Service Users in the category Mental Disorder (MD) must be aged 50 years or over. 9th September 2005 Date of last inspection Brief Description of the Service: Abbots Lawn is a care home with nursing, registered with the Commission for Social Care Inspection to provide care for up to thirty-seven people in the category MD/E (Mental disorder elderly) and DE (Dementia). The establishment is situated in a quiet residential road in Bognor Regis, close to the sea front. It has recently been extended and upgraded. This is an overview of what the inspector found during the inspection. This unannounced inspection was carried out on 05 06 06. Preparation for the inspection included a review of the documentation on file, a review of the service history and complaints. Information supplied in the pre inspection questionnaire sent to the Commission was also read. The manager was available who facilitated the inspection and made available any documentation requested. Residents and staff were spoken to and their comments are considered in this report. Residents able to express an opinion stated that they are treated with kindness. A visiting district nurse was spoken to and she expressed satisfaction with the manner in which the home is run and the sped of referrals to her service. The home was inspected against the National Minimum Standards and the majority were met in full. There are no outstanding requirements from a recent environmental inspection. This is a well run home, run in the best interests of the residents. The fees charged are £411 to £520 What the service does well: What has improved since the last inspection?

Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1. Evidence of assessment of need carried out prior to admission, to ensure that the home will be able to meet the service users needs was seen in care plans inspected. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, The residents health, personal and social care needs are set out in an individual plan of care. 8, Residents health care needs are fully met. 9, Residents, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. 10, Residents feel they are treated with respect and their right to privacy is upheld. Quality in this outcome area is good. This judgement has been made from evidence gathered both during and before the visit to this service EVIDENCE: Three residents were case tracked. The care plans were inspected and were found to be comprehensive showing practices in place to meet the health and personal care needs of each service user. Regular review of the care plans was also evident. A district nurse visiting one of these residents confirmed that the care offered in the home is good and that her opinion is sought as necessary and without delay. Policies and procedures are in place to ensure the safe handling of medicines. Medicines are stored correctly and medicine

Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. This judgement has been made from evidence gathered both during and before the visit to this service EVIDENCE: There is an activities coordinator in the home Monday to Friday and a range of activities are offered including karaoke, crafts, musical entertainment, exercises to music and mini bus trips. Two residents have been taken on holiday. Visitors are welcome in the home and this was evidenced in relative surveys seen. Problems experienced by one relative who had cause to complain were discussed with the manager and a resolution has been achieve. The cook was spoken to and menus were seen which offer choice. Pureed diets are served appropriately. It was noted that food was stored in the fridge without being labelled and dated. This was discussed with the cook and the

The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16, Residents and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. 18, Residents are protected from abuse Quality in this outcome area is good. This judgement has been made from evidence gathered both during and before the visit to this service EVIDENCE: Policies and procedures are in place on the protection of adults and the registered manager confirmed that staff attended training. There is a complaints procedure in place. Staff spoken to confirmed that they would know the procedure to follow in the event of suspected abuse although, it appears in practice that the registered manager is called to deal with allegations and complaints. A visiting District Nurse stated that she never saw anything of concern in the home but if she did the she would be able to discuss with the registered manager. Restraint was discussed with the rm and another staff member. There is a policy in place and bed rails are used only following risk assessment. The issue of mentally frail Residents climbing over them and being at increased risk was discussed. The registered manager investigated a recent complaint and as the complainant was unhappy with the outcome The Commission decided it should be discussed further today. A satisfactory outcome has been achieved.

Sufficient cleaning and laundry staff are employed to achieve this. The home has recently been extensively refurbished and remains in good decorative order. There are suitable well-decorated lounges and a dining room for residents use. Resident’s rooms are comfortably furnished and personalised. Bedroom doors have locks. Grab rails are in place and sufficient wheelchairs for residents use. Communal shampoos were in use in the bathrooms and this was discussed with the registered manager who stated that the practice will cease. This judgement has been made from evidence gathered both during and before the visit to this service EVIDENCE: Staff rota supplied to the commission show adequate numbers of staff on duty. Staffing today was not as listed due to sickness and the deputy was called in for the duration of the inspection. Staff were observed interacting appropriately with residents. Three staff files were inspected and had all relevant documentation in place. Staff training is in place to ensure the health and safety of residents. The home has recently received the investors in people award. Six monthly questionnaires are in place and there is annual audit of this information. Suitable accounting procedures are in place and monies are held securely. Funds are held for three Residents in a communal account and the inappropriateness of this was discussed with the registered manager. She discussed the difficulties experienced in attempting to open accounts for these residents. Suitable training, policies and procedures are in place to ensure the health, safety and wellbeing of Residents and staff.
