Conditions of registration: None Date of last inspection 15.03.05 Brief Description of the Service: Windward is located on the outskirts of the village of Stoke Fleming 2 miles from Dartmouth. There are several country lanes leading from the home, which can provide wheel chair walks for Service Users and their relatives. The home has a driveway leading to parking for several cars. The home is registered to provide care to Service Users requiring personal care with nursing. The home overlooks countryside and has sea views from the lounge and some Service Users rooms. A large patio area off the lounge also overlooks the sea. The home currently provides 25 beds; some rooms have wheel chair accessible ensuite facilities. All rooms have a nurse call bell and television point. A passenger lift is available to the first floor. There is a variety of equipment to support physically disabled Service Users needs and specialist equipment is available following an assessment of need. There is an assisted Parker bath; two assisted shower rooms are also available on the ground floor. A registered nurse is on duty at all times supported by a team of Health care assistants, administration, catering, domestic and maintenance staff. This is an overview of what the inspector found during the inspection. The inspection was announced. It took place over 5 hours and 50 minutes (10am until 3.50 pm) and was conducted with the matron and her deputy. The owner also made himself available during the inspection. A completed pre inspection questionnaire and completed service user and visitor/ relative comments cards returned prior to the inspection were also used to write this report. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 Comprehensive pre admission assessments are made and therefore people are admitted to the home appropriately. EVIDENCE: The matron explained that she (or the deputy) normally sees documentation about the joint assessment carried out by social services before carrying out a pre admission assessment visit to the prospective client in their current environment. The information gathered helps them to make a decision as to whether the home can meet a persons needs and helps to inform the initial care plan should they move in. Privately funded clients also have to have a pre admission assessment. Funded nursing care assessments are carried out soon after admission. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 9 and 10. The health care (in relation to medication) needs of the residents are identified and met. Interactions between staff and residents promotes and protects patients privacy and dignity. EVIDENCE: The procedures for receipt, recording, storage, handling, administration and disposal (especially in light of the change to waste management regulations in April 2005) examined and discussed during the inspection are all carried out in accordance with laid down legislation. Service User comment cards completed (5) indicated that patients feel well cared for and safe and have their privacy respected. A tour of the home confirmed that staff treat the patients and visitors with respect and dignity and interact with them appropriately. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13, 14 and 15. The staff show a good understanding of the patients cultural, social and recreational needs and the importance of maintaining contact with family and friends and engage the patients in a variety of ways. The meals in the home are excellent offering both choice and variety and catering for special dietary needs. EVIDENCE: A range of activities are available to suit all preferences and abilities. An activities co-ordinator is employed for 30 hours a week ensuring that a variety of activities takes place within the home and grounds (weather permitting). Outings and trips are arranged on a more individual basis. During the inspection clients were seen reading the newspaper, walking in the grounds, watching different TV channels and listening to the radio. Visitors were seen coming and going during the inspection and can see their relative/friend either in their own room or in the communal areas. Documentation discussed confirms that clients can see who they wish to or not and are encouraged to handle their own financial affairs for as long as they wish to or are able to. Some rooms seen during the inspection were personalised with clients own furniture and belongings.

The cook is well known to the clients and will make sure all requests for meals /food likes are met. Drinks and snacks are available 24 hours a day. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18 Clients in the home are protected by staff having a knowledge and understanding of Adult protection issues. EVIDENCE: The policies and procedures around adult protection are up to date and include a whistle-blowing policy. The training records indicate that most staff have attended training in ‘adult protection’ including whistle-blowing. This is carried out by an external trainer. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20,21 and 26. The standard of the environment within the home and the grounds is good providing clients with attractive and homely place to live. The overall quality of the furniture and fittings is good providing a safe environment. The home is clean and hygienic. EVIDENCE: The location and layout of the home is suitable for its stated purpose as adaptations to the building have been made over the years to make more room for large equipment and provision of some en-suite facilities. A tour of the home confirmed that all of the communal areas and the gardens are accessible to clients. There are 2 lounges and dining space on the ground floor. The furniture and decoration in the communal areas is of good quality and domestic in character. The building complies with the local fire service and environmental health department requirements.

There are a variety of bathrooms and toilets throughout the home adapted to meet the needs of the clients. Domestic staff were seen cleaning the home during the inspection. The home was tidy, clean. The intended outcomes for Standards 27 – 30 are: 27. 28. 29. 30. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 and 30. Clients needs are met by a satisfactory mix of nurses, carers and ancillary staff who are trained and competent to do their job. EVIDENCE: The duty rota provided with the pre inspection questionnaire details the number of staff on duty and during the inspection people looked well cared for. The pre inspection questionnaire indicates that staff turnover is low. All 5 of the completed relatives/visitors comment cards returned prior to the inspection indicated that they feel that there are ‘sufficient staff on duty at all times’, and all 5 of the completed Service users comments cards returned prior to the inspection indicated they felt ‘well cared for’. Domestic, catering and administrative staff are employed to support the care staff in meeting the needs of the patients. Training records examined were up to date and covered a variety of subjects including all statutory training, PEG management, diabetes, care of the dying, subcutaneous infusion and catheter management. Future planned training includes continuation of the NVQ programme. Both the matron and the deputy are studying for their NVQ 4 (registered managers award). All staff have an induction period when first recruited and have a minimum of 3 days paid training per year. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 37 and 38. There is good leadership and guidance for staff to ensure residents receive consistent quality care. The systems in place in the home promote and protect the health and safety and welfare of the clients and staff. EVIDENCE: The registered manager is a first level nurse and has several years experience of managing a care home with nursing. She will soon complete her Registered Managers Award. She demonstrated a clear leadership style during the inspection and there are clear lines of accountability within the home. The matron has the key to the one containing staff files. Windows on the first floor were restricted. Some portable electrical tests (PAT) were being carried out on the day of the inspection. The fire-log book and accident book were examined and were found to be up to date. The hot water temperature within the home is better regulated than previously and ‘hot water’ signs are displayed where the temperature remains hot. Risk assessments carried out for use of bed rails on clients is comprehensive and details the risks involved with fitting such items.
