This inspection was carried out on 11th August 2009.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Staff at the home care for people with diabetes well. Registered nurses have the relevant training and experience to deal with complex diabetes. Communication with local GP surgeries is very good. Dietary provision at the home is good. Low sugar deserts are provided and low sugar and low fat varieties of the main menu are provided to ensure people have a healthy balanced diet. Staff at the home recognise that people like to have occasional `treats` and can accommodate this in the management of diabetes. Staff are also good at caring for people with complex wounds. Staff at the home access specialist help when required from the wound care specialist nurse at Derriford hospital. The home have a dedicated link nurse who is responsible for educating the nursing team of any changes or trends in research based practice. The introduction of a new robust wound care management system has improved record keeping and helps to show what care has been provided.

What the care home could do better:

Although no regulations were breached at this time a discussion was held regarding how the service can learn from the issues raised. A discussion was also held regarding the usefulness of performing spot checks and audits on how quickly staff respond to call bells and how this relates to peoples perception of how long they wait.The Provider gave assurances that he would look at ways to improve the complaint document process.

St James Lodge is a large Georgian property that has access from the main road and offers limited parking space within the front entrance. The property has been extended several years ago to provide further bedrooms on the ground floor. The home is arranged over two floors, the first floor being accessed by stairs or a five-person passenger lift. There are several separate communal areas on the ground floor and a well-maintained garden and patio area to the rear and side of the property. St James Lodge is one of two homes owned by Mr Sutherland in the Plymouth area. It is situated in the Stoke area of Plymouth and is close to local amenities. It is also on a bus route that goes direct into Plymouth City Centre. Care Homes for Older People Page 3 of 8 What we found: We performed this unannounced inspection as part of the complaints, concerns and allegations process at the Care Quality Commission. We looked at the personal care of people with specific interest into the care of wounds and diabetes. We spent three hours at the home on Tuesday 11th August 2009. We looked at one specific plan of care. We looked at complaint investigation notes, care plans, medication records and other relevant records. We visited the kitchen and looked at dietary provision for people with diabetes. We spoke with the Provider, manager, administrator and the chef. Although no regulations were breached on this occasion, we did note issues about: Lack of clarity of the recording and the perceptions of how long people wait for call bells to be answered, Unanswered questions about a dressing not being changed on one occasion and the effectivness of complaint letter writing. We noted that since the initial complaint, staff at the home have introduced a more robust method to record and monitor the treatment of wounds.
