Conditions of registration: Date of last inspection 22nd September 2005 Brief Description of the Service: Windward is located on the outskirts of the village of Stoke Fleming 2 miles from Dartmouth. There are several country lanes leading from the home, which can provide wheel chair walks for Service Users and their relatives. The home has a driveway leading to parking for several cars. The home is registered to provide care to Service Users requiring personal care with nursing. The home overlooks countryside and has sea views from the lounge and some Service Users rooms. A large patio area off the lounge also overlooks the sea. The home currently provides 25 beds; some rooms have wheel chair accessible en-suite facilities. All rooms have a nurse call bell and television point. A passenger lift is available to the first floor. There is a variety of equipment to support physically disabled Service Users needs and specialist equipment is available following an assessment of need. There is an assisted Parker bath; two assisted shower rooms are also available on the ground floor. A registered nurse is on duty at all times supported by a team of Health care assistants, administration, catering, domestic and maintenance staff. This is an overview of what the inspector found during the inspection. The unannounced inspection took place over 3 hours and 40 minutes (11.20 am – 3pm) and was conducted with the deputy matron, with the assistance of the administrator. A variety of documentation was examined and a tour of the home took place. Patients were observed during the inspection but not spoken to individually. The focus of the inspection was to assess the core standards not assessed during the last inspection. What the service does well: What has improved since the last inspection? Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7&8 The health care and personal needs of the residents are identified and met. EVIDENCE: The plans of care examined included general information about the patient, a detailed daily statement about their wellbeing, nutritional assessments, tissue viability assessments and manual handling assessments. They detailed the actions to be to be taken by care staff (day and night separately) to ensure health/ nursing, personal and social needs of the patient are met. The plans had been reviewed as required. Carers complete a ‘shower book’ daily with information about the care given to a patient during their shift this is used to inform the care plan. The nurses have also recently used the Liverpool Care Pathway assessment tool when providing palliative care to a patient. A tour of the home confirmed that equipment necessary for the promotion of tissue viability and the prevention or treatment of pressure sores was available throughout the home and in sufficient quantity for the size of the home. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. Service users’ legal rights are protected. Service users are protected from abuse. JUDGEMENT – we looked at outcomes for the following standard(s): 16 The home has a satisfactory complaints procedure. EVIDENCE: The complaints procedure is displayed on the wall by the front door. The deputy matron could not find the complaints file but said that there had been no complaints since the last inspection. She said that any issues are raised with her or the matron, as they see the patients and relatives regularly, these are then acted upon immediately and resolved. She said that issues are also discussed during the daily staff handover so that all staff are aware if there are any concerns or problems to be aware of. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 (partially) The overall quality of the furniture and fittings is satisfactory providing a safe environment. EVIDENCE: A tour of the home confirmed that the environment had been improved - one of the first floor corridors has had a new carpet fitted and at least one room has had a new carpet. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 29 The homes recruitment policy and practice protect the residents. EVIDENCE: The deputy matron said that there is a standard procedure which includes getting 2 written references, one of which must be from the last employer, and a CRB check. The staff recruitment policy/checklist, request for reference form and the application form examined confirmed this. She said the matron interviews potential staff using a standard set of questions. The home has some overseas staff and the recruitment practices were discussed with the administrator, who was also able to produce relevant documentation relating to their employment. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33 and 35 There is good leadership and guidance for staff to ensure residents receive consistent quality care. The home regularly reviews aspects of its performance through a programme of self review and consultation, which includes seeking the views of the residents, relatives and staff. The systems in place in the home promote and protect the welfare of the clients and staff. EVIDENCE: The matron and the deputy matron who are both level 1 registered nurses have both achieved a level 4 NVQ registered managers award. During the inspection it was clear that are clear lines of accountability within the home and with external management.

Any issues raised in the surveys are dealt with accordingly on an individual basis. The staff, patients and relatives are able to approach the matron (or any other staff) if there are any concerns knowing they will be taken seriously. The home has a family atmosphere and the staff and patients enjoy this quality and so strive to maintain it. Patients who are able to manage their own finances are encouraged and facilitated to do so. If money is kept by the home for any patient they have an individual form with the current balance on this is recorded any incoming or outgoing money. Receipts are kept as proof of the transaction.
