The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

Service users were, without exception, complimentary about the care received and the kindness and consideration of the staff. The home provides comfortable and homely accommodation. The atmosphere in the home is relaxed and staff were observed to interact with service users in a professional and caring manner. The ongoing financial investment in the service by the registered provider demonstrates a proactive commitment to the welfare of the service users.

What has improved since the last inspection?

There is clear evidence of ongoing expenditure and improvements to the home, an example being the new laundry area and machines, and the landscaped courtyard to the rear of the building.

What the care home could do better:

There are a number of requirements and recommendations, which are primarily of a managerial or `systems` nature, one requirement and one recommendation has been carried over from the previous inspection.

It is a listed property with purpose built extension situated near the centre of Liskeard in a quiet residential area. Accommodation is provided in 25 single and 2 double rooms, all rooms have television points, and if required telephones can be installed at the service users expense. Service users requiring nursing care are provided with ground floor accommodation. A stair lift provides access to the first floor. There is a sheltered courtyard garden/patio at the rear of the home, providing a sitting area for the service users. At the front of the home is some off road parking, with a car park to the rear of the property. The home has a Registered Manager, Mrs Julie Weale who is in charge of the day to day management of the home. Mr & Mrs Stratton took over the home last year as the new owners, and they visit the home on a regular basis. A qualified nurse is on duty at all times and additional health care is arranged as required e.g. dentistry, chiropody. Social and entertainment activities are provided. This was an unannounced inspection and the inspector’s first visit to this home. Overall impressions were good. The home displayed an unhurried and calm approach to daily life, which was reflected in the comments of the service users. Service users confirmed that they would feel able to approach and speak with staff. The registered manager and the deputy manager were cooperative and informative throughout the inspection. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 3, 6 Although service users generally have the information they need, an up-todate Statement of Purpose and Service User Guide are lacking. All prospective service users are assessed prior to moving into the home. EVIDENCE: The registered providers are in the process of reviewing and developing the Statement of Purpose and Service User Guide – this process is nearing completion. The new document will be distributed to all service users in the near future and a copy sent to the Commission for Social Care Inspection. Care needs assessments are in place, but these are not as comprehensive as they could be due to the use of a tick system rather than explanatory comments about the service user’s care needs. The home does not provide intermediate care. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 The lifestyle offered at the home meets the social/recreational expectations of the service users. EVIDENCE: The registered manager confirmed that service users are free to determine their own waking day, and this was borne out by the comments of the service users. Service users are able to choose an alternative meal should they so desire, and are able to choose whether or not to participate in the social/recreational activities offered. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16, 17, 18 Service users are protected from abuse, and are able to confidently voice any complaints. EVIDENCE: The home has a Complaints Policy, which is clearly displayed in the entrance hall and the statement of purpose and service users guide. The Commission for Social Care Inspection has received no complaints since the last inspection. A complaints book is kept. The service users are entered on the electoral roll and some participated in the last general election. Mail is distributed unopened to the relevant service user, or where necessary, it is opened with the relevant service user. A Protection of Vulnerable Adults policy is in place. This should be complimented by a Protection of Vulnerable Adults procedure, which provides clear instruction to staff as to the steps to take in the event of an allegation of abuse (including contact details for the relevant agencies). Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 23, 25, 26 The home is clean and well maintained, providing service users with a safe and comfortable environment. Service user rooms were seen to be personalised and comments received from service users in respect of the home were positive. EVIDENCE: The registered provider employs a maintenance person who carries out the ongoing maintenance and upgrading of the home. There is an ongoing upgrade of all the vanity units in individual service users bedrooms. There is a sheltered patio/courtyard area where service users can sit out, which the current owners have recently upgraded. Service users own rooms suit their needs. Service users spoken with during the inspection commented that they were happy with the accommodation offered and were able to enjoy a comfortable lifestyle. Beech Lawn Nursing & Residential Home D52-D04 55098 Beech Lawn V243092 060905 stage 4.doc Version 1.40 Page 13 The Registered Provider has commenced a programme of installing hot water thermostats throughout the home (total immersion areas, such as baths, have already been completed). As discussed, this programme must be determined by the findings of risk-assessments that take into account the individual service user and the service user’s room. The home appeared clean, hygienic and free from offensive odours throughout. There were suitable hand washing facilities for both staff and service users, with plentiful supplies of liquid soap and paper towels. Disposable gloves and aprons were provided in strategic places throughout the home. Cleaning staff were evident during the inspection. The laundry facilities have been moved to a larger and fully fitted laundry room. The home employs separate laundry staff. The laundry is separate from food preparation areas. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 28, 29, 30 Care is provided by experienced and competent staff, and service users are protected by the home’s recruitment procedures. EVIDENCE: Aside from the registered manager, one member of staff has completed NVQ Level 4 training. Twelve staff have completed NVQ Level 2 training, and there are currently eight staff undertaking this training. There are 25 care staff, of which 13 have achieved NVQ level 2 or above, approximately 55 . Three staff personnel files were inspected and seen to demonstrate a robust employment procedure in operation. The registered manager should keep a record of interview. An induction programme is in place for new staff, but the registered manager must implement an induction programme that complies with the National Training Organisation (www.topss-england.net - now called ‘Skills For Care’). The registered manager should ensure that all staff have a copy of the General Social Care Council Handbook (www.gscc.org.uk). Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33, 34, 36, 38 The home is run in the best interests of service users. Staff are supervised. EVIDENCE: Feedback is actively sought from service users and their relatives/representatives about the services provided to them. This is undertaken through service user questionnaires, which can be submitted anonymously, and service user and relative meetings. The meetings are listed on the notice board with minutes taken and distributed to all who attended the meeting. The outcome from the service users surveys are discussed at the management and staff meetings. The registered manager should publish a summary of the findings of the annual quality assurance questionnaires (e.g. in the Service User Guide) and should extend the questionnaires to staff and other professionals who have input to the home. The Inspector identified the need to ensure compliance with the requirements of the Data Protection Act. The registered manager undertook to ensure that the supervision diary displays planned supervisions and two initials when supervision has occurred. The home displays health and safety posters, the contact details of the health and safety executive and environmental health department should be displayed. All staff receive training relating to health and safety, including moving and handling and fire training.
