Tolverth House is a registered home providing accommodation and personal care for up to 14 older people aged 65 years or over. Of these up to 4 may have dementia and a further 4 may have mental health care needs. The home is situated in the village of Long Rock on the outskirts of Penzance. It is close to shops and public transport routes and is set in its own grounds, slightly off the main road. It has two floors, the upper floor being accessible by stairs with a stair lift. What the care home does well This inspection focused on the environment only. The care home was clean and tidy throughout with no odours. New furnishings have been purchased. A maintenance programme is in place to ensure that the homes decoration and furnishings are kept to a satisfactory standard. Improvements to the garden area are in process. What the care home could do better: No statutory requirements or recommendations were identified at this inspection. The registered manager is aware that they are restricted to make some physical improvements to the home due to the age and layout of the property. However the registered manager has agreed to discuss with the registered provider the possibility of installing two en suite rooms in the home which will then remove toilets from bedrooms and will keep the Commission updated regarding this. Additional charges are made for newspapers, hairdressing, private chiropody and outings organised by the home. 2 1 1 0 2 0 0 8 Care Homes for Older People Page 3 of 8 What we found: An inspector visited Tolverth house care home on the 18 May 2010 following concerns we received regarding the environment of the home, in particular the condition of peoples bedrooms and toileting facilities. No concerns were raised in respect of the care that people receive at the home and therefore this was not inspected. I arrived at the home unannounced at 10.00am and the inspection lasted for one and three quarter hours. I met with the registered manager, Mrs Burnett and observed staff and people using the service. Thirteen people were resident during this inspection. Two care staff, cook, domestic, maintenance and administration staffs were on duty. The registered manager was in the home on her day off catching up on paperwork. From a tour of the home I observed the home to be clean throughout with no odours present. I inspected some of the people who use the service bedrooms. The registered manager told me that in some of the rooms where there was a shower cubicle, as this was not being used they have removed them and replaced them with a toilet and hand basin. The consequence of this is that a toilet is in the bedroom with no partition walls but a shower curtain for screening has been erected. The registered manager agreed that this is not ideal but she felt that this was a better option than having a commode in the room. We discussed the possibility of future building works in that some rooms could have the possibility of an en suite provision which she agreed to discuss with the registered provider. The registered manager was aware of redecoration and maintenance works that need to be carried out and has a list of works written up to be completed. She is also aware of furnishings that need replacing/repair and has recently purchased some new beds. Some of the bedrooms had vinyl wood effect flooring. The registered manager was aware that if a person wished to have carpet this option should be considered and only in light of a risk assessment indicating that this is not in a persons best interest should it be declined. The registered manager confirmed that in the persons statement of terms and conditions the room that the person will occupy is recorded. I recommended that the home should ensure that they gain the persons view as to the room they will be occupying and if they are satisfied with the accommodation. If they are not, then what practical and realistic adjustments could be or not be made. The registered manager told me that they are currently installing a sluice, improving the garden, and undertaking redecoration when practical. There are telephone points in the majority of rooms for people who use the service to use. The registered manager stated there remains sufficient numbers of bathrooms for the people who use the service. What the care home does well: This inspection focused on the environment only.
