This inspection was carried out on 9th September 2005.

What the care home does well

Abbots Lawn offers the residents who live there, good care, good food and many activities, should they wish to join in. All of the people who were spoken to on the day of inspection responded favourably with regard to the care on offer at Abbots Lawn. The home has recently undergone a total refurbishment, and a large lounge/sunroom has been added to the front of the building.The majority of the staff members at Abbots Lawn have worked for Ashton Care a long time, and the Manager informed the Inspector that they do not use agency workers, ensuring that the residents of Abbots Lawn always know their carers.

What has improved since the last inspection?

At the previous inspection there was an issue regarding the provision of footplates on wheelchairs, and the need for monthly reviews. At this inspection it was seen that all the wheelchairs had footplates, and care plans were being reviewed regularly.

What the care home could do better:

Everyone who the Inspector spoke to said very positive things about Abbots Lawn, and said that they could think of nothing that Abbots Lawn could do better with regard to their care, the food or the entertainment.

The establishment is situated in a quiet residential road in Bognor Regis, close to the sea front. It has recently been extended and upgraded. Mrs S Newman privately owns the service, and is also the registered Manager. This was an announced inspection, the second of a minimum of two statutory visits that an inspector must make to each care home during the course of a year. The first inspection, which was unannounced, was undertaken earlier in the year. At this inspection the Inspector looked at standards that were not assessed during the previous inspection, as well as any outstanding issues from the last report. The Manager was present throughout the inspection. During this inspection the Inspector toured the building, visited every room, and joined some residents for lunch. As well as talking to Mrs. Newman, the registered Manager, the Inspector spoke to residents, plus two visitors. Staff members were also spoken to on the day of inspection, about how they find working at Abbots Lawn. All the people that the Inspector spoke to were very positive about the home and told the Inspector how much they liked living, working and visiting there. The Inspector examined records of care being provided to residents; as well as records of any accidents, issues, concerns or complaints, to make sure that the residents at Abbots Lawn are being taken care of. Prior to this inspection, residents, their relatives and visitors to the home, were offered the opportunity to complete a short questionnaire where they could make their views on Abbots Lawn known to the Commission for Social Care Inspection. Many people responded, although some residents only answered some of the questions, with all but one stating that they were satisfied with the care provided by the home. At this inspection Abbots Lawn was audited against the National Minimum Standards for Older Persons. All of the elements in each of the standards assessed were met, one was exceeded. The Inspector would like to thank everyone who cooperated with her on the day of this inspection. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 and 6 The Manager informed the Inspector that prior to moving into Abbots Lawn prospective residents and their representatives are given sufficient information about what the home offers, to enable them to make a choice as to whether or not they want to live there. In addition before moving into the home, people are assessed, by a member of staff, in their own home whenever possible, to make sure they are suitable, and invited for a trial period. In that way people know exactly what they can expect from Abbots Lawn. Abbots Lawn does not offer intermediate care. EVIDENCE: Due to their mental frailty the residents spoken to on the day of inspection were not able to tell the Inspector whether or not they had been given any information about the home, but a visitor did confirm that he had received information all about the home, and that his wife’s needs had been assessed prior to her being admitted. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 9 and 10 Due to the mental frailty of the people who live at Abbots Lawn, none of the residents are able to administer their own medication. Residents are treated with respect, and their privacy is upheld. EVIDENCE: Records of drug administration are maintained, and the correct procedures, relating to medication, are being adhered to. Mrs. Newman confirmed to the Inspector that staff members administering medication had all received the appropriate training. Locks are provided for residents who, following a risk assessment, are able to use them. One resident told the Inspector that the staff members treat her “like a lady”. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 and 15 The resident’s health, personal and social care needs are set out in an individual plan of care. Residents are encouraged to make decisions about how they live their lives, with assistance as needed. Food is home cooked, with a choice offered. Abbots Lawn has a varied activities programme. EVIDENCE: One visitor spoken to on the day of inspection told the Inspector that they visited the home every week, and that the person they visit was very settled at the home. The Manager informed the Inspector that the home offers a choice of menu, with people able to eat either with other residents in the dining room, or in their own room, or the lounge, should they prefer. On the day of the inspection, the Inspector shared a meal with two residents, who told the Inspector that the food was very tasty, there was plenty of it, and confirmed that there was a always a choice. During the afternoon of the inspection two activity organisers entertained the residents with singing, and in some cases dancing. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18 Policies and procedures in the home, plus training for all staff members regarding abuse, ensure that, as far as is possible, the people who live at Abbots Lawn are protected from bad practice. EVIDENCE: Visitors told the Inspector that should they have any concerns at all with regard to how the person they visit, or any other resident in the home, is being treated, then they would talk to whoever was on duty. Staff members confirmed that they were aware of how to address any Adult Protection issues. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22 On the day of inspection Abbots Lawn appeared clean and well maintained, and the location of the home is suitable for the residents who live there. The grounds to the front of the property are secure to enable the residents to access them safely. The home was seen to provide a lift, aids, and assisted bathing facilities. EVIDENCE: At the previous inspection there was an issue regarding the provision of footplates on wheelchairs. At this inspection it was seen that all the wheelchairs had footplates. The home has recently been refurbished throughout, and people told the Inspector that they were really pleased with the result. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 29 and 30 The Manager informed the Inspector that the number of staff employed at any time always exceeds the required number, and that staff training, including NVQ, is ongoing and all training undertaken is recorded on staff member’s files. EVIDENCE: The duty rotas indicated that enough staff are on duty over the 24 hours period to meet the needs of the residents. Staff members told the Inspector that they enjoyed working with the type of people who live at the home. Many of the staff have worked for Mrs. Newman for a long time, and a staff member spoken to on the day of inspection told the Inspector that felt “well supported” by her. People visiting the home on the day of inspection told the Inspector that there always appeared to be enough people on duty to assist the residents when they needed it. The Manager informed the Inspector that they do not use agency workers at Abbots Lawn, ensuring that the residents always know the person who is taking care of them. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 35 Mrs. Newman, the registered Manager, is a qualified R.M.N, and experienced in working with people with mental health issues. EVIDENCE: The inspector saw that insurance cover is in place to meet the National Minimum Standards. The Manager informed the Inspector that that all residents have family or a solicitor or an advocate to take care of all their financial dealings.
