What the care home does well The home held clear records for staff members employed by the home. This included obtaining all relevant information to ensure people living in the home are protected as much as possible.The owners of Amberley House provide a well maintained and excellent environment to meet people`s needs. What the care home could do better: All medication recorded and administrated correctly would protect people living at the home. The Home is situated close to Plympton

The Home is registered to provide accommodation with personal care for up to 39 people over the age of 65 of both sexes and for reason of old age, dementia, mental disorder and physical disability. The Home also offers respite care for up to 3 people at anyone time. Accommodation is provided over 3 floors and consists of 39 single rooms with the majority being greater that 12sq.m. and with en suite facilities. The Home has 4 lounge rooms and 3 dining rooms. A shaft lift provides access to the first and second floors. A day centre is attached to the Home, having its own facilities and being run separately. The home has a very pleasant landscaped garden and a parking area. The current weekly fees range from 278.40 pounds to 375. pounds. Items not included in the weekly fee are chiropody treatment, hairdressing, newspapers and magazines and personal toiletries. We visited Amberley House on the 15th May 2010 at 11:20am. Mrs Lisa Webb the Registered Manager was available to assist throughout this Random Inspection. The purpose of this inspection was to review the records and hold discussions with the last two people admitted to the home and to include a medication review of these two admissions. Also to review the records and hold discussion with the last two staff members to be appointment, review any controlled drugs held, a brief tour of the premises and to review any notification and accident recorded. Two files were examined for people living in the home. Each file held comprehensive details on each person ensuring all staff were fully aware of the needs for individuals. The files examined held records for each person on any medication prescribed, mental health issues and how staff will manage personal care and health care for each person. Several people living in the home were spoken too during this visit. All spoke very highly of the staff in the home and also stated that the food offered was very good. With one commenting, Its very good and another saying, Staff after lovely. The medication was examined for each of the last two admissions to the home and the Controlled Drugs system was also checked. The Registered Manager confirmed that senior staff members have overall responsibility for the medication held in the home and this includes ordering, returns and administration of all medications. The manager confirmed that all senior staff completes regular training on medication and have been employed at the home for a number of years. On examination of the medication held for the last two admissions and the Controlled Drugs system it was found that the home had serious shortfalls in their medication procedures including, recording of the number of controlled drugs held, amount administered and when and poor record keeping on daily medications administered. During the inspection of the Controlled Drug administration system it showed major errors in the recording, administration and storage of the Controlled drugs. Records showed that senior staff members had not signed for controlled drugs after administration, not checking the number held and on several occasions no signature recorded were two signatures were required. There were errors in the administration of a pain relieve for one person, were it showed recorded a different prescribed time from the pharmacist and what the home were administrating. The Registered Manager confirmed that the doses had been confirmed by a relative. The records show that on several occasions this dose had been administered at inconsistent times. It is Required that the home checks with the GP to confirm the correct time for this medication to be administered. Other errors were also noted including missing MARS sheets (medication records sheets) therefore showing one persons daily medication had not been signed for a number of days and poor recording practices. We recommended to the manager that all handwritten amendments to the medication

Due to the poor practices noted during the inspection and the observation of poor recording practices the issues were further discussed with the Registered Manager to ensure all people living in the home are protected from poor medication practices. It was a Requirement that the home send the Commission a detailed plan on how these errors would be avoided in future and what procedures they have put in place to ensure all staff follow safe practices on medication administration and recording. Errors in the administration, recording and storage of medication in particular the controlled drugs places people receiving medication at risk. We found that Amberley House held clear records for the staff files of the last two staff employed by the home. This included a completed enhanced CRB (Criminal Record Bureau) check, checking of documents including proof of address and a full recruitment practices. Evidence on the staff members files examined showed that they had completed, or in the process of completing a staff induction programme. Other training including Liverpool Care Pathways and End of Life Care training and booked training for the future included Continence Course, a Health and Safety and Infection Control course. One newly employed staff member was spoken with and agreed that the management of the home were approachable and that they had been welcomed into the staff team. This staff member confirmed they had completed an Induction and shadowed experience staff members for a two week period from their start date. A brief tour of the premises and discussion with some people living in the home confirmed that the home is regularly updated and maintained to a high standard. The manager confirmed that many parts of the home are upgraded when needed. The accident book was examined and showed that all accidents recorded were also followed up in individuals daily records with medical intervention if required either by the GP or local hospital. Surveys received and completed by relatives for some of the people living in the home recorded, We are happy with all aspects of care with another saying, Amberley strive to improve standards all the time. One other survey from a relative said, The methods used to make mother happy and feel valued are excellent. One staff member wrote in their survey returned to the Commission, Staff training up to date and another stating, More equipment needed for moving and handling the residents. However the manager confirmed that the home has several mobile hoists and would discuss this issue at the next planned staff meeting to ensure all staff knows how to use this equipment.
