What the care home does well The service has only been in operation since June 2009 when two people moved into the home under an extended assessment agreement. As a result of the limited length of time Tattenham Lodge has been providing care for these two individuals it is difficult to fully ascertain to effectiveness of the service. There is however detailed policies and procedures in place which if fully implemented should provide people who live at the home with safe, secure and stable environment to live in. The environment in which the residents will live in is of an exceptionally high standard, with good quality furnishings and fittings throughout the home. The quality and quantity of the communal rooms will provide good areas for the residents to engage in individual or group activities without being invasive or cramped to others who are not participating. What the care home could do better: The manager must ensure that all policies and procedures are implemented. This is to ensure that once the home is in full operation the policies and procedures will offer protection to the people who will live in the home. There must be a clear understanding of the roles and responsibilities of the manager and future staff in order to operate a care home under the requirements of the National Minimum Standards for Younger Adults. Date of last inspection Brief Description of the Service: Tattenham Lodge is a detached two-storey house, developed to provide accommodation for 6 service users with mental disorders. The home is sited in a residential road with similar properties, and is within walking distance of local shops and amenities. The home is within easy reach of the town centre of Epsom and the villages of Banstead and Tadworth. Accommodation is arranged over both floors with bedrooms being situated on the ground and first floors. There are communal bathrooms on the ground and first floors and some bedrooms have their own ensuite facilities. Communal and recreational areas are situated on the ground floor with a quiet lounge, main lounge, dining room, well equipped domestic kitchen breakfast room and a multi function conservatory looking over the garden. The garden is a well maintained and secure. There is car parking for a limited amount of cars at the front of the house. This is an overview of what the inspector found during the inspection. This inspection of the Service was an unannounced “Key Inspection”. The Inspector arrived at the Service at 09.15 and was in the Service for four and a half hours. It took into account detailed information provided by the Service’s owner or manager and any information that the Commission has received about the Service since it was registered. This is a new service registered in November 2008. The home has capacity to accommodate 6 people; at the time of the visit 2 people had moved into the home under undergoing an assessment period to ascertain the appropriateness of the home for their future care. Although the home was registered in 2008 it was considered a dormant service until June 2009 when the two individuals moved into the home for their assessments. The people living at the home have only been there since June 2009 and it was therefore difficult to gage and measure the service they were receiving during this site visit. In addition the service has not engaged any staff and had not implemented a good deal of the policies and procedures it has developed for the future needs of the home. As a result of the limited time and experiences the people have been in residence at Tattenham Lodge and the information available at the time of the site visit and quality rating for this service is 1 star. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected. We have made this judgement using a range of evidence, including a visit to this service. People who are considering moving to the home and their representatives are given limited information about the home to help them decide if the home will be suitable for them. The homes admission and assessment procedures should ensure people’s individual needs are understood and can be met. EVIDENCE: Two people have moved into the home since the service was registered last year. The registered manager stated that both individuals are still in the process of assessment to ensure that there complex needs can be effectively met by the service. There is a detailed assessment process where people who are considering moving to the home, their advocates, families and care managers are invited to the home. The home has developed some limited information about the services offered and this is available to help people decide if the home will be suitable for them. The information contained in the statement of purpose and service user guide is also limited and only contained very basic information about the home.

The manager stated that the statement of purpose and any relevant information required to assist people with understanding the services offered by the home would be further developed and introduced prior to any more people being admitted. It was evident that the initial assessments of the people who are considering moving to the home can be confident that qualified and competent people carry out their procedure. The people resident at the home under assessment both had very detailed needs assessments in place to allow for a relatively seamless move into Tattenham Lodge. However the assessments had not been developed by the service they were the existing assessments from care management and their current care facility. The manager stated that as both people under assessment and in light of their complex mental health needs the assessment process had been extended and was still developing before any future decisions could be made about their residency at Tattenham Lodge. The manager stated that wherever possible the initial assessments will takes place in the person’s current home, so the environment that is currently meeting their needs can be viewed as well, gaining valuable information from current carers and/or family. In addition to this, the person’s former care plan, risk assessments, historical information and the Care Managers assessment of needs, where available, is used to inform the assessment process and enable a planned transition to take place. We have made this judgement using a range of evidence, including a visit to this service. People who live in the home have detailed pre existing care plans and risk assessments in place from external bodies. Measures have been put in place to ensure their needs are met and they are protected form harm whilst being supported to lead fulfilling lives during their residential assessment at the home. EVIDENCE: The manager stated that the as both individuals living at the home were still in the process of assessment full care plans have not be fully developed. The home was continuing to use the multi disciplinary plans which had been developed by the care managers, mental health professionals and their current place of residence. The care plans were developed to provide a clear understanding of the needs of both individuals under assessment at the home.

DS0000072911.V376433.R01.S.doc Version 5.2 Page 11 There was detailed information on file to ensure that the people under assessment could be confident that their individual needs and preferences are understood. The manger stated that each person is assessed as an individual and their future care plan will be developed according to their individual need. There was evidence within the assessment files that each person has been involved in the planning and review of their care over the last two months of their residency. There is evidence on file that both individuals are being encouraged to make decisions according to their individual needs and abilities. There is an understanding of how to support and guide people to make appropriate decisions. Both care plans were sampled as part of the case tracking process. These documents include detailed information about each persons physical, personal and health care needs, their social skills, communication, independence, activities and daily living skills. Aims and objectives are clearly recorded. However clear guidance will have to be provided and developed to ensure that if both individuals are admitted into the home their wishes to be supported with their personal care, religion and choices they make about how they wish to spend their day. The manager stated that there will be a full review of the current information to ensure care plans, risk assessments and specific guidance for each individual will be easily accessible and user friendly. At the time of this site visit the individuals at the home are protected form harm through risk management processes. Care plans sampled include individual risk assessments, which provide guidance on how to minimise risk in regard to specific activities, it was agreed that these should be expanded to include all daily living activities. However as has been previously stated the home has not developed its own assessments and care plans. The manager stated that a full set of risk assessments would be developed once the decision has been made to admit both people into Tattenham Lodge. People who live in the home are encouraged and supported to participate in a range of activities both within the home and in the local community. They are provided with a flexible, healthy and balanced diet. EVIDENCE: There is evidence that the people living at the home are supported to take part in a variety of activities and to be a part of the local community. The manager stated that the activities are still being developed and will include the use of local facilities such as, shops, pubs, restaurants/cafes, library, and leisure centre. However the main source of activities is still based on the previous lifestyle opportunities engaged by both individuals in their previous care facilities. The manager stated that during the assessment period the service has ensured that both individuals receive continuity by continuing to attend day services in areas where their old care homes were based.

The service will endeavour to supports people to maintain appropriate relationships with their families and friends. The manager stated that where families are involved they will be invited to be involved in the care of their relatives. The service has developed a robust policy ensure the respect peoples privacy and dignity by knocking on bedroom doors before entering; calling people by their preferred names and providing personal care in the privacy of their own rooms. During this site visit the interaction observed between the manager and the individual residents was appropriate. Information provided in the AQAA informs that Equality and Diversity are promoted through the Equal opportunities Policies and Procedures. Evidence found during the site visit supports the information sent to us in the Annual Quality Assurance Assessment (AQAA) completed by the provider as part of our inspection. We have made this judgement using a range of evidence, including a visit to this service. People who live in the home should be protected from harm through the safe handling of medication. Measures are in place to ensure that individuals can be confident that their physical and health care needs will be met. EVIDENCE: Arrangements are in place to ensure that the people who live in the home can be confident they will receive the personal care and support they require. The manager stated that each person is supported to choose the time they go to bed and get up in the morning and the clothes they wish to wear. The manager provided details of who the home will promote the health needs of the people who will live there. The evidence was reviewed if the home follows the procedures the health needs should be met. There are provisions in place to ensure that each person will have an individual health action plan. The residents of Tattenham Lodge will all be registered with the local GP practice, and have access to a Dentist, Optician, and Chiropodist, and all National Health Services as required.

DS0000072911.V376433.R01.S.doc Version 5.2 Page 15 The manager continued to state that records of all appointments and health checks will be maintained and that these would be stored in the individuals care files. There were limited records available due to the shortness of the stay of the people who were living there however a review of the records demonstrated that health needs are monitored and any potential problems are identified and dealt with. There is evidence that the manager understood know who to contact in an emergency for any immediate health concerns. The home is developing appropriate links with the local community mental health team to ensure that they maintain continuity of care when individuals are admitted to the home. People who live in the home are protected through the safe handling of medication. At the time of this site visit the home was using medication and medication sheet that came with the individuals from the care homes they were fully resident. However the medication was being appropriately stored and administered. The manager stated that he was the only person who was currently involved in the process of medication handling, but when staff are employed the provisions for medication training and handling would be implemented to ensure that the people will be safeguarded. The records of medication administered to the people living at the home have been well kept to ensure there is no mishandling. Provisions are in place to ensure that people who live in the home will be listened to and protected from abuse. EVIDENCE: The Commission has not received any complaints about this care home since registration. The people who were living in the home stated that they were listened too and the manager recognised when they were unhappy and responded appropriately. One person who lives in the home stated during the site visit that he would know who to talk to if he was unhappy about anything. The home has a robust procedure for concerns complaints and protection. The manager stated that the complaints procedure is explained to people who are living in the home. People are protected from abuse through policies, procedures and provisions are in place for future staff training. We have made this judgement using a range of evidence, including a visit to this service. People who live in the home benefit from the excellent facilities and the safe, homely and comfortable accommodation. EVIDENCE: People who live in the home benefit from the spacious, comfortable and safe environment. A full tour of the premises was undertaken, all areas were bright and airy, and the premises were clean and well maintained throughout. The two individuals living in the home were delighted to assist in the tour of the home and to proudly take the inspector round their own bedrooms. The bedrooms were highly individual, the manager stated that where ever possible people moving into the home can personalise their own living environment. The property is detached and the accommodation is on two floors, accessed by stairs.

We have made this judgement using a range of evidence, including a visit to this service. Some arrangements have been made to ensure that the people who will use this service will be safeguarded by appropriate policies and procedures. EVIDENCE: At the time of this site visit the home had not employed any staff. The manager stated that when the people who are currently under assessment are placed at the home, staff would be appointed to support their needs. There was evidence that the manager has introduced the process of identifying and selection potential staff for the home. The home has a set of policies and procedures in place. The policies and procedures however should be reviewed and updated prior to any staff being appointed to ensure that they fully comply with current employment legislation and the national minimum standards of the care standards act for younger adults. A discussion with the manager highlighted the need for the home to obtain further clarification in the employment of care staff and specifically the

We have made this judgement using a range of evidence, including a visit to this service. People who will live in the home should be confident the home is run in their best interests by a well qualified and experienced manager. There arrangements are in place to promote the health and safety of people who live in the home. EVIDENCE: Arrangements have been made to ensure that the people who will live at the home could be confident the home is run in their best interests. The manager has relevant qualifications and is experienced in caring for people with the individual and complex needs. Tattenham Lodge DS0000072911.V376433.R01.S.doc Version 5.2 Page 22 There is a quality assurance (QA) procedure in place, to ensure that the people who live in the home and their families or any other professional involved in their care will have the opportunity to comment on the home. At the time of this site visit the only people resident at the home were those under assessment and had only been there for a very limited period therefore no QA had been undertaken. There was evidence found during the site visit, which supports the information sent to us in the Annual Quality Assurance Assessment (AQAA) completed by the manager. This document also shows there is a development plan to further improve services to people who live in the home. People are protected from harm through safe working practices. The manager stated that he understood the need for staff to undertake all the mandatory training as required and that these would be actioned when staff were in post. Information provided in the AQAA and submitted during the registration process and supported by the manager during the site visit shows that safety checks of all equipment and installations have been undertaken. However during the site visit no documentary evidence of these checks could be found and the manager was unsure where the information was stored. This was discussed with the manager and it was agreed that all information pertaining to the health and safety of the residents and staff of the home must be retained on site at all times in compliance with the national minimum standards for younger adults.
