Date of last inspection 23rd June 2005 Brief Description of the Service: Shrublands Centre comprises of a Domiciliary Care Agency, a day centre for the elderly and a small residential home registered for 5 older persons. The contents of this report relates to the Residential Home. The home is family owned and managed. The registered manager is Lucy Dallimore but the dayto-day management is being undertaken by Nigel Collins. The home is located in a rural location some 5 miles from the City of Oxford. The home is able to arrange transport for those residents wishing to attend appointments or visit family and friends. The inspector was present when the homes oldest resident who is 104 was asked her opinion of the home. She was able to be very positive about her time in the home and the quality of the care provided. Residents are encouraged to take part in the day-to-day running of the home and to take part in any activities it provides. It does however respect the wishes of residents who do not want to participate. Previous feedback from residents, relatives and friends has been very positive about the home and its staff. Phase one of the proposed extension to separate the day care and residential provision have been completed and approved. The second phase will commence shortly. This is an overview of what the inspector found during the inspection. This was a follow up unannounced inspection, which took place on a weekday afternoon. The inspector was made welcome in the home. The inspector toured the building and gained feedback from one resident. Core documentation relating to residents care was inspected. Staff files and allied documentation was inspected and a random check done on medication and its recording. The inspector was satisfied with the quality and content of this documentation. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10,11. Documentary evidence was found that the health and personal care of residents is being met. EVIDENCE: The inspector examined care plans and associated documentation for all residents. The inspector observed care being given in a dignified and private manner with the addition of gentleness and humour. All documentation inspected was found to be satisfactory and all standards met. Recommendations made at the last inspection regarding the recording of medication have now been implemented. The inspector would recommend that all care staff administering medication have a sample copy of their initials next to their name. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14,15, The home provides a good standard of daily life and an appropriate range of social activities. EVIDENCE: On the day of the inspection a communion service was taking place in a residents room to which other residents were free to join in the home should be commended for this initiative. All standards within this section were assessed as met at the last inspection. Evidence was found through documentation and observation that this remains the case. JUDGEMENT – we looked at outcomes for the following standard(s): 16,17,18, The home has appropriate complaints and protection procedures, which are known to residents and those visiting the home. Service users rights are being protected. EVIDENCE: In line with a recommendation from the last inspection the home have updated its complaints procedure. This standard is now met. All other standards within this section were assessed as met at the last inspection no changes have been made to alter this assessment. EVIDENCE: All standards within this section were assessed as met at the last inspection. Evidence was found through documentation and observation that this remains the case. The home recruits and trains staff in an appropriate manner. EVIDENCE: All standards within this section were assessed as met at the last inspection. Evidence was found through reading all staff files and observation that this remains the case. All staff currently employed are undergoing some form of vocational training. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,32,33,34,35,36,37,38. The home is run and managed in a professional manner. The organisation has the appropriate documentation to assist its managers and staff to fulfil their duties in an appropriate manner.
