The White House is a care home providing personal care and accommodation for thirty four older people. Eleven of the thirty four beds are available to service users with a dementia or a mental disorder. The home provides a respite care service in a dedicated bedroom through a booking service which is well used. The home is a large premises situated on a hill overlooking Bodmin town to the Gilbert Memorial and the surrounding countryside. The premises has a garden on the West and North side of the home. Car parking is available in the grounds of the home. The building is a two-storey Victorian house, which has been extensively modernised including a three storey extension. Internally, the house has been adapted whilst retaining the characteristics and age of the building. Accommodation includes two separate dining rooms and two lounges, which are available on the ground floor as are some bedrooms. All bedrooms are for single occupancy use. A total of nineteen bedrooms on all floors of the home have ensuite facilities. Access between floors is aided by a passenger lift and a stairlift. What the care home does well Each person has a care plan in place. These include some risk assessments and some care plans. Daily records are maintained and these are detailed and comprehensive. What the care home could do better: We looked at care plans for three people who have specific care needs. The care plans have some risk assessments and some monitoring tools to identify if peoples needs and level of dependency have changed. However, not all areas of assessed and identified risk have a assessment in place. Because these risks have not been assessed not all areas of need have an associated plan of care. This is needed to ensure that staff working at the home have a clear plan to follow to ensure that all areas of assessed risk and need are met. The areas identified at this time as needing a specific care plan included, people who have a high risk of falls, mobility needs and assistance with personal care. To not include a specific plan for each area may mean that staff do not know the detail of care needed and there is a risk that care needs may not all be met. The registered manager must ensure that all areas of risk have a risk assessment in place and where needed a plan of care to meet that risk safely. We looked at the daily records for all three care plans. These daily records were detailed and comprehensive. They highlighted the care given and identified changes in peoples needs. These needs and changes need to be added to the persons care plan to ensure that the care plan is current. The detail provided for staff must be clear and specific to include how and when people would be monitored, how records will be kept and who to report changes too. Furthermore, this will mean that when care plans are reviewed the information in them informs the reviewer of the most current health and welfare of the person and the current care needs. To have accurate records also provides a clear audit trail of decisions made about care needs and any changes made. The registered manager must ensure that each persons care plan includes a plan of care for all areas of assessed need and is detailed and person cenetered. We observed from reviewing records that Medication Administration Records which are hand transcribed are not signed and dated by two people. The registered manager is recommended to ensure that all hand transcribed medication records are signed by two staff to ensure that there is no risk inaccurate transcribing. For the purpose of this inspection the term We will be used when referring to the Care Quality Commission. The random inspection is part of a review as a result of a safeguarding concern raised. The purpose of this inspection was to review the risk assessment and care planning process used by the service to inform staff how to manage peoples care needs. We looked at care records for the three people and discussed care records and management practice with the registered manager .
