What the care home does well

The service is a small family run home and as such is able to offer a very personalised service in a family environment. The home has above average staff to service user ratio`s The registered manager is a trainer, ensuring staff receive regular training relative to their job. Service users are consulted as to how the service should be delivered to them. The home is flexible in the way in which it delivers care to its service users The home has the services of a qualified nurse twice a week and physiotherapist five days per week.

What has improved since the last inspection?

Menus have been reprinted using a larger print Improvements have been made to the management and administration of medication.

What the care home could do better:

Date of last inspection 6th March 2007 Brief Description of the Service: The Shrublands Centre is a family owned and managed residential home for older people. There is a registered manager but a new application has been submitted as the registered manager does not have the day-to-day management responsibility for the home. The home is located in a rural location some 5 miles from the City of Oxford. All rooms are single, two of which have en suite facilities. There are two-day rooms on the ground floor for service users use. There are pleasant gardens and an outdoor swimming pool for residents use during the summer months. The home is well supported by local GP’s and community nurses. The home is able to arrange transport for those residents wishing to attend appointments or visit family and friends. Residents are encouraged to take part in the day-to-day running of the home and to take part in any activities it provides. It does however respect the wishes of residents who do not want to participate. Previous feedback from residents, relatives and friends has been very positive about the home and its staff. Information about the service offered can be requested from the home if required. The Commission has, since the 1st April 2006, developed the way it undertakes its inspection of care services. This inspection of the service was an unannounced “Key Inspection”. It was a thorough look at how well the service is doing. It took into account detailed information provided by the service’s manager, and any information that the CSCI has received about the service since the last inspection. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 1,2,3,4,5,6. Quality in this outcome area is good This judgement has been made using available evidence including a visit to this service. Residents are encouraged to visit the home on many occasions prior to admission. EVIDENCE: The home provides prospective service users and their families with a Statement of Purpose and a Service User Guide. These documents contain core information as to what the home is able to offer and what is not. The inspector was pleased to note that the information pack also contained a pen picture of all the staff who work at Shrublands. It details who it can care for and who it cannot. A copy of the homes terms and conditions of residence state what is expected from the prospective resident and what will happen if The Shrublands Centre DS0000013185.V351566.R01.S.doc Version 5.2 Page 9 there is a problem on either side. A copy of the written contract and the Statement of Purpose are given to the resident on acceptance to the home. A full assessment of the prospective service users is undertaken prior to admission and where appropriate interested professionals are included in this assessment. Service user are encouraged to visit and stay at the home prior to admission to establish suitability on both sides. Service users are encouraged to talk to those who live there, read the inspection report and experience the care first hand prior to making a decision. The home does not provide an intermediate care provision. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 8,9,10,11. Quality in this outcome area is good This judgement has been made using available evidence including a visit to this service. The healthcare needs of the homes residents are being met. EVIDENCE: The inspector viewed all seven residents’ care plans. Information contained in the care plans was presented in a way which would enable carers to be fully informed as to the care needs of the residents. Plans were presented in plain English and were accurate and up to date. Information was written in a very friendly and informal fashion and displayed an in depth knowledge of the individuals needs and wants. The home operates a key-worker system whereby care is provided by the same group of people. The home has a good relationship with the local healthcare practice. The manager highlighted a recent change to the healthcare provision for its residents. Errors were found in the recording of medication at the last inspection. The inspector requested the medication charts and inspected the medication on arrival at the home. The home has taken on board the requirement and recommendations made at the last inspection. Procedures have been tightened up and the inspector is pleased to report no errors were found at this inspection. One resident has full control of his medication. All others need varying degrees of assistance. The inspector witnessed residents being treated with respect and dignity Evidence was found at the inspection that the wishes of residents following serious illness or death are being recorded and acted upon. Three residents had passed away recently and evidence was found that this was handled with sensitivity and with the wishes of the resident implemented. Some Residents comments: “ I have a room which can take my wheelchair and have access to the garden “ “ Lots of vegetables and healthy meals “ “ It’s not easy to find activities which will suit people with very diverse needs but they do regularly arrange activities of an appropriate nature “ The Shrublands Centre DS0000013185.V351566.R01.S.doc Version 5.2 Page 12 Daily Life and Social Activities The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14,14,15. Quality in this outcome area is good This judgement has been made using available evidence including a visit to this service. The home provides residents with activities and interests which meets their needs. EVIDENCE: Service users spoken to and those who filled in the questionnaires felt that there were enough activities to keep residents busy stimulated and healthy. On the day of the unannounced inspection a keep fit session was taking place. This was being done at the residents pace and was a very popular activity which all residents enjoyed. Evidence was found at inspection that residents are encouraged to have family friends and visitors to the home with the home having very few rules with regard to visiting times. Service user care plans evidenced that residents are given as much choice and control over their lives, as they are willing or able to accept. Residents were seen to be asked if they would like what was on offer today and if not an alternative would be found. On the day of the inspection the main meal was of steak and kidney pie, which was cooked from fresh with fresh vegetables. One resident said, “ I am a very fussy eater and the home know this and try hard to give me things I like (choice is always available)“ The Shrublands Centre DS0000013185.V351566.R01.S.doc Version 5.2 Page 14 Complaints and Protection The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Quality in this outcome area is good This judgement has been made using available evidence including a visit to this service. The relatively small size of the home enables complaints to be dealt with at source in most cases. EVIDENCE: All residents spoken to or who filled in a questionnaire were aware of how and who to make a complaint to. The home try to deal with any dissatisfaction at its source and as such no formal complaints have been recorded. A recommendation made at the last inspection was for the home to record minor concerns. The have set up a system for this to happen but as yet nothing has been felt to need recording. Feedback from service users and their representatives indicated that, where there is cause for complaint or dissatisfaction, the home has formal processes for it to deal with them. The homes complaints procedure was viewed and found to be satisfactory. The inspector was shown letters of complement All but three staff have had Alert to Abuse training. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 19,20,21,22,23,24,25,26. Quality in this outcome area is good This judgement has been made using available evidence including a visit to this service. The home provides a safe comfortable environment for its residents. EVIDENCE: The house is a large detached property set back from the main road between Oxford and Swindon. The home has large grounds with an open-air swimming pool. All residents have their own room some have en-suite facilities. The home has obtained planning permission to undertake building work, which will result in the addition of extra en-suite bedrooms. Residents will be consulted as to the best way to achieve this with minimum disruption to the life of the home. The home has sufficient toilet and washing facilities to meet the needs of its residents, with all rooms having their own en-suite and kitchen facilities. The home provides adaptations for those who need them without making the home feel institutional. A random inspection of residents’ bedrooms showed that residents are encouraged to have their own possessions around them. Residents can bring small items of furniture by prior arrangement with the manager. The overall atmosphere of the home is one of a homely atmosphere and a staff group who enjoy what they do. On the day of the inspection the home was found to be clean and fresh. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29,30. Quality in this outcome area is good This judgement has been made using available evidence including a visit to this service. Staff are provided in sufficient numbers and with appropriate experience to meet the needs of its residents. EVIDENCE: Staff files inspected contained the required information. Staff are supervised and supported informally on a regular basis. However formal staff supervision is not taking place the required four times per year. Staff spoken to felt supported and training was said to receive a high priority. Staff spoken to indicated that they felt that Shrublands was a good place to work and that they enjoyed their job. The home is recruiting staff in sufficient numbers to meet the needs of its residents. Positions are advertised, interviews take place, references and other checks take place to ensure the suitability of its staff. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 31,32,33,34,35,36,37,38. Quality in this outcome area is good This judgement has been made using available evidence including a visit to this service. The home is managed in a professional manner. EVIDENCE: The inspection process gave weight to evidence that this home is well run and managed. The manager is pending being registered and is experienced and trained to manage the service. The home is family run and therefore benefits from the collective ethos, leadership and management approach of a family unit. The family take a very active part in the running and management of the home. This is seen as a positive part of what the home has to offer. The home has sound financial and accounting procedures. The accounts were not inspected as part of this inspection process. Service users are protected from financial abuse and exploitation by the home having robust policy and procedures where staff and service users are informed of what action the organisation will take in this event. Records policies and procedures inspected were accurate and up to date and recorded and stored in a way which ensures confidentiality is maintained and privacy and dignity is respected. Measures are in place to ensure that the health well being and safety of its residents are safeguarded and protected. From the evidence seen by the inspector and comments received, the inspector considers that this service would be able to provide a service to meet the needs of individuals of various religious, racial or cultural needs.
