All of those who are currently in residence are older people. The premises are a large detached building. The accommodation is on four floors. There is a passenger lift that
Annual Service Review Page 2 of 7 0 9 1 0 2 0 0 8 gives step free access around the accommodation. When full, six of the bedrooms can be shared by two people each. All of the other bedrooms are singles. The bedrooms are laid out as bed sitting areas. Each has a bed, bedside table, comfortable chair and wardrobe. All of them have a private wash hand basin and one of them also has its own shower. There is also a television point in each bedroom and people can pay to have a private telephone installed. There is a main lounge that has a conservatory that joins onto it. There is another smaller, quiet lounge. There is a separate dining room. There are two bathrooms. One of these has a special rise and fall bath that is easy to access for people who have limited mobility. There is also a wet room. This has a special shower that again is easy to use. There is a call bell system. This means that people can alert members of staff if they need assistance. There are three gardens. These have both lawns and flower beds. The main one is level and there is garden furniture such as seats, a table and a parasol. The premises are set in a quiet residential street close to the centre of Upper Walmer. This is a small village about three miles from Deal. There is a general store just down the road. There is both off street and on street parking. There is a bus stop nearby. The nearest mainline train station is about half a mile way. The Registered Provider is a partnership of four people. The Registered Manager is a qualified nurse. The weekly charge for living in the Service runs from £750.00 to £890.00. Annual Service Review Page 3 of 7 Service update since the last key inspection or annual service review:
What did we do for this annual service review? We looked at all the information that we have received, or asked for, since our last Key Inspection. We completed this on 9 October 2008. The information we considered includes the following: The Annual Quality Assurance Assessment (the Assessment) that was sent to us by the Registered Provider. The AQAA is a self-assessment that focuses on how well outcomes are being met for people using the Service. It also gives us some numerical information about the Service. Information we have about how the Service runs. This includes what the Manager has told us. Information that people who live in the Service, members of staff and visiting health and social care professionals have given in questionnaires they kindly completed for us. What the Registered Provider has told us about things that have happened in the Service. These are called notifications and are a legal requirement. The previous Key Inspection Report. Relevant information from other organisations. What has this told us about the service? The Registered Provider sent us its Assessment. It is very detailed and gives us a good description of how things are going in the Service. Various improvements have been made since our last Key Inspection. The Manager has used a model called the Essence of Care to double check the quality of some of the nursing care and personal care provided in the Service. Also, the menu has been made more varied in response to suggestions from people who live in the Service. More trips out have been undertaken. These have been possible because a new easy-to-access vehicle has been purchased. Locks have been fitted to two bedroom doors in response to requests from the rooms occupants. All members of staff have undertaken training about how best to promote equality and diversity. At the previous Key Inspection, the Registered Provider was not required to undertake any particular improvements. We thought that things were going well. We gave the Service three stars. This means that it provides an excellent response to the needs of the people who live there. We did not require or recommend that the Registered Providers make any particular improvements. In their questionnaire replies, the people who live in the Service say that they continue to be happy there. They are pleased with the help they receive. They think that they have good quality meals. One person says, (the Service) feels homely and friendly, clean and comfortable. Staff are friendly and helpful. Another person says, I like Annual Service Review Page 4 of 7 everything about it, its extremely good. The Assessment says, we continue to offer a very accepting, flexible culture within the Home, enabling service users to make as many choices about their daily life as possible in the least restrictive way. People are supported to stay healthy and they are helped to seek medical attention as and when it is needed. Reliable arrangements are in place to assist people to take medicines in the manner intended by their doctors. One health and social care professional says, excellent policies are in place, patients are well fed and taken care of. Another says, a good nursing home with enthusiastic staff. Clients are well looked after. The Assessment says that the accommodation is presented to a comfortable domestic standard. In their questionnaire replies, people say that the accommodation is kept fresh and clean. The Manager says that there have not been any significant accidents in the Service since our last Key Inspection. We have not received any complaints about the Service since our last Key Inspection. The Registered Providers have received one complaint. This was about an aspect of the laundry arrangements. We understand that the matter was promptly investigated and that it has been resolved to the satisfaction of the person who made the complaint. There is always a qualified nurse on duty. In the morning they are supported by five health care assistants. In the afternoon and evening there are three health care assistants on duty. There is a waking staff presence in the Service at night. During the day there are other members of staff who do the housekeeping and who work in the kitchen. The Assessment says that suitable security checks are completed for new members of staff. These include taking up references and obtaining a police check. They are done to make sure that new members of staff are trustworthy people. The Assessment says that new nurses and health care assistants receive introductory training before they begin work. There is then ongoing training. This is designed to make sure that nurses and health care assistants have the skills and knowledge they need to support people in ways that are right for them. The records show that there are shortfalls in the fire safety training that some nurses and health care assistants have received. The Manager says that this matter will be resolved by 1 February 2010. More than half of the health care assistants have a National Vocational Qualification (NVQ) in health and social care. Sensible things are done to help keep people safe. This includes providing some people with rails on the side of their bed. This has been done to give them reassurance that they will remain safe at night. We understand that the local Department of Environmental Health says that good standards of hygiene continue to be achieved in the kitchen.
Annual Service Review Page 5 of 7 The premises are fitted with an automatic fire detection system. This provides a high level of protection. The Assessment says that fire safety equipment and other things such as the electrical wiring are regularly checked. This is done to make that they remain in good working order. Our overall judgement is that the Service continues to provide an excellent quality of care for the people who live there. What are we going to do as a result of this annual service review? We are not going to change our inspection plan. We are going to complete a further Annual Service review by 8 October 2010. However, we can visit the Service at any time.
