse. EVIDENCE: The complaint procedure had been produced in a pictorial format. Records sampled indicated that there have been no complaints made since the last inspection. However issues that could be considered as a potential complaint between residents are not recorded as a complaint. The manager stated that in the event of conflict between residents they are recorded on individual files, and not raised as a complaint. It is recommended that the manager and staff record complaints on behalf of the residents to ensure that a clear and accurate picture of the home is maintained. The Commission for Social Care Inspection was informed of 7 safeguarding adult referrals made in the last 12 months, only 2 of which resulted in safeguarding adult investigation. The investigations were both results of incidents that occurred outside the service. The manager demonstrated that effective measures were actioned in order to effectively safeguard the individual concerned. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 24 & 30 were assessed during this visit. People who use the service experience a good quality outcome. This judgement has been made using available evidence including a visit to this service. The communal areas of the home provide a comfortable standard of accommodation suitable to the needs and lifestyles of the people who live there. The home was safe, clean and hygienic. EVIDENCE: A partial tour of the home was completed. During the site visit all communal rooms were seen and generally they were well furnished and effectively met the needs of the people who live at the service. However the condition of the paintwork in the communal areas was poor and in areas there was considerable scuffing and damage to the walls and doors. A recommendation was made to develop a plan of maintenance to ensure that the paintwork throughout the home remains at a good standard. At the time of the tour of the home only two residents were at home and both requested that the inspector did not access their rooms as a consequence the bedrooms were not inspected during this visit. The residents do have access to the small garden at the rear of the property where there is a patio with seating. This judgement has been made using available evidence including a visit to this service. Competent and qualified staff support the people who live in this service. Individuals are protected by the homes recruitment policy and appropriately trained staff meets their individual and group needs. EVIDENCE: People who live in the home benefit from the support of the stable and welltrained staff team; the majority of staff demonstrated a good understanding of individuals needs. Interaction observed throughout the visit was warm and respectful and staff work hard to engage with the residents consistently promoting their well being. Conversations with staff during the visit showed a high level of commitment to promoting the best interests of people who live in the home. A sample of the staff rota indicated that there are sufficient staff on duty to meet the needs of the peoples’ who live at the home. A number of staff have completed National Vocational Qualifications (level 2) or above. The manager informed the inspector that further members of staff are scheduled to complete their NVQ level 2 early in 2008, this will bring the total of NVQ trained staff to over the governments required 50 level. Each member of staff has their own training record in place and it was evident that staff have received mandatory training in safeguarding adults, fire, food handling, food hygiene manual handling, health and safety, first aid and managing medication. The staff personal files sampled were maintained in good order and contained the required information. POVA first checks are carried out and enhanced police checks are completed. During the review of the files the inspector identified gaps in the educational and employment history in all three members of staff. A recommendation has been made to ensure that all relevant information regarding anomalies found on application forms is correctly recorded on the individual’s personal file. This judgement has been made using available evidence including a visit to this service. The home is managed in the best interests of the people who live there. An effective quality assurance system is in place. People are protected through effective systems, which promote their health and safety. EVIDENCE: The people who live in the home benefit from the experience and commitment of the manager to promote their best interests. The registered manager stated in the completed Annual Quality Assurance Assessment that she gained a Registered Managers Award October 2007 and is part of the companies “Challenging Behavioural Support Team”. The home holds regular resident and staff meetings which was confirmed by staff spoken to and from records sampled Fengates Road (5) DS0000013510.V357593.R01.S.doc Version 5.2 Page 22 The completed Annual Quality Assurance Assessment states that “Service user Quality assurance questionnaires” had been recently sent out, however at the time of this visit no forms had been returned to the service. The Responsible Individual carries out monthly regulation 26 monitoring visits and in addition company representatives also complete a 3 monthly “robust inspection”. In addition to the regular in house audits undertaken by the manager and the organisation the service successfully applied for and has gained an Investors In People award on November 2007. This achievement came after a serious of audits and visits by Investors In People to ensure that the staff were appropriately supervised and that the service was promoting the appropriate training to the staff group to enable them to protect and care for the people who use the service. The completed Annual Quality Assurance Assessment indicates that the company provides a range of policies, and procedures, which have been updated with the manger bringing this information to staff attention during staff meetings. Substances hazardous to health (COSHH) were stored securely and appropriately. Health and safety checks are completed and recorded regularly some of which were sampled including fire prevention records, fridge and water temperatures. The manager provided information to indicate that records and certificates and identified systems are in place for routine service and maintenance arrangements for the environment.
