If yes, what have they been: Date of last key inspection: Date of last annual service review (if applicable): Brief description of the service St Michaels Cheshire Nursing and Residential Home is formed from the conversion of a former TB sanatorium, managed by the Anglican Sisterhood from 1878 until 1968. The
Annual Service Review Page 2 of 6 0 2 1 0 2 0 0 7 home now caters for Younger Adults with Physical Disabilities and is managed by The Leonard Cheshire Foundation. The home is set in large grounds on a sloping site that allows access to the grounds from each floor; many of the original features of the house have been retained. It is within walking distance of Axbridge Town where there are shops and other local facilities. Accommodation is on three floors; all bedrooms are single occupancy. The home has been well adapted for the younger service user and can accommodate self-propelled wheelchair users, thereby promoting maximum independence. The grounds are sloping and, in parts, accessible by wheelchair. There are tables and chairs for use on the patio in the warmer weather. There are two bungalows adjoining the home. These are used as by the Leonard Cheshire Foundation as transitional homes for people. These bungalows are not registered for nursing or personal care. What did we do for this annual service review? We looked at all the information that we have received, or asked for, since the last Key Inspection. This included: The Annual Quality Assurance Assessment (AQAA) was sent out to the home; this was completed in detail and returned to us. The AQAA is a self-assessment that focuses on how well outcomes are being met for people who live at St Michaels. It also gave us some numerical information about the service. Information we have about how the home has managed any complaints. What the home has told us about significant things that have happened, these are called notifications and are a legal requirement. The outcome of the last Key Inspection we carried out in October 2007. The outcome of the last Annual Service Review we carried out in October 2008. What has this told us about the service? The homes AQAA explains that they have systems in place to ensure the views of people who live in the home, and those close to them, are promoted. They have service user focus groups, regular service user meetings (supported by a facilitator), annual service user surveys, regular reviews (inviting people close to the particular service user such as their social worker, friends and family and their key worker) and making external advocacy services available. They have made many changes as a result of listening to people who live in the home; these include holding meetings which both service users and staff attend, reassessed the menus, completed work on a sensory garden, provided Aromatherapy and Reflexology, introducing a new cafe and juice bar, reassessed group activities and increased evening and weekend activities, increased the number of switches and communication aids within the home, support service users to self medicate and they have trained service users so that they are able to take part in the recruitment of staff and volunteers. In the summary of the homes AQAA some of the things they believe they do well are providing a safe environment for service users to express their needs and individuality, employing high quality and experienced staff, promoting relationships with families and friends, providing personal relationship facilitators who are trained to support service users in complex personal relationship issues and having good quality monitoring procedures to ensure the best possible outcomes for service users. There are more improvements they wish to make including; ensuring greater service user involvement in the decision making in the home, continuing to increase evening Annual Service Review Page 4 of 6 activities, to work with service users to introduce advanced care planning, improve the involvement of parents and relatives, develop a garden which is wheelchair friendly to provide service users with the facilities to grow their own plants and recruit more volunteers to support service users in activities of their own choice. There are currently 36 people who live at the home. St Michaels has 32 permanent care staff who support them; each has been provided with a thorough induction which meets best practice standards and 18 members of staff have attained a National Vocational Qualification (NVQ) at Level 2 or above. Following the last Key Inspection we made these judgements about the quality of the service provided at St Michaels: Choice of Home, Individual Needs and Choices, Lifestyle, Personal and Healthcare Support, Concerns Complaints and Protection, Environment and Staffing were all rated as good. Conduct and Management of the Home was rated as excellent. The overall rating of the home was 2 star. The last Annual Service Review we carried out confirmed the home was continuing to provide good levels of care and support; improvements were also noted in many areas. There has been one complaint made to the home in the last 12 months: this was looked into in line with the homes policy and the AQAA states it was partially upheld. The home continues to let us know about things that have happened since our last visit and they have shown that they have managed issues well. They work well with us and have shown us that their service continues to provide good outcomes for the people who live at St Michaels. What are we going to do as a result of this annual service review? We believe the home is maintaining or improving the standard of care and support we assessed during the last Key Inspection. We will therefore carry out another full inspection by 2nd October 2010. However we can inspect St Michaels at any time if we have concerns about the quality of the service or the safety of the people who live here. Annual Service Review Page 5 of 6 Reader Information
