Conditions of registration: Date of last inspection 2nd March 2006 Brief Description of the Service: 41 Beach Road is a care home registered to accommodate 1 young adult with mental health problems aged 18 to 65 years. The resident has lived with the owners and their family since the age of 10 years. The resident lives with the owners as a member of the family and refers to them as his mum and dad. The home is run as a normal domestic household and not a residential care home. The home is situated in Cleveleys close to the town centre and is convenient for all local amenities. The home has a Statement of Purpose and Service User Guide providing information about the care provided, the qualifications and experience of the owners and the services residents can expect if they choose to live at the home. A copy of the Service User Guide and most recent inspection report would be issued to any prospective resident and their relatives/representatives to help them make an informed choice whether to move into the home. The fees being charged to the resident living at the home are £450.00 per week covering all aspects of care, food, entertainment and accommodation. This is an overview of what the inspector found during the inspection. This unannounced site visit was undertaken as part of the homes Key Inspection. The site visit commenced at 9.30am and took place over 2 hours. The Inspector spoke to the homes owners and the resident living at the home. Care and maintenance records were examined and a full tour of the premises was undertaken with the resident. At the time this site visit was being undertaken the owners of the home had applied to the local social services department to become Adult Placement Carers as apposed to being registered as a care home and their application was being dealt with. What the service does well:
The resident at this home is well looked after. He lives in a normal family environment as a member of the homeowners family and is involved in decision making about the running of the home and his own lifestyle. The resident confirmed routines were very flexible and he could make his own decisions on a day-to-day basis and didn’t feel restricted by the homeowners. The resident said, “ I have a really great room and I like to spend some time on my own listening to music and watching television. My privacy is always respected”. Discussion with the resident confirmed he is very happy living at the home and is encouraged to pursue an independent lifestyle with a minimal level of supervision and guidance being provided by the homeowners. This included visiting relatives in Ireland on his own. Observation of the care plan confirmed clear risk assessment strategies had been put in place to minimize any potential risks to his personal safety. The resident said he had really enjoyed the experience. The resident said since the last inspection he had been to Ireland three times to visit relatives of the homeowners and had also been to Las Vegas on the annual family holiday. The resident was able to describe his involvement in the planning and preparation of the holiday. The resident said, “ We have some really great holidays and I really enjoyed Las Vegas although it was very hot”. The resident is encouraged to pursue personal interests and has recently completed a computer course at night school. The resident also continues to work as a volunteer in a local charity shop, which he says he enjoys very much.

Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 2 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The admission and assessment procedures were clear to ensure the care needs of residents are met. EVIDENCE: There have been no new residents admitted to the home since the last inspection. The existing resident had full assessment information in place including his religious/cultural, relationship and nutritional needs. Observation of practice and discussion with the resident confirmed he was happy that his needs were being met. The Commission considers Standards 6, 7 and 9 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 6,7 and 9 Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. The resident is supported to pursue an independent lifestyle with identified risks monitored and reviewed. EVIDENCE: An Individual record was kept for the resident with a plan of care setting out in detail the action that needed to be taken to ensure all aspects of his health, personal and social care needs were met. Significant events had been recorded and daily entries made setting out the care given. The care plan clearly described the level of support and assistance being provided to the resident with his decision-making. Discussion with the resident confirmed he is involved with decision making within the home as a normal family member and his views are listened to. The homes owners had documented on the care plan discussions about family holidays and the

The resident said he was happy that he is kept informed about developments within the home and was able to have his say. The resident confirmed he was being supported and encouraged to live independently with the knowledge that support and assistance is available if this is required. The resident said he has been encouraged to attend an evening class on computers, which he really enjoyed. The owner of the home said they were encouraging the resident to widen his social circle and pursue a more independent lifestyle. Observation of the care plan and discussion with the resident confirmed he continues to undertake voluntary work four days a week at local charity shops. The resident said, “I like working as I get to meet lots of different people”. Observation of care plan records confirmed the resident is encouraged to take responsible risks with the support of the homeowners. These have included minor tasks like using public transport to flying to Ireland to visit family on his own. The resident was able to describe his level of involvement in planning the trip to Ireland and the steps taken to minimize any potential risks to his personal safety. The resident said, “I was assisted to Liverpool airport and then the airline staff made sure I got on the right plane. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,15,16 and 17 Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. Education and employment opportunities are promoted to ensure the resident has opportunities for personal development. EVIDENCE: The home encourages the resident to pursue education and employment opportunities and these are clearly described within the care plan. The resident said he had just finished attending an evening class on computers and wanted to take further training in this area. The resident also said he had attended a course on digital cameras and had enjoyed this so much he wants to join another course. The owner of the home said they are encouraging the resident to widen his social circle and pursue a more independent lifestyle. Observation of the care plan and discussion with the resident confirmed he continues to undertake voluntary work four days a week at a local charity shop. The resident said, “I really enjoy working at the shop and have recently increased my days from three to four. The work is really interesting and I have lots of fun”.

The resident said, “I can receive visits from my friends any time and they always made welcome”. Discussion with the resident confirmed he was happy with the routines within the home and these were being arranged around his individual needs. The resident said he was provided with the choice of spending time on his own or in the lounge areas and the owners of the home respected his privacy. Discussion with the resident confirmed he did not have any special dietary needs and enjoyed the food made available to him. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 18,19 and 20 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Promotion of health is taken seriously and personal support is provided in a flexible and sensitive manner. EVIDENCE: Discussion with the resident confirmed he was happy with the level of support he receives within his daily routines and could make his own decisions when to bath, retire to bed, get up in the morning and have his meals. The resident said he was able to attend to his own care and personal hygiene needs and was happy with the level of supervision and guidance available to him and this was provided in a sensitive dignified way. At the time of this visit the resident was in good health and wasn’t taking any prescribed medication. The Commission considers Standards 22, and 23 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 22 and 23 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Arrangements for complaints are handled well and taken seriously ensuring the resident feels listened to. EVIDENCE: The home has a detailed complaints procedure, which has been made available to the resident. The resident said he was happy living at the home and had never had cause to make a formal complaint. The home has a procedure in place for dealing with allegations of abuse. The Commission considers Standards 24, and 30 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 24,26 and 30 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home has a planned maintenance and renewal programme for the redecoration and refurbishment of the home to ensure residents live in a comfortable, homely and safe environment. EVIDENCE: The premises are in keeping with the local community and have been decorated and furnished in the style of a normal household and provide comfortable and bright accommodation for the resident. Since the last inspection the homes owners have continued to make improvements to the environment with new carpets being fitted in the front and rear lounges. The owner said the refurbishment of the home was ongoing and there were further plans for refurbishment. The resident was very happy with the improvements being made.

The resident said he were very happy with his room and could spend as much time there as he wanted without being disturbed by the owners. The resident said, “ I love my room. I like listening to music and watching television”. This judgement has been made using available evidence including a visit to this service. The home is well managed and run in the best interests of the resident. EVIDENCE: The owner has many years experience in working with people with a learning disability and has achieved a BA Honours in Learning Disabilities. Records seen confirmed the owner has access to training to ensure his knowledge and skills are updated. As discussed previously in this report the resident has lived with the owners of the home since he was 10 yrs old and lives as a member of the family. The home is run as a normal family household and quality assurance systems are

Observation of his care plan and discussion with the resident confirmed he is involved in every day family decision-making and his views are listened to. The resident was able to describe how decisions are made about family holidays and his involvement in these. The residents care plan had recorded significant events confirming he is encouraged to pursue leisure and educational interests and make his own decisions about his lifestyle. Inspection of maintenance records confirmed facilities and equipment was being maintained as required by health and safety legislation to provide a safe environment for the resident to live.
